Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C256-19-AP-1589

1. Contracting Activity: Department of Veterans Affairs, VISN 16, Central Arkansas Veterans Health Care System (CAVHCS) 2200 Fort Roots Drive North Little Rock, 72114 598-19-2-784-0092

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This request is to purchase 15 BQ900 Haag-Streit Slit Lamps.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Haag-Streit BQ900 slit lamps are used for patients’ funduscopic examination. The BQ900 is primarily utilized for slit lamp microscopy and can be magnified to many levels for clear images of various ocular disorders. The current slit lamps that are being utilized by the Central Arkansas Veterans Healthcare System is Haag-Streit. Fifteen (15) of the CAVHS forty-eight (48) slit lamps have exceeded their life cycle expectancy and must be replaced. In an effort to maintain consistency throughout the Medical Centers and CBOCs and not compromise patient care, the same brand is required to ensure continuity for all ophthalmologist staff. It would not be cost effective to replace all of the Medical Center’s slit lamps at this time.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted, and it has been determined that there are multiple SDVOSBs and at least two (2) small businesses that are capable of meeting the needs of this requirement.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
