The purpose of this amendment is to provide answers to questions and a revised Statement of Work. A time extension was provided with Amendment 0001. An additional time extension is not granted with this Amendment 0002. All responses to this Request for Quotations is due by 12PM CDT on 08/19/2022.

Questions and Answers
1. Can they grant a time extension?
Answer: A time extension has been executed with Amendment 0001 issued on 08/12/2022.
2. Since MV SWGR will be decommissioned can this be removed from RFI?
Answer: Waco MV switches testing was removed from the Statement of Work (SOW).
Temple MV switches only will need to be tested, not include relay.
3. Can ground grid test be removed?
Answer: Ground resistance testing at Temple and Waco must include in this job. It will not be removed from SOW.
4. Provide shut down procedure/form?
Answer: Contractor responsibility to submit the shutdown/testing plan and procedures after the job is awarded the job.
5. Provide IRCA for Temple and Waco locations?
Answer: Since it is outside the hospital, it is IRCA minimum requirement.
6. How many buildings will be shut down at a time?
Answer: Coordinate with COR for each building testing. Every outage is convenance of the Government schedule.
7. Can we get in writing what buildings will be omitted? Waco site had several buildings that we did not look at because of this.
Answer: Please see attached revised Statement of Work. There are added breakers and omitted breakers.
8. Provide as built drawings for Temple and Waco locations.
Answer: Could not provide the as built drawings. Breakers can be seen at the panels or switchboards at Temple and Waco.
9. Provide updated SOW for Temple and Waco locations?
Answer: Please see attached revised word document. There are addition breakers and omitted breakers.
10. Provide site visit sign in sheet?
Answer: Provided.
11. Since MV equipment is being decommissioned, can it be omitted form the SOW?
Answer: Please see attached revised Statement of Work. There will be addition breakers and omitted breakers.
12. Are relays expected to be tested if the MV equipment are not omitted?
Answer: Temple MV relay testing will be omitted. Waco MV breaker testing was omitted.
13. Are just the XFMR’s to be tested or just DGA to be performed?
Answer: Dissolved Gas Analysis test to all transformers at Temple and Waco. Fill N2 gas at all transformers to have positive at transformers.
14. Work to be completed will have to be done on weekends and after hours, can the period of performance be extended to accommodate this?
Answer: Work will be done weekends (Temple and Waco) and nights (may also be at Waco). Coordinate with COR after the job is awarded. The job will be performed during cold weather since AC at the buildings will be down during the testing. Time frame to start will be November time frame.
15. During the site walk it was noted that some of the original equipment counts where incorrect and will need to be adjusted. Can the due date be extended to accommodate these changes?
Answer: Please see attached revised word document. There are addition breakers and omitted breakers at Temple and Waco.

SCOPE OF WORK
Requirements
Requesting a Contractor be responsible for providing Maintenance, Calibration, Inspection, and Testing of Switchgears, Breakers and Transformers and Dissolved Gas Analysis for Temple and Waco locations.
The Contractor shall provide all labor and materials for the services required. Work will be done at the Central Texas Veterans Health Care Facility in Temple TX and Waco, TX. After all work is completed, the contractor will have a walkthrough with the Project Manager (PM) or VA COR to ensure all work is completed and ready to receive payment.
The Contractor shall provide and install any required materials, equipment, labor, and trades for the completion of this project. The Government will not provide any materials, equipment, labor, or trades for this project. It is the sole responsibility of the Contractor to meet the requirements of this project without any assistance from the Government. The Government will provide access and information only for this project.
The Contractor shall contact the VA COR 15 days ahead of starting work.
The Contractor shall provide a work schedule/hour for pre-approval by the VA COR that includes a detailed list of work to be done for each requirement and the sequence in which it will be accomplished.
The schedule shall be in the form of a progress chart of suitable scale to indicate the work scheduled for completion by any given date during the work. The work plan shall indicate time frames and programmed power shut-off, if applicable. Contractor shall highlight on the schedule any off-hours and overtime work requirements.
The contractor shall provide all skilled personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained. A site visit will be coordinated for Contractors quoting and all are encouraged to be present to verify measurements and investigate site conditions prior to submitting quotes. All work must comply with federal, state, and local regulations.
The Contractor shall obtain all required permits.
Contractor and all associated subcontractor employees shall comply with applicable facility and local security policies and procedures. Contractor shall provide names of Contractor and Subcontractor employees to the COR or Government representative. VA will issue contractor badge/pass to contractor and subcontractor employees, as applicable. Employee list shall include name, title, qualifications and responsibilities like hazardous materials, PPE, fire safety penetrations, material disposal, lock-out/tag-out, etc.
The Contractor shall provide one (1) year performance agreement on labor and material in addition to the manufacturer’s warranty. Contractor shall provide a warranty management plan copy. The plan shall include information relevant to the warranty of materials and equipment incorporated into the project, including the starting date of warranty. The plan shall be in narrative form and contain enough detail to render it suitable for use by future maintenance and repair personnel, whether tradesmen, or of engineering background, not necessarily familiar with this contract. The plan shall be assembled in a binder and shall be turned over to the Government at the final walkthrough. The warranty period shall begin on the date of project acceptance and shall continue for the full product warranty period.
Overview
A. This Statement of Work (SOW) describes the requirements for the maintenance, inspection, testing, and/or calibration of the Electrical Power Distribution System and all their components including dissolved gas analysis. Additional applicable procedural guidelines shall be available at https://www.cfm.va.gov/TIL/spec.asp where appropriate specifications may be found by the Contractor.
B. This SOW describes the requirements for the Qualified Electrical Contract Professionals
– known as “Contractors” in this document, hereafter. Contractor shall perform maintenance and testing of the Electrical Power Distribution System, and all system components shall meet all requirements stated in this SOW.
C. This SOW describes the requirements to accomplish maintenance and testing reports.
D. This work is accomplished to maintain proper calibration of systems at two different VA
Facilities located at Waco, Texas and Temple, Texas.
Note: Required reports to the Government: Due Date: 30 after completion.
Provide complete written report of the switchgear testing. Reports shall include but not limited to the transformer location, size, manufacturer, voltage, date of sample. The results of the oil analysis are prepared in a written report and a pdf file.
A. Information shown in the report shall be as followings but not limited to:
1) Provide and install testing label at each breakers/switchgear
2) Company’s name, address, telephone & FAX numbers.
3) Name and signature of Contractors who perform the maintenance and testing.
4) VA Contract Number, and name and phone number of Contracting Officer’s
Representative.
5) Date and Time of work.
6) Copies of Contractors’ valid licenses, professional and training certificates.
7) Descriptions and model number of specialized tools and equipment used, such as torque wrench or infra-red scanning camera.
8) Location, Type, Name, and nameplate information of electrical equipment to be https://www.cfm.va.gov/TIL/spec.asp calibrated and tested.
9) Descriptions of work items.
10) Test data.
11) Reference materials such as equipment manufacturer’s specifications, coordination study, etc.
12) Remarks on conditions of electrical equipment. List all deficiencies, if any.
13) Recommended corrective actions, if any.
B. Report shall include but not limited to the transformer location, size, manufacturer, voltage, date of sample. The results of the oil analysis are prepared in a written report and a pdf file and shall include following gases but not limited to Hydrogen, Oxygen, Nitrogen, Methane, Carbon Monoxide, Carbon Dioxide, Ethane, Ethylene.
C. Submit four (4) hard copies of the complete written report, and one (1) CD-Rom of the electronic version of the report in pdf format to the COR within seven (7) calendar days of testing for each location. All reference materials shall be included in the electronic version of the report, either through scanning or other means of electronic text import methods.
D. Contractors shall report deficiencies that are deemed critical or catastrophic immediately to the Contracting Officer’s Representative.
Note 3: Qualifications of Contractors:
A. Contractors shall be certified by the InterNational Electrical Testing Association (NETA) as NETA Certified Technician and completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training.
B. Contractors shall have current technical trainings, and track records of working experience in maintenance, inspection, and testing of the Electrical Power Distribution System and its components in healthcare, industrial, educational, and commercial facilities for a minimum of five (5) continuous years.
C. Contractors shall have current safety trainings – either on-the-job or class-room type - in electrical safety outlined in the OSHA Standard 29 Code of Federal Regulations (CFR) 1910 Subpart S – Electrical, and the NFPA 70E – Standard for Electrical Safety in the Workplace. Training certifications shall be submitted to the Contracting Officer’s Representative (COR) prior to work. If no training certifications are available, the Contractor’s employer shall certify that he/she has met this requirement in writing and submit it to the Contracting Officer’s Representative prior to work commencement.
D. Contractors shall have own ready access to the latest versions of the following references:
1) NFPA 70, National Electrical Code.
2) NFPA 70B, Recommended Practice for Electrical Equipment Maintenance.
3) NFPA 70E, Standard for Electrical Safety for the Workplace.
4) NFPA 110, Standard for Emergency and Standby Power System.
5) OSHA Standard 29 CFR 1910, Subparts I & S.
6) InterNational Electrical Testing Association, Inc. (NETA) – Maintenance and Testing Specifications.
7) Operating/Maintenance manuals, and specifications of the electrical equipment to be maintained and tested in accordance with manufacturer’s guidelines.
E. Contractors shall provide own tools, equipment, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Tools, equipment, and PPE shall comply with the requirements of OSHA Standard 29 CFR 1910, Subpart I, and NFPA 70E.Weekend to limit the affect on patient care areas.
Note: The requirements under Qualifications of Contractors may be requested at any time after contract award. Failure to provide proof and meet these requirements shall result in immediate termination.
Performance Period: Work to be completed within 180 days of contract award.
Project Requirements:
1. The Contractor shall perform testing of listed components within this statement of work located at Temple and Waco locations.
2. The Contractor shall provide all labor, tools, equipment, materials, and supervision required to perform testing, removing, and re-installing of all breakers, switches, testing switchgears, breakers for testing, calibrating, adjusting, and lubricating of switchgears and circuit breakers.
3. The Contractor shall during reinstallation torque all wires to specifications and record specs in final reports.
4. The Contractor shall provide equipment, labor, materials, and sample bottles to draw transformer oil sample and test/provide oil dissolve gas analysis and dielectric strength data per ASTM D3612 and ASTM D300-00.
5. The Contractor’s work items, definitions, and references shall comply with the latest edition of the NETA – Maintenance Testing Specifications.
6. The Contractor shall calibrate and test all equipment listed in Attachment A located at Temple and Waco VAMC. Only 400 Amp & larger are being tested.
7. Contractor shall test and measure ground resistance at all listed transformers and building main switchboard electrical cabinets and/or grounding road locations at Temple and Waco. Provide the test data and inform COR any ground resistance above the NFPA70 ground resistance requirement at Temple and Waco.
8. The Contractor shall schedule all power outages through the VA COR for weekend work only 5 days prior to requirement. The outages shall minimize the impact on patient care and hospital operations.
9. The Contractor shall follow Lock out & tag out procedures.
10. The Contractor shall provide and install new labels on all equipment included in this SOW.
11. The Contractor shall at the end of the project, clean up, dispose of demolished, used materials, debris, or waste at an approved off-limits location in accordance with applicable local, State and federal laws and regulations and pay any related fees.
12. Perform a final walkthrough with the VA COR or PM when completed.

(b) In addition to the days designated as holidays, the Government observes the following days:
a. Any other day designated by Federal Statute
b. Any other day designated by Executive Order
c. Any other day designated by the President’s Proclamation
It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.

b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.
c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.
d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.
i. Time period being invoiced for.
ii. Description of services performed.
iii. Unit Cost billed.
iv. Extended amount due.
v. Invoice number, date, and;
vi. Contract and obligation numbers.
Other Considerations
• The contractor will be required to submit a safety plan for VA review and approval prior to the performing work.
• It is the contractor’s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements.
• The contractor’s supervisor shall always be available to address any needs the VA COR or PM may have during the period of performance. All employees or sub-contractors shall be accountable solely to the Contractor, not the Government.
• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc.
• The Government will not provide facilities or staging area for the Contractor’s use to support this contract. The contractor is responsible for all shipping, receiving, storage and security of any materials or equipment for the duration of this project.
• The Government will provide water and electrical utilities. All utilities in the facility will be available for the Contractor’s use in performance of tasks outlined in this SOW. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment.
• The Government will not provide materials for this project.
• Contractor personnel’s conduct shall not reflect discredit upon the Government. The Contractor shall ensure that personnel present a professional appearance. The Contractor’s employees shall observe and comply with all local policies and procedures concerning fire, safety, environmental protection, sanitation, security, and possession of firearms or other lethal or illegal weapons or substance. The Contractor is responsible for ensuring that any Contractor http://www.tungsten-network.com/us/en/veterans-affairs/ http://www.sam.gov/ employees providing services under this contract conduct themselves and perform services in a professional, safe, and responsible manner. The Contractor shall remove from the job site any employee for reasons of misconduct or security. The Contractor shall ensure no contractor employees conduct political related activities or events on VA Facilities.
• The Contractor shall notify the COR or Government representative of any accidents requiring medical treatment. Emergency medical treatment and services for Contractor personnel is the responsibility of the Contractor.
Codes and Standards The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), Inspectors Manual, VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.
The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:
(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.
(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.
(c) Compliance with EPA and State environmental regulations.
SAFETY PLAN REQUIREMENTS
Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.
The safety plan shall include, but not limited to the following:
A. Safety and Health Policy (i.e. company policy) B. Safety and Health Objectives (i.e. company goals, procedures, etc) C. Job Site Inspections (i.e. safety inspection/potential hazards/prevention, etc) D. Contractor Safety Administrator (i.e. competent person/safety officer) E. Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.)
F. Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)
G. Job Safety Training (i.e. OSHA, safety meetings, etc)
• 30 Hour OSHA Construction Safety Course – competent person
• 10 Hour OSHA Construction Safety Course – all others
H. Safety Discipline (i.e. correction of violations, etc) I. Hot Work Permit (i.e. responsibilities, procedures, etc) J. Emergency Procedures (i.e. responsibilities, procedures, etc) K. Written Hazard Communication Program (i.e. responsibilities, procedures, list of hazards, MSDS, etc)
SAFETY INSPECTIONS
On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:
A. Means of Egress
• Fire/Smoke Doors unlocked, free of obstructions
• Means of Egress (includes Exits) free of obstructions and clearly marked
B. Fire Alarm, Fire Detection, and Fire Suppression Systems.
• Fire Alarm Pull Stations are unobstructed
• Smoke Detector smoke caps in use, or removed at end of the day
• Hot Work Permits are current, in use
• Sprinkler Heads are unobstructed
C. Housekeeping and Traffic Control
• Trash and debris are removed promptly and transported outside the construction area
• Floor mats and dust tack mats are located at the entrance to construction area
• Tools and equipment are properly stored
• Signs installed to restrict patient access (doors locked as needed)
• Dust control measures are in place
• Barriers are in place and properly sealed
D. Hazardous Chemicals/Conditions
• Appropriate storage of hazardous chemicals
• MSDS sheets are readily available on-site
• Lock out/tag out procedures are in place
• Hot work permit is current and posted
E. Clothing
• PPE is available and properly worn
• Clothing is relatively dust free with the construction area and patient areas
