Description of Requirements for the items to be acquired (salient characteristics/specifications):

1. Delivery and acceptance are F.O.B Destination (FAR 52.247-34) at the Veterans Affairs Medical Center, located at 1898 Fort Road, Sheridan WY 82801-8320
1. 52.212-1, Instructions to Offerors--Commercial ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL
1. Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable.
1. Contractor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM’s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.
1. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA’s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
1. Any award made as a result of this solicitation will be made on an All or Nothing Basis.
1. State if quoted items are available and priced through contractors existing Government-wide Acquisition Contract (GWAC), GSA Multiple Award Schedule (MAS) contract, or VA-wide Acquisition Contract.
1. Contractors socioeconomic status must be verified in the VA Vendor Information Pages (VIP): https://www.vip.vetbiz.gov/. Unverified contractors will be considered non-responsive.
1. Contractors are warned against contacting any VA personnel other than the Contracting Officer and Contract Specialist prior to placement of any award made resulting from this RFQ. If such contact occurs and found to be prejudicial to competing contactors, the contractor making such contact may be excluded from award considerations.
1. All proprietary information shall be clearly and properly marked. If the Offeror submits annexes, documentation, attachments or the like, not specifically required by this solicitation, such will count against the Offeror’s page limitations unless otherwise indicated in the specific volume instructions below.
1. All information shall be confined to the appropriate file. The contractor shall confine submissions to essential matters, sufficient to define the quote and provide an adequate basis for evaluation. Contractors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each quote. Any award made resulting from this solicitation will be made based on the best overall quote that is determined to be the best overall value to the Government, considering (I) Price.
1. Respondents to this announcement shall submit their quote on Contractors Letterhead in accordance with FAR 52.212-1. Submission of quote shall include the following volumes: (I) Price.

Price - The offeror shall complete the pricing schedule provided in Section (v) above: Price/Cost Schedule.

1. 52.212-2, Evaluation--Commercial Items ADDENDUM to FAR 52.212-2 EVALUATION—COMMERCIAL ITEMS:
2. The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers:
Price – The Government will evaluate offers by adding all line item prices - the Total of All CLINS will be that sum. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offers that do not meet or exceed the technical capability requirements of the statement of work shall not be selected regardless of price.
2. All Offerors are advised that, in the interest of efficiency, the Government reserves the right to conduct the review in the most effective manner. Specifically, the Government may first sort the quotes of all Offerors by price from lowest price to the highest. Thereafter, the Government will review the Technical Capability of the lowest priced quote only. If the lowest priced Offeror’s Technical Capability is determined to be acceptable and meets the requirements, the Government may make award to that Offeror without further reviews of the remaining Offerors’ Technical Capability. If the lowest priced Offeror’s Technical Capability is determined to not meet the requirement, then the Government may review the Technical Capability next lowest priced quote, and so forth and so on, until the Government reaches a quote that is determined to meet the Technical Capability requirements.

Statement of Work Data Innovations Instrument Manager Core Software Ver. 8.15 Service Contract
I. Overview The Contractor for the Sheridan VA Medical Center in Sheridan, Wyoming shall provide the Sheridan VAHCS with a Software Support Solution on the Data Innovations Instrument Manager Core Software Ver. 8.15, and HL7 outbound reports.
(1) Contractor must be an authorized service representative for the Data Innovations Instrument Manager Core Software Ver. 8.15 Workstation
(2) Contractor is responsible for Helpdesk Support – Contractor will provide technical telephone support and assistance seven (7) days per week, twenty-four (24) hours per day for the Supported Software. The Customer will receive a response from Contractor within four (4) hours of Contractor’s receipt of the Customer’s request for support or repair of Supported Software, during normal business hours.
(3) Contractor is responsible for Remote Support – Contractor will provide remote technical support for Supported Software during the hours of 7:00 A.M. (EST) to 8:00P.M. (EST), Monday through Friday. Remote technical support will be provided over a secure Internet connection in most cases.
(4) Contractor is responsible On-Site Support - On-site support will be performed during normal business hours. In the event that contractor determines that support and maintenance cannot be resolved with Remote Support. Contractor is responsible for all labor and travel expenses to provide all services covered, including during the term of the agreement. Contractor shall provide weekday on-site technical representative maintenance, troubleshooting, and repair coverage from 8:00am – 5:00pm MST., Monday thru Friday (excluding holidays). Preferred rates for labor and travel outside the principle period of coverage will be required. Federal Holidays observed by the Sheridan VA Healthcare Systems are:

(5) Contractor is responsible for all Operating System Software updates and Hardware Reliability updates. Contractor may be able to provide remote diagnostic capabilities.
(6) The contractor shall furnish a malfunction incident report to the institution upon completion of each repair call. The report shall include, at a minimum, the following:
a) Date and time notified;
b) Date and time of arrival;
c) Serial number, type, and model number(s) of equipment;
d) Time spent for repair;
e) Description of malfunction and repair; and
f) Proof of repair that includes documentation of a sample run with acceptable performance.
(7) During the term of the Service Contract, should the repair record of any individual piece of equipment covered reflect a downtime of 4% or greater of the normal working days in one calendar month, a determination will be made by the designated representative of the Government at the Sheridan VAHCS to replace the initial equipment with new equipment. The responsibility of maintaining the equipment furnished in good condition in accordance to the manufacturer’s instruction, shall be solely that of the contractor. All equipment covered in this Service Contract shall maintain an uptime of 96% in each month of the term of the agreement.
(8) The contractor shall treat each notification for an emergency/repair service call from the Government as a separate and new service call.
(9) A full system inspection shall be performed prior to execution of the agreement. Any service determined necessary from the inspection must be completed prior to execution of the agreement upon which a system site number will be assigned.
(10) The service contract agreement will run during the term of the contract for one (1) year, followed by four (4) option years, with the ability to cancel with 60 days notice by the Government.
a) All licenses, inspections, adjustments, calibrations, and certifications must be current at all times. VA shall be provided copies of these documents within ten days of award and within ten days of inspection/adjustments/calibrations/change of personnel after award.
