Questions for VA Sonny Montgomery Reference: 5.3 Installation The contractor shall coordinate the phasing schedule with the GVSMVAMC Pharmacy POC, and shall submit a phasing schedule in writing to the GVSMVAMC Pharmacy POC for approval two (1) week prior to the start of any work.
Question: Does the Government accept a mutually agreed upon project schedule with dates?
Response: Yes Reference: 5.5 Installation The contractor shall verify that the medication refrigerators are seamlessly connected and integrated with existing BD Pyxis Medication Management and meets all the requirements in the contract through demonstration and validation prior to any sign off and/or acceptance of the system.
Question: Will the infrastructure be in place with power for the refrigerator and floor space allocated next to the MedStation for the Refrigerator?
Response: Yes Reference: 6.2 System Support:
Contractor will provide a call back response for any needed repair(s) of the system within four (4) hours of a call from the VA during the normal working hours of 7 a.m. – 5 p.m. Central Standard Time, Monday through Friday, for one (1) year following the date of acceptance of the system.
Question: Does customer accept that the Technical Support Center (TSC) is available 24/7/365 for immediate remote assistance and that during the initial customer call, the TSC Agent will reschedule a mutually agreeable call back time with the customer should the issue not be resolved during the initial call?
Response: Yes Reference: 6.3 System Support:
Contractor will be available for any repair(s) of the system and provide repair(s) of the system on site twenty-four hours per day and three-hundred and sixty-five (365) days per year for one (1) year following the date of acceptance of the system.
Question: BD offers limited three-month warranty, including repair and onsite support 24/7, on the Pyxis Refrigerator, however with the purchase of a Support Plan, the customer will continue to receive post-warranty product support. Without a support plan, onsite support is provided 24/7/365 at then-current T&M rates. Is this acceptable or would the customer like to contract for a support plan?
Response: Yes Reference: 6.4 System Support:
Contractor will conduct one (1) preventive maintenance visit for each unit during the one (1) year period following acceptance of the system.
Question: Does the customer accept that preventative maintenance is not included in the three-month warranty, but is included in a support plan, which can be purchased at a later time, and in accordance with the then-current preventative maintenance schedule?
Response: Yes
Reference: 6.5 System Support:
Service reports, along with, quarterly aggregate reports, shall be emailed or mailed or faxed to GVSMVAMC’s Chief of Biomedical engineering or designee.
Question: Does customer accept that although service reports and quarterly aggregate reports are not generally provided, the customer may request custom reports, subject to additional charge, by contacting a TSC agent?
Response: Remove reference 6.5 system support Reference: 6.6 System Support:
The VA shall have general administration rights to the hardware and software of the entire system, including, but not limited to, any diagnostic software features. The contractor shall brief the VA engineering POC on any software upgrades and/or changes prior to making any upgrades and/or changes, and the VA engineering POC must agree to any software upgrades and/or changes prior to any upgrade or installation. The contractor shall provide and install manufacturer recommended software upgrades and changes at no additional charge during the warranty and/or contract time period.
Question: The software component of this requirement is not applicable as there is no software directly in the Pyxis Refrigerator. Will the customer remove this requirement?
Response: Yes, remove requirement Reference: 6.7 System Support:
For any repairs or services that will be performed during normal working hours, the contractor’s service representative will report to the GVSMVAMC Pharmacy POC or designee upon arrival. Upon completion of the work, the vendor’s service representative must report in person to the Point of Contact and must present a copy of his/her field service report signed by the service using the equipment. This report must reflect date and time of service, name of company, and the name of the vendor’s service representative. At a minimum, this report must contain a detailed description of any services or repairs performed and identification of the units serviced. It must include a listing of replacement parts, when applicable. The report will also include the vendor’s recommendations necessary to maintain the equipment in best operating condition. Preventive maintenance procedures followed should be thoroughly documented (step-by-step) on the service report.
Question: For any on-site support provided, the customer will be informed upon arrival. Upon completion, the customer will sign documentation to acknowledge acceptance of the work performed and a receipt will be provided. Customer receipt will include a list of the equipment serviced, a description of the service completed, date and time of the work performed, and the name of the representative who provided the service. Upon request, additional information regarding the on-site visit may be provided if available. Is this acceptable to the customer?
Response: Yes Reference: 9.1 Packaging All items shall be marked clearly with the order number, obligation number, and delivery location within the hospital (e.g. warehouse).
Question: Does the Government accept that we provide the Shipping contact name, sales order, and device name.
