Complete Section 15A - C Below

The purpose of this amendment is to extend the due date for solicitation 36C25721Q0030 and answer vendor questions. The due date was 11-30-2020 at 11:59 PM CST and is now extended to 12-07-2020 at 12:00 PM (Noon) CST.

SOW has been modified under Testing-Adjusting-Balancing (TAB) of Heating, Ventilation and Air Condition, line 15.

Questions and Answers, and SOW attached below:

Questions and Answers

1. As of this time, the Government has not provided the “Mechanical Drawings” that they are required per the first paragraph (Requirements) of the Statement of Work.

All mechanical drawings are included with this amendment

2. Will there be any asbestos, lead, or other hazardous material that we will be required to abate?

No, per Safety office.

If so, please provide us the required hazardous material assessment report.

3. What will the ICRA level be for this project? We assume ICRA level 1. Please confirm or correct.

Prepare for Level 4 the highest, it could change if the conditions change. This is an assisted living facility.

4. Will any of the work be required to be performed after normal duty hours or on weekend?

Not as of now. Any changes to this will be addressed if need arises.

5. The Statement of Work requires “. . . a Test Adjusting and Balance (TAB) of the entire

HVAC system at Building 221A, 221B, 221C, 221D, and 221M, Temple Veterans

Administration, Temple, Texas.” To properly do this TAB, it is imperative that the Government provide us with the mechanical as built drawings for these buildings since we must obtain information such as number and size of air handling units, design air f lows in cubic feet per minute, supply and return air duct sizing, supply and return fixture makeup and size, design supply and return air f low and a multitude of other information.

All mechanical drawings are included with this amendment

To be clear, without as built drawings of the entire HVAC system for the Buildings listed above, an actual TAB cannot be performed.

6. The current Statement of Work makes no mention of the work required on the building exhaust systems. If work on the exhaust systems is required, please provide clarity on what is required (including TAB requirements associated with the exhaust system if any).

Air Systems: Includes all outside air, supply air, return air, exhaust air and relief air systems. All air must be accounted for to perform an accurate Testing, Adjusting, and Balancing (TAB) per specification. There won’t be any physical/work done on the exhaust systems.

7. There was nothing about the exhaust system in the scope. We assume that would be needed to do a correct balance on the building. Is the exhaust system par t of the scope of work?

8. Please provide the "design drawings and specifications (Q&A)" as referenced on page

5, Project Requirements, Inspection and Verification. (also referenced multiple times throughout the Statement of Work)

9. In addition to RFI #1 and 2, please provide any other relevant Specifications that apply to the equipment service OEM.

Refer to the drawings /Site visit.

10. Please specify what services are expected to be included under the one year performance agreement in CLIN 00002; reference page 10 of the Solicitation - "Price / Cost

Schedule" - CLIN Item #0002: "One (1) year performance agreement labor and material in addition to the manufacturer's warranty".

One (1) year workmanship warranty on the work and replacement parts.

11. What are the current COVID19 requirements pertaining to site/building access?

COVID-19 test every 30 days.

COVID 19 screening at building entry daily.

Current Flu shot.

Mask/safety glasses or goggles or face shield daily.

Contractor is responsible for the cost and scheduling of the tests.

12. What is the anticipated start date?

The VA anticipates awarding this contract by December 15, 2020

STATEMENT OF WORK

Building 221

For HVAC Service and TAB

Temple VA Project

(Updated November 30, 2020)

Requirements

The Department of Veterans Affairs (VA), Central Texas Veterans Health Care System is requesting that the HVAC systems be serviced per OEM Specifications and a Test Adjusting and Balance (TAB) of the entire HVAC system at Building 221A, 221B, 221C, 221D, and 221M, Temple Veterans Administration, Temple, Texas. It is the Contractor’s responsibility to review VA provided Mechanical Drawings to determine and calculate the number of units, equipment, valves, coils, etc. as described in the scope of work. A site visit prior to bid is required to be fully acquainted with existing site conditions, scope of work, and field verify all existing conditions and working spaces for inspections, services and TAB.

The Contractor shall provide COVID-19 tests and results for all contractor personnel working on this project every 30 days.

The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision, Personal Protective Equipment (PPE) and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained. All work must comply with federal, state and local regulations.

The building will be occupied, and Government operations will continue a normal basis for the duration of the contract. The Contractor shall take all precautions to ensure that his operations are conducted in a manner so as not to interfere with the normal operation of the building’s. The Contractor shall execute work in a safe manner and shall not subject personnel to unsafe conditions.

Work scheduled shall include a detailed list of work to be done for each requirement and the sequence in which will be accomplished. The schedule shall be in the form of a progress chart of suitable scale to indicate the work scheduled for completion by any given date during the work period. The work plan shall indicate time frames and programmed power shut-off, if applicable. Contractor shall highlight on the schedule any off-hours and overtime work requirements.

The Contractor shall provide one (1) year performance agreement labor and material in addition to the manufacturer’s warranty. Contractor shall provide the warranty management plan copy. The plan shall include information relevant to the warranty of materials and equipment incorporated into the project, including the starting date of warranty. The plan shall be in narrative form and contain enough detail to render it suitable for use by future maintenance and repair personnel, whether tradesmen, or of engineering background, not necessarily familiar with this contract. The plan shall be assembled in a binder and shall be turned over to the Government at the final walkthrough. The warranty period shall begin on the date of project acceptance and shall continue for the full product warranty period.

The Contractor shall obtain and adhere to all required permits. In addition to other needed permits the following permits are required for this contract:

A. CTVHCS Project Review Preconstruction Risk Assessment and Exposure Control Checklist. (PCRA)

B. Interim Life Safety Measures (ILSM)

C. Infection Control Risk Assessment (ICRA)

D. Above Ceiling Permit (ACP)

All work under this contract shall be accomplished by a Licensed Professional skilled in the trade.

The Contractor shall notify PM at completion of work each day.

The contractor will provide an overall report of all findings, deficiencies, improvements and recommendation. Provide one hard copy and one electronic (PDF) report.

All work shall be done according to the Infection Control Risk Assessment (ICRA) specified for the respective areas where work is performed.

Note: “The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract.

Project Requirements

Inspection and Verification

1. Inspect and Verify HVAC Equipment is installed per design drawings and specifications (Q & A).

Florida Heat Pump (FHP/Bosch), Outside Air Heat Pumps, Water Cooled Condenser Heat Pumps, DX Mini Splits, Balancing Valves, Reheat Coils, Three Way Valves, Induct Humidifiers, and

Control Circuits.

2. Inspect and Verify HVAC Condenser Water Loop is installed per design drawings and specifications (Q & A). Cooling Tower Pumps, Heating water Pumps for Condensate loop, Boilers, Variable Frequency Drives (VFD), all piping to include Balancing Valves, By Passes, Strainers, Mixing Valves, Electronic Actuators, Manual valves, Make Up Water, Cooling Tower Fill, Fan

Blades, Bearings and Basin Heaters.

3. Inspect and Verify HVAC Duct Work is installed per design drawings and specifications (Q & A).

Filter Housings, Filter Bases, Dampers, and Actuators.

4. All discrepancies and deficiencies discovered during inspections will be brought to the Project

Managers (PM) attention to evaluate and determine if immediate corrective action is justified.

Upon justification Contractor will provide the Contracting Officer Representative (COR) an itemized material list, scope of work and pricing of all findings.

Service Per OEM

1. Service all HVAC Equipment per Manufactures Recommended Service Requirements to include cleaning Coils and replacing Filters. Florida Heat Pump (FHP/Bosch), Outside Air Heat Pumps, Water Cooled Condenser Heat Pumps, DX Mini Splits, Balancing Valves, Reheat Coils, Three Way

Valves, and Induct Humidifiers.

2. Service HVAC Condenser Water Loop System per Manufactures Recommended Service

Requirements to include flushing of the Condenser Loop. Heating water Pumps for Condensate loop, Boilers, Variable Frequency Drives (VFD), all piping to include Balancing Valves, By Passes, Strainers, Mixing Valves, Electronic Actuators, Manual valves, Make Up Water, and Bearings.

3. Service and clean Drain Pans and Condensate Drain Lines. Flush and treat all drain lines. Check and correct all drain pans for proper drainage and ensure they are leveled.

4. Service all HVAC Filter Housings, Filter Bases, Dampers, and Actuators per Manufactures

Recommended Service Requirements.

5. All discrepancies and deficiencies discovered during Service will be brought to the PM’s attention to evaluate and determine if immediate corrective action is justified. Upon justification contractor will provide COR an itemized material list, Scope of work and Pricing.

Testing-Adjusting-Balancing (TAB) of Heating, Ventilation and Air Condition (HVAC) Systems

1. TAB Specialist shall be identified by the General Contractor within 3 days after the notice to proceed. Shall sign the TAB reports that bear the seal of the TAB standard. The reports shall be accompanied by report forms and schematic drawings required by the TAB standard, AABC, TABB or NEBB. The Contractor shall follow the VA Office of Construction & Facilities

Management Master Construction Specifications (PG-18-1) Division 23 Section 23 05 93 for TAB requirements, provided upon request by the contractor. The TAB reports (hardcopy and electronically) are to be provided to the PM or COR prior to final payment.

2. The contractor shall perform TAB in accordance with American Society of Heating, Refrigerating and Air-Conditioning Engineers (ASHRAE) – HVAC Applications, Testing, Adjusting and Balancing manual ANSI/ASHRAE Standard 111-1988 – “Practices for Measurement, Testing, Adjusting, and

Balancing of Building Ventilation, Air-Conditioning, and Refrigeration Systems.”

3. Building drawings and equipment schedules shall be furnished to the contractor.

4. The Contractor shall conduct TAB procedures to ensure HVAC design objectives for the facility are met.

5. Balancing airflow and water flow at source equipment and/or within distribution systems, including sub-mains, branches, and terminals, to indicated quantities according to specified tolerances.

6. Adjusting total HVAC systems to provide indicated quantities.

7. Verifying that automatic control devices are functioning properly.

8. Reporting results of the activities and procedures.

9. The contractor shall plug all holes that have been made in the ducts for taking test readings, in order to prevent air leakage. Disruption of duct, pipe, and/or equipment insulation shall also be appropriately repaired by the contractor.

10. Perform a final walkthrough with the VA PM and COR or Alternative when completed.

11. The Contractor shall start work within 10 days of the award date and work shall be completed within 60 days of the start date.

12. All outages will be coordinated with the Project Manager 10 days out and does not interfere with patient care.

13. Work is to be performed during normal business hours 0800 to 1630 Monday – Friday. Work performed outside of normal hours must be approved by COR.

14. Coordinate with VA COR before the start of this service. See information below:

15. Air Systems: Includes all outside air, supply air, return air, exhaust air and relief air systems. All air must be accounted for to perform an accurate TAB per specification.

A. Government Holidays:

(a) The Government hereby provides notice and Contractor hereby acknowledges receipt that

Government personnel observe the listed days as holidays:

Martin Luther King's Birthday Third Monday in January

Washington’s Birthday Third Monday in February

Memorial Day Last Monday in May

Independence Day July 4th

Labor Day First Monday in September

Columbus Day Second Monday in October

Christmas December 25th

(b) In addition to the days designated as holidays, the Government observes the following days:

a. Any other day designated by Federal Statute

b. Any other day designated by Executive Order

c. Any other day designated by the President’s Proclamation

It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.

16. Submit the proper documentation and reports to receive payment on this service. See information below:

A. Invoices and Payment

a. Payment to be made monthly, in arrears, following satisfactory performance by

Contractor’s personnel and upon receipt of a properly prepared invoice.

b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.

c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.

d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.

i. Time period being invoiced for.

ii. Description of services performed.

iii. Unit Cost billed.

iv. Extended amount due.

v. Invoice number, date, and;

vi. Contract and obligation numbers.

Other Considerations

• The contractor shall be responsible for the verification of all requirements for this project. All measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the

Contracting Officer’s attention.

• The contractor will be required to submit a safety plan for VA review and approval prior to the performing work.

• The contractor must comply with the requirements for Type C/Class II (Lowest Risk) Infection

Control Risk Assessment (ICRA).

• It is the contractor’s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements.

• Majority of the work shall be performed during normal working hours unless otherwise noted by the COR. Normal working hours are 8:00 am – 4:30 pm, Monday – Friday.

• The contractor will be required to submit a phasing plan prior to performing work. The phasing plan shall be developed and coordinated with VA COR.

• The contractor is responsible for any leaks or damage that may occur during this project. All damage must be repaired and inspected by the VA COR prior to final walkthrough.

• The contractor’s supervisor shall always be available to address any needs the VA COR may have during the period of performance.

• The contractor may be required to coordinate work activities and shutdowns with VA COR and

M&O Supervisors.

• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc.

• The contractor shall be responsible for the removal of project demo materials/debris

/equipment from this facility when complete.

Codes and Standards

The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection

Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and

Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.

The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and

DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy

Memorandums, including but not limited to the following:

(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.

(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA

30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.

(c) Compliance with EPA and State environmental regulations.

SAFETY PLAN REQUIREMENTS

Prior to performing any work, the contractor will be required to submit a safety plan in accordance with

OSHA and VA Regulations.

The safety plan shall include, but not limited to the following:

A. Safety and Health Policy (i.e. company policy)

B. Safety and Health Objectives (i.e. company goals, procedures, etc)

C. Job Site Inspections (i.e. safety inspection/potential hazards/prevention, etc)

D. Contractor Safety Administrator (i.e. competent person/safety officer)

E. Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.)

F. Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)

G. Job Safety Training (i.e. OSHA, safety meetings, etc)

• 30 Hour OSHA Construction Safety Course – competent person

• 10 Hour OSHA Construction Safety Course – all others

H. Safety Discipline (i.e. correction of violations, etc)

I. Hot Work Permit (i.e. responsibilities, procedures, etc)

J. Emergency Procedures (i.e. responsibilities, procedures, etc)

K. Written Hazard Communication Program (i.e. responsibilities, procedures, list of hazards, MSDS, etc)

SAFETY INSPECTIONS

On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:

A. Means of Egress

• Fire/Smoke Doors unlocked, free of obstructions

• Means of Egress (includes Exits) free of obstructions and clearly marked

B. Fire Alarm, Fire Detection, and Fire Suppression Systems.

• Fire Alarm Pull Stations are unobstructed

• Smoke Detector smoke caps in use, or removed at end of the day

• Hot Work Permits are current, in use

• Sprinkler Heads are unobstructed

C. Housekeeping and Traffic Control

• Trash and debris are removed promptly and transported outside the construction area

• Floor mats and dust tack mats are located at the entrance to construction area

• Tools and equipment are properly stored

• Signs installed to restrict patient access (doors locked as needed)

• Dust control measures are in place

• Barriers are in place and properly sealed

D. Hazardous Chemicals/Conditions

• Appropriate storage of hazardous chemicals

• MSDS sheets are readily available on-site

• Lock out/tag out procedures are in place

• Hot work permit is current and posted

E. Clothing

• PPE is available and properly worn

• Clothing is relatively dust free with the construction area and patient areas

13. WARRANTEE: The contractor shall warrantee work to be free of defects and workmanship for a period of 1 year from date of acceptance.

14. QUALITY ASSURANCE: The PM/COR will inspect all work at completion.
