1. What is the contract number for the current contract effort? Please provide.

GS-06F-003N VA258-16-F-0203.

2. Are there any specific local conditions we need to be aware of that would prevent us from providing service on a specific day/date? We understand federal holiday restrictions apply.

3. Have there been any required termite applications within the last five years? If so, please provide the facility(ies) location(s). Also, please provide the chemical used. No termite applications have been required in the last 20+ years; however, annual termite inspections are required.

4. Have there been any foul nuisance (e.g. pigeon, mud swallow, etc.) issues in the past five years? Some pigeon issues have occurred under covered parking, with Engineering attempting predator emission to assist. No mud swallow issues to date. Over five years ago, we required contractor to assist with fowl control over patient or employee patio or congregation areas and spikes were placed on window ledges. This system needs review and update as needed.

5. What was the previous awarded Solicitation Number, Date awarded and awarded value? Files are from 2016. Orkin (Large Business).

6. What is the amount of hours each month the previous contractor spent on site to provide the pest services to all areas outlined within the scope of service? Does the government consider this service adequate? VA Estimate is 4 hours once per month for full perimeter and bait check/replacement, 1 hour once per month for mid-month food service treatment, 2 hours to treat all elevator pits. Yes, the service from current contractor has been very effective.

7. How many after hours service requests at each site have then been in the past 12 months? VA calls pest operator to report pests as found, each of which call is responded to within 24 hours. There may be 2-6 calls per month, depending on the season.

8. What are the regular service days for these sites? The regular days of service are agreed to between the vendor and the VA COR.

9. Who is the current pest vendor? Orkin (Large Business).

10. What is the number of bed bug service requests at each site over the past 12 months? The VA has not asked Orkin to treat bed bugs per their contract. EMS VA receives bed bug calls, inspects, treats using steam and monitors to determine effectiveness. EMS VA has engaged contractor support for more persistent presence in a limited environment no more than once per year.

11. Has any of the scope of service changed from this Solicitation compared to that of the last awarded contract? No.
