Brand Name Justification IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Zeiss

FORUM Ophthalmic image and data management system software for clinical display groupings for Glaucoma, Retina, and Cataract cases.

3. Unique characteristics that limit availability to only one manufacturer, with the reason no other manufacturer can be used: The software system and related equipment will be integrated into an existing system. The new software system must be compatible with existing equipment. The alternative would be a costly replacement of the entire system. The existing system is utilized VA-wide so replacing it would not be cost-effective or logical. The existing system is called VistA Imaging. Each month, VistA Imaging serves more than 1.3 million patients, captures more than 39 million new images, displays 9-11 million images for review, and displays about 550,000 studies for diagnosis and is used to interpret more than 160,000 studies.

The new software integration will allow for raw data to be compared across ophthalmic instruments. The data will be accessible in any office. Currently, clinicians work in different locations. With this new software integration, they will be able to review test result data on the same day from different locations. This will result in increased efficiency and will positively impact patient care and treatment

The software has the capability to show 3D images of structures and can manipulate the gathered raw data to give a better understanding of the disease process when explaining it to the patients.

The medical center currently uses Zeiss instruments. If other software were purchased, it would not be compatible with the existing Zeiss instruments.

4. Description of market research conducted and results or statement why it was not conducted:

Market Research was conducted by searching the Vendor Information Pages (VIP) database and the

Federal Procurement Data System (FPDS). Market Research indicates that this requirement is available from at least two SDVOSB concerns.

5. Requirements Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.

6. Approvals:

a. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
