I. Basis of Award The Government intends to award a delivery order resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation meets or exceeds the requirement at the lowest price, Lowest Price Technically Acceptable (LPTA).
The quotes will be evaluated on the basis of the evaluation factors listed below. The technical acceptability will be evaluated on an "acceptable" or "unacceptable" basis. Only those contractors determined to be technically acceptable will be evaluated on price. The following areas will be evaluated:

Factor 1: Technical Acceptability – The contractor shall provide a written statement addressing each of the following items. The written statement must describe each point and must provide enough detail to determine minimum qualifications.
a. Description of Specifications: Provide a listing of specifications meeting the specifications contained in the Statement of work.
Factor 2: Price – Contractor shall complete B.3 Price/Cost Schedule. The schedule may be altered, or an alternate schedule used where appropriate in order to express quote detail.
El Paso Veterans Affairs Health Care System (ELPVAHCS) ZEISS HD CIRRUS 5000 Optical Coherence Tomography system or equal Statement of Work (SOW)
1. GENERAL
1.1. The contractor shall comply with Federal, State, local laws and regulations applicable to the performance of this contract. The El Paso VA Health Care System (EPVAHCS), Eye Clinic, requires the purchase of a Zeiss Cirrus HD 5000 with OCT angiograph Tomography, Optical Coherence Unit or equal, Table and Printer. The EPVAHCS requires one (1) system for the main facility, Eye Clinic Section.
2. GENERAL REQUIREMENTS
2.1. The contractor shall provide equipment suitable to provide the EPVAHCS Eye Clinic Section with the ability to accurately and efficiently obtain optical tomography images (optic nerve, RNFL, Macular Cube, Wide angle, Angle to Angle Anterior chamber measurements, and detailed Corneal Pachymetry measurements and OCT Angiography through the use of an HD OCT system with OCT Angiography capabilities.
2.2. The contractor shall provide all tools, equipment, technical information, travel and installation and training.
2.3. The Contractor is expected to respond with 24 hours of the maintenance call. No reports or documentation is required to be submitted after repairs.
2.4. EQUIPMENT REQUIREMENTS
a. Salient characteristics, technical specifications and imaging specifications the shall be met for consideration.
b. The specifications set forth in this section are the minimum requirements of the government needs.
c. The quoted equipment shall meet or exceed the minimum requirements listed to be considered.
d. MUST INCLUDE ALL THE FOLLOWING at a minimum for consideration: see below
· Disease Progression analysis results are a must using every existing glaucoma and diabetic patient testing results on our current system. Every patient’s information, testing images, previous progression results must be able to be transferred successfully to the new system for use without losing any form of existing data.
· OCT testing results will be used to discover, track, and analyze disease progression and pathological events from multiple points of view. The system needs to examine retinal detail at the pixel level for critical pathological elements, focus the signal lower in the scan window for assessment of the deeper choroidal tissue, and precisely target and capture the same tissue every time to ensure consistent comparison.
· The systems need to perform reproductive analysis (algorithms to measure and display layers for tissue targeting, segmentation, and producible measurements), comparative reference (diversified normative database of OHN, RNFL, ganglion cell/IRL, and evaluation and measurements of macular thickness to facilitate at-a-glance identification of anatomy outside normal limits).
· The system must AUTOMATICALLY register with historical data, allowing for point-to-point measurements of change. This system will be used also as a Primary tool used for comprehensive glaucoma management, Ophthalmic Diabetic related patient management, and for Anterior Chamber and Pachymetry measurements. Testing results must read similarly in testing results/reports to include color and numerically and in testing results to existing HD Cirrus 5000 OCT w/angiography unit in place already. Zeiss style glaucoma/glaucoma progression/macular cube/anterior chamber measurements/and Pachymetry measurement reports must read similarly in Zeiss style format equal to our existing HD Cirrus 5000 OCT unit.
· Must be able to successfully transfer existing patient information and testing results from the existing Zeiss HD OCT 5000 system onto the new system to make available for review by physician. Reports must be in Zeiss format, similar to current Zeiss Glaucoma progression results, Macular Cube Progression results, using preexisting and new patient testing/images. Testing results in the form of accurate and reliable progression analysis results similar to the old system must be able to be created using existing testing results and new testing results.
· Zeiss style Progression analysis characteristics and results are a must as we have years and years of existing tests and previous progression analysis that must not be lost
· There can be NO loss of patient information, or progression analysis results in the transfer of existing patient testing results and personal health information.
· Capability for OCT Angiography is provided by a 1 single OCT scan that allows for visualization of both micro vascular and structural information in a noninvasive dye free scan by the OCT.
· Has the ability to import and integrate the Cirrus Model HD OCT 4000 raw data.
· The raw data must be interchangeable between the Cirrus 4000 and Cirrus 5000unit (or new unit being procured).
· The unit must have exact Macular Scan Pattern of 512x128 for retina scan with exact scanning speed at 27,000 A-scans a second within a 6mmx6mm. Must be able to provide on Company Literature
· The unit must have exact scan pattern of 200x200 in a 6mmx6mm cube for Glaucoma Scan with exact scanning speed at 27,000 A-scans a second. The scan pattern must be able to capture and provide RNFL, ONH, Neuro Retinal Rim Tissue, and 3-D Cube capabilities all in one scan Scanning speeds: Scan at exactly 27,000 A-scans/sec, as well as the ability to scan at 68,000 A-scans/sec
· Unit must be able to provide a report for Ganglion Cell Analysis that is derived from a scan must be comprised of 6mmx6mm cube scan with a minimum by 200x200 line scan pattern with no more than 30 microns between each slice.
· Minimum Pupil Requirements must be 2.0mm or less.
· Unit must utilize an LSO Fundus Camera (Line Scanning Laser Ophthalmoscope).
· The unit must Automatically Track to Prior Scan: The unit must be able to do this without technician needing to align to prior scan: Anterior Segment scan that is at minimal 15.5 mm in length x 5.8 mm in depth. which is the full view of the Anterior Chamber. Must be able to provide on Company Literature.
· Global Pachymetry report that is comprised of minimum of 24 radial scans and must be at a minimum 9mm in length.
· The unit must be a 90-degree configuration, so that the tech (operator) must sit to the side (90 degrees) of the patient.
e. Additional Technical Specifications that also MUST be met for consideration: see below
· OCT Imaging: Methodology – Must be Spectral domain OCT
· Optical source - Super luminescent diode (SLD), 840 nm
· Scan speed - 27,000 A-scans per second or 68,000 A-scans per second*
· All existing scan patterns in version 6.5 software will run at 27,000 A-scans per second, which is the same scan speed as the CIRRUS 4000.
· All patient data from the Cirrus 4000 instrument can be transferred into the Cirrus 5000 seamlessly and used immediately for Guided Progression Analysis or any other Cirrus analytical program
· A-scan depth - 2.0mm (in tissue), 1024 points
· Axial resolution - 5 um (in tissue)
· Transverse resolution - 15 um (in tissue)
· Cube scan pattern - 512 x 126 with over >67 million data ports and 47um spacing between lines
· Cube scan pattern - 200 x 200 with over >40 million data ports and 30um spacing between lines.
f. Additional FUNDUS IMAGING SPECIFICATIONS THAT MUST BE MET FOR CONSIDERATION:

g. Additional Salient Characteristics that must be met for consideration:
· Dicom system compliance
· Requires exceptional visualization in testing results
· Reliable Optical coherence measurements of optic disc retinal nerve fiber layers
· Reliable Macular Optical coherence measurements
· Must be able to track and compare RNFL thickness and ONH measurements over time to determine if significant change has occurred using Zeiss Style Guided Progression Analysis (GPA)
· Must include Noninvasive Angiography, able to obtain OCT Angiography in just one scan
· Angiography must include Real time eye Tracking
· Angiography must include Tracking to Prior images to help with precise location imagery
· Precise measurement application
· Imaging results must also reach deeper tissues like the choroidal tissue.
· Needs to be able to discover, track, and analyze single pathological events from multiple points of view for greater certainty
· Needs to examine retinal detail at the pixel level, and in a deeper choroidal tissue for consistent comparisons
· Needs to perform reproductive analyses (algorithms measure and display layers for tissue targeting, segmentation, and producible measurements)
· Needs to perform progression analysis of retinal nerve fiber layer such as in glaucoma suspects and glaucoma patients.
· Needs OCT angiography capabilities
· Needs to automatically store and deliver each patient historical testing data to provide accurate results in change assessments in the form or Zeiss HD Cirrus 5000 with Angio testing results as used in our current Zeiss HD Cirrus 5000 unit.
· Each Cirrus testing image must be able to be registered to prior visits and testing, for measurement of point to point changes in macular Thickens
· Must include FastTrac type Retinal tracking system/image capturing software that captures the same tissue every time a new image is obtained (helpful with patients who have difficulty holding gaze in place), but provides reliable progression results for the same tissue test type every time.
· Must include normative database of tests for Optic Nerve Head (ONH), Ganglion Cell/irl, Macular Thickness, Macular thickness change analysis, Optic nerve head progression changes to identify results outside the normal limits when compared to other previous Zeiss HD OCT 5000 testing results in our patient database.

3. DELIVERY AND INSTALLATION
3.1. Equipment shall be delivered to the main facility below:
EPVAHCS
Eye Clinic Section, 2nd floor Main Building 5001 N. Piedras El Paso, TX 79930
3.2. The contractor shall provide installation, start-up assistance and operator instructions and training.
3.4. The contractor shall provide in-depth training on use of system, maintenance of system, to eye clinic personal to include up to 15 staff (doctors and technician) with proficient time to allow for division into smaller groups of staff for hands on training, practice, and questions and answers at a minimum of 6 hours total training time.
3.5. The contractor shall provide TWO (2) hard-copy (paper) Operator’s Manuals. One for eye clinic department and one for biomed staff.
4. WORK HOURS:
4.1. Normal Work Hours: The service schedule shall be developed between the contractor and Contractor’s Representative (COR) prior to any service being performed.
4.2. The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday and any other day specifically declared by the President of the United States to be a national holiday.

7. EQUIPMENT OWNERSHIP.
7.1. Title to equipment shall remain with the contractor until installed and established. After completion, a satisfactory inventory and inspection is completed by Contractor, COR and Maintenance personnel. Upon approved inspection, title, equipment, accessories and ownership shall be released to EL Paso Veterans Affairs Health Care System (ELPVAHCS).
8. LIMITED WARRANTY.
8.1. All equipment listed to the attached quote, shall be fit and sufficient for the purpose intended as set forth in the user manuals; and merchantable, of good quality and free from defects in materials or workmanship; for a period of one (1) year from the date of the first invoice under this agreement. The contractor shall provide a minimum of one (1) year warranty. The warranty will begin upon date of installation.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(End of Clause) C.4 VAAR 852.219-10 VA NOTICE OF TOTAL SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS SET-ASIDE (JUL 2016)(DEVIATION)
(a) Definition. For the Department of Veterans Affairs, “Service-disabled veteran-owned small business concern or SDVSOB”:
(1) Means a small business concern:
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans or eligible surviving spouses (see VAAR 802.201 Surviving Spouse definition);
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans (or eligible surviving spouses) or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran;
(iii) The business meets Federal small business size standards for the applicable North American Industry Classification System (NAICS) code identified in the solicitation document;
(iv) The business has been verified for ownership and control pursuant to 38 CFR 74 and is so listed in the Vendor Information Pages database, (https://www.vip.vetbiz.gov); and
(v) The business will comply with subcontracting limitations in 13 CFR 125.6, as applicable
(2) “Service-disabled veteran” means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
(b) General.
(1) Offers are solicited only from verified service-disabled veteran-owned small business concerns. Offers received from concerns that are not verified service-disabled veteran-owned small business concerns shall not be considered.
(2) Any award resulting from this solicitation shall be made to a verified service-disabled veteran-owned small business concern.
(c) Agreement. A service-disabled veteran-owned small business concern agrees that in the performance of the contract, the concern will comply with the limitation on subcontracting requirements in 13 CFR §125.6.
(d) A joint venture may be considered a service-disabled veteran owned small business concern if the joint venture complies with the requirements in 13 CFR 125.15, provided that any reference therein to SDVO SBC is to be construed to apply to a VA verified SDVOSB as appropriate.
(e) Any service-disabled veteran-owned small business concern (non-manufacturer) must meet the requirements in FAR 19.102(f) of the Federal Acquisition Regulation to receive a benefit under this program.
(End of Clause) C.5 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING—MONITORING AND COMPLIANCE (JUL 2018)
(a) This solicitation includes .
(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.
(c) All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.
(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

C.6 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001;
(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests;
(4) Invoice payment has the meaning given in FAR 32.001; and
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
