Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07-2.
This solicitation is set-aside for SDVOSB.
The associated North American Industrial Classification System (NAICS) code for this procurement is 561720, with a small business size standard of $19.5M The FSC/PSC is S299.

· Replace to match thirty (30) 48 “ x 24” HVAC grills at top of atrium.
· Clean all sprinkler heads in atrium ceiling above 1st floor, 3rd floor, and 5th floor landings and main atrium ceiling.
· Wash atrium side of atrium wall glass and frame.
· Paint fifth floor ledges on atrium side to match, VA to provide color.
· Stain or re-finish third floor planter wood edges on atrium side.
· Clean glass, both sides on third and fifth floors and dust and/or vacuum adjacent ledges.
· Clean or replace with same or similar artificial plants in eight (8) third floor planters.
· Dust and/or vacuum base ledges of three skylights in Main and East Atriums.
· Wash inside and outside of elevator shaft glass and wipe frames.

STATEMENT OF WORK
Title of Project: Clean Clinical Addition Atrium Background: The atriums in the Clinical Addition have surfaces that require special high access equipment and training to reach for cleaning. Additionally, some painted areas and wood trimmed areas on the 3rd and 5th floor ledges have become water-stained or faded and require painting or treatment of the wood surfaces. The goal is to accomplish these tasks at low peak times such as over weekends, after normal working days and hours, or during a holiday. Normal Working days and hours are Monday through Friday, 8am to 4:30pm. Patient, staff, and visitors shall be able to access clinical areas with minimum pedestrian reroutes or closures. All reroutes or closures must be coordinated and approved by the VA one week prior to starting.
Scope of Work: The contractor shall provide all labor, supervision and all other resources required to:
· Repair and paint to match water damaged areas in ceiling above Clinic 1 Waiting Area.

Period of Performance: Work to be accomplished on weekends, after hours, or during a Holiday between September 21, 2020 and November 30, 2020. The contract period of performance is 60 days after notice to proceed.
Hours: The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday and any other day specifically declared by the President of the United States to be a national holiday.

Type of Contract: Firm-fixed-price Technical Requirements:
· Contractor must attend VA North Texas Safety Orientation prior to beginning work. This orientation is only one (1) hour and can be scheduled through the Contracting Officer Representative (COR).
· Contractor shall be capable of maintaining Infection Control Precautions as listed in attached Description of Required Infection Control Precautions by Class.
· The contractor shall provide proof of 30 Hours Occupational Safety and Health Administration (OSHA) Certification for On-Site Manager and 10 Hours OSHA Certification for all On-Site Staff in accordance with OSHA 29 CFR 1926.
· The Contractor shall describe two projects the firm has completed within the past three years, which are equal to or similar in scope of this requirement.
· The Contractor shall be responsible for employing technically qualified personnel to perform the work specified in this Statement of Work. This work will require personnel that have the ability and skill to work Automatic and Manuel Lifts, Pulleys, etc. as per manufacture specifications.
· Contractor shall adhere to VA North Texas COVID-19 Protocol. This protocol requires:
(1) All personal to be screened by VA representatives before entering Dallas Campus Building.
(2) All staff shall wear masks.
(3) Sanitize or Wash hands on a regular basis.
(4) Observe employees daily to determine if they show signs of illness. An employee who is displaying continuous coughing, sneezing, or high temperatures shall be sent home and shall not return until clearance is provided by their medical provider.
GOVERNMENT BADGES: The Contracting Officer Representative (COR) or VAMC Representative will coordinate the issuance of required access badges to Contractor personnel. Contractor personnel shall wear laminated identification badges provided by the Government always when performing. Badges shall be worn on the outer garment, attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard.
Government issued identification badge remains the property of the Government and shall be returned to the issuing office or other designated Government personnel upon transfer of Contractor employees from the contract.
CONTRACTOR BADGES (NOT APPLICABLE): All Contractor personnel attending meetings, answering Government telephones, and working in other situations where their Contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public or Congress that they are Government officials. They must also ensure that all documents or reports produced by Contractor personnel are suitably marked as Contractor products or that Contractor participation is appropriately disclosed.
SAFETY/SECURITY REQUIREMENTS: The Contractor shall comply with all applicable federal, state and local laws and ensure performance is secure while protecting material, equipment, and actor supplies from damage and loss. Government security personnel shall have the express right to inspect for security violations at any time during the term of the contract. All information generated in performance of this contract becomes property of the Government and must be surrendered upon request and/or completion of the contract. The Contractor must protect against the destruction and/or misplacement of all information produced under this contract, unless directed under local policy to destroy. This includes both hard and soft copies of data.
The Contractor shall not release any information (including photographs, files, public announcements, statements, denials, or confirmations) on any part of the subject matter of this contract or any phase of any program hereunder without the prior written approval of the COR.
CONDUCT OF PERSONNEL: Contractor personnel shall conduct themselves in a professional manner (i.e. timeliness, communication; spoken and written etc.). The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government. The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this performance work statement. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel. Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length in excess of three inches, while assigned under this contract.
CONTRACTORS ATTIRE AND APPEARANCE: Contractor personnel shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities. The Government reserves the right to review Contractor personnel qualifications. Upon request, resumes shall be provided to the Contracting Officer prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority.
Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services.
The COR/VAMC POC and the Contractor will assure that-
1. No other information except what is in this contract will be shared with the contractor in any follow up communication.
1. None of the information in this contract includes Protected Health Information (PHI) or Individually Identifiable Information (III).
Privacy Officer: The contractor will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA.
Records Management:
1. Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chs. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with the safeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion.
2. In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation.
3. In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law. Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use of the records and data.
4. VANTHCS and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Records may not be removed from the legal custody of VANTHCS or destroyed except for in accordance with the provisions of the agency records schedules and with the written concurrence of the Head of the Contracting Activity. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701.
In the event of any unlawful or accidental removal, defacing, alteration, or destruction of records, Contractor must report to VANTHCS. The agency must report promptly to NARA in accordance with 36 CFR 1230.
5. The Contractor shall immediately notify the appropriate Contracting Officer upon discovery of any inadvertent or unauthorized disclosures of information, data, documentary materials, records or equipment. Disclosure of non-public information is limited to authorized personnel with a need-to-know as described in the [contract vehicle]. The Contractor shall ensure that the appropriate personnel, administrative, technical, and physical safeguards are established to ensure the security and confidentiality of this information, data, documentary material, records and/or equipment is properly protected. The Contractor shall not remove material from Government facilities or systems, or facilities or systems operated or maintained on the Government’s behalf, without the express written permission of the Head of the Contracting Activity. When information, data, documentary material, records and/or equipment is no longer required, it shall be returned to VANTHCS control or the Contractor must hold it until otherwise directed. Items returned to the Government shall be hand carried, mailed, emailed, or securely electronically transmitted to the Contracting Officer or address prescribed in the [contract vehicle]. Destruction of records is EXPRESSLY PROHIBITED unless in accordance with Paragraph (4).
6. The Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, contracts. The Contractor (and any sub-contractor) is required to abide by Government and VANTHCS guidance for protecting sensitive, proprietary information, classified, and controlled unclassified information.
7. The Contractor shall only use Government IT equipment for purposes specifically tied to or authorized by the contract and in accordance with VANTHCS policy.
8. The Contractor shall not create or maintain any records containing any non-public VANTHCS information that are not specifically tied to or authorized by the contract.
9. The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected from public disclosure by an exemption to the Freedom of Information Act.
10. The VANTHCS owns the rights to all data and records produced as part of this contract. All deliverables under the contract are the property of the U.S. Government for which VANTHCS shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Any Contractor rights in the data or deliverables must be identified as required by FAR 52.227-11 through FAR 52.227-20.
11. Training. All Contractor employees assigned to this contract who create, work with, or otherwise handle records are required to take VHA-provided records management training, Talent Management System (TMS) Item #3873736, Records Management for Records Officers and Liaisons. The Contractor is responsible for confirming training has been completed according to agency policies, including initial training and any annual or refresher training.
The contract period of performance is 60 days after notice to proceed.

All quoters shall submit the following:

Award shall be made to the lowest priced technically acceptable offeror whose quotation meets the requirements of the solicitation.
This requirement is set-aside 100 percent service-disabled veteran-owned small business.
Note: Offerors must be certified in the VIP database and registered in SAM to be eligible for award.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."

The VA is strongly encouraging all interested parties to attend the Site Visit scheduled on September 03, 2020, 10:00AM CST. Participants will meet at VANTHCS, building 9, outside on south side, Parking Lot 7 and walk over to be screened to enter the building. Interested parties must inform the VA of your intent to attend the Site Visit by email. Please note that interested parties will not be authorized to use this site visit to present or “pitch” their offer, services, or capabilities to any Government representative.
“Interest in Site Visit” notices must be received by Wednesday, September 02, 2020 at 2:30PM CST by email to Lynn Pettit at Lynn.Pettit@va.gov. The content of this notice should be as follows: The company name, the names of the representatives who will be attending, valid phone number and email.
Any questions or concerns regarding this solicitation and site visit should be forwarded in writing via e-mail to the Contracting Officer at Lynn.Pettit@va.gov. Questions in response to this solicitation are due no later than September 04, 2020, 10:00AM CST.
Submission of your response shall be received not later than September 11, 2020, 4:00PM CST at Lynn.Pettit@va.gov. Reference “RFQ 36C257-20-Q-1284 Clean Clinical Addition Atrium” on subject line of email.
Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
