The line items will be evaluated on total price. The purchase order will be awarded to the lowest priced technically acceptable (LPTA) offeror.
Each offeror must be able to supply the total quantity of the order for their quote to be considered responsive.

STATEMENT OF WORK (SOW)

2. Contract Title. Wandering (Elopement) System Replacement
3. Background. The current system has reached its replacement date and is behind in technology.
4. Scope.
The Thomas E Creek Amarillo VA Healthcare System is seeking a replacement Patient Elopement system for the Community Living Center. The system will encompass 2 floors of the Community Living Center. The system shall monitor 6 door zones on the first floor, 2 sets of double doors, 4 single doors and 2 elevators and on the 2nd floor there are 2 zones, 1 set of double doors and 1 single door. The Contractor will deinstall and remove all components of the existing elopement system.
The door controllers must use a combination of bluetooth low energy (2.4ghz) and low-frequency (125 kHz) exciter field to avoid false alarms (37.3 dB micro Amps at 10 meters). Must also have the ability to change the perimeter of the zones (24” up to 20’) to not interfere with other zones and patient examination areas by using a mobile application. Door controllers must connect wirelessly to the solution. Biomed will be provided with a tablet to do wireless programming and configuration.
The system must be capable of utilizing two types of patient tags simultaneously. First tag type must stay off/standby until energized by the exciters in the zones. The second tag type must be able to give patient location using a locate feature (always actively looking for and engaging with the tag) and transmit location to the nurse call phones (Cisco 7925). Tags will respond/transmit in bluetooth low energy signals (2.4 GHz), have a high-impact tamper-resistant case, easily sanitized, waterproof (IP67 compliant), and have a unique electronic serial number. The wristbands need to have a cut-proof option. Both nursing stations will be provided with a handheld detector for tag activation and battery checks.
The door controller must be able to lock down the doors utilizing 24-volt magnetic locks, when a patient is detected in the zone and the doors are closed. The door controllers must have the ability to alarm, close, and lock the doors if a monitored patient enters the zone and the doors are open. All monitored doors and elevator zones must remain locked or disabled until staff can come and relocate the patient. Each door will have a keypad with a display screen to clear the alarmed door on both sides of the door way to allow access during emergencies. The elevators will have a keypad with a display screen on the inside of the car. The keypad with a display screen and the keypad will clear the alarm or bypass the door utilizing staff individual PIN codes (up to 1000 different PINs).
System must have 2 types of keypads.
The 1st type of keypad needs a display screen that will show status, errors, and events, tag ID, and a buzzer that is activated with alarms and events. This keypad functions must be performed with either PIN codes or a badge.
The 2nd type of keypad needs backlit push buttons, waterproof, powered by the door controller, and integrated card reader that supports the use of badges and individual PINS.
The system must have the capability to store patient roaming history and status of the zones. The system must display alarms utilizing CAD drawing provided, along with the Patient name, picture, and any warning messages. There must be one of these displays on each floor at each Nurses station for ease of monitoring and patient data entry. The Nursing station displays will be connected to the server located in Biomedical Engineering’s Server bay, room 5102-01. These units will connect via an IT provided switch utilizing VLAN 610.
The system and patient activity history must be able to print customizable, web-based reports on a printer located in Bldg. 29, 1st floor. The system must provide patient location feature that actively looks for and engages with a patient tag. This location feature must monitor each patient to within 20 feet of their actual location. The system must integrate with Rauland Responder 5 and send alerts to our existing CISCO 7925 phones through the responder 5 nurse call system. The door zones must also be interfaced with the existing fire alarm system, allowing all doors to unlock in the event of a fire alarm being activated.

5.3 Biomed Server Bay installation: Install rack mountable 1U server in provided 19” rack space and connect to provided ports to switch. Server should include all operating systems and software required for operation. Server 2012 is preferred.
5.4 Nursing Staff Training: Training for Biomed and Nursing Staff will be provided on site and for each shift
5.5 Old Equipment Removal: When new equipment has been installed and tested then old equipment can be removed and turned over to Biomed for proper disposal.
Deliverables: Will be received by warehouse and delivered to a designated space until supplies are needed for installation.
6. Performance Monitoring
6.1 The COR or designated VA Engineering Employee will visually inspect daily for quality of work and adherence to the SOW, ILSM and ICRA.
6.2 VA Staff will verify daily cleanliness of the area before, after and during the workday.
7. Security Requirements
7.1 Contractor will check in with the VA Police Service for temporary ID badges.
7.2 The Thomas E Creek VA Medical Center Police Service will determine the status of each Contractor or Affiliate’s background investigation status for the issuance of a Non-PIV or PIV-Flash card.
7.3 Contractor will not have access to PHI or PII information or sources.
7.4 Contractor will be escorted at all time, by Authorized VA Engineering Staff, while they are performing work in the IDF or Server Bay.
8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Data drops, or ports will be provided at the Unit Clerk areas and the Biomed Server Bay 5102-01.
9. Other Pertinent Information or Special Considerations.
9.1 All penetrations through walls will be filled with VA approved fire caulk. All penetrations will be inspected by VA Safety Personnel.
9.2 A copy of the approved Interim Life Safety Measures, (ILSM) and Infections Control Risk Assessment, (ICRA) documents must be displayed at all time during the installation.
9.3 Work shall be accomplished Monday-Friday 8:00am-4:30pm.

9.4 Contractor must check in with the Police Service in the Lobby of building 28, daily.
9.5 System must be installed without disturbing the existing system. If existing system must be disturbed, a two-day lead time will be required to bring on extra staff to cover the entrance/exit.

9.12.1 Nursing Staff training for all shifts will be accepted by the COR, after the Chief of Community Living Center is satisfied that the staff on different shift are trained.
9.12.2 Biomedical Staff training will be accepted by the COR, after the Biomedical Supervisor is satisfied there was acceptable training.
9.12.3 The COR or designated VA Staff will verify correct operations, programming, and training before installation will be complete.
10. Risk Control:
10.1 When pulling wire in areas, where patients will be present, minimize amount of cabling on the ground at one time and utilize safety cones, when there is a trip hazard, due to cabling.
10.2 Pulling of wire where it may make a potential trip hazard, will only be performed in slack traffic period, (i.e. around the dining rooms at meal times, etc.).
10.3 All wiring/cabling will be plenum rated
10.4 Full compliance with ICRA and ILSM shall be upheld.
10.5 OSHA: All equipment personnel will be OSHA compliant.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, an appropriate Inspector General appointed under section 3 or 8G of the Inspector General Act of 1978 (5 U.S.C. App.), or an authorized representative of either of the foregoing officials shall have access to and right to—
(i) Examine any of the Contractor's or any subcontractors' records that pertain to, and involve transactions relating to, this contract; and
(ii) Interview any officer or employee regarding such transactions.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), and (c), of this clause, the Contractor is not required to flow down any FAR clause in a subcontract for commercial items, other than—

C.4 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001;
(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests;
(4) Invoice payment has the meaning given in FAR 32.001; and
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:
(1) VA’s Electronic Invoice Presentment and Payment System at the current website address provided in the contract.
(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI).
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances in this paragraph (e), the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for—
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.5 VAAR 852.246-71 REJECTED GOODS (OCT 2018)
(a) Supplies and equipment. Rejected goods will be held subject to Contractor’s order for not more than 15 days, after which the rejected merchandise will be returned to the Contractor’s address at the Contractor’s risk and expense. Expenses incident to the examination and testing of materials or supplies that have been rejected will be charged to the Contractor.
(b) Perishable supplies. The Contractor shall remove rejected perishable supplies within 48 hours after notice of rejection. Supplies determined to be unfit for human consumption will not be removed without permission of the local health authorities. Supplies not removed within the allowed time may be destroyed. The Department of Veterans Affairs will not be responsible for, nor pay for, products rejected. The Contractor will be liable for costs incident to examination of rejected products.

Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show—
(1) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers.
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and—
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.
(i) Availability of requirements documents cited in the solicitation.
(1)(i) The GSA Index of Federal Specifications, Standards and Commercial Item Descriptions, FPMR Part 101-29, and copies of specifications, standards, and commercial item descriptions cited in this solicitation may be obtained for a fee by submitting a request to— GSA Federal Supply Service Specifications Section Suite 8100 470 East L'Enfant Plaza, SW Washington, DC 20407 Telephone (202) 619-8925 Facsimile (202) 619-8978.

Additional copies will be issued for a fee.
