DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C256-19-AP-3175

1. Contracting Activity: Department of Veterans Affairs (VA), Network Contracting Office 16, 2002 Holcombe Boulevard, Houston, Texas, 77030.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Michael DeBakey Medical Center (MEDVAMC) located in Houston, Texas, Medical Care Line Program Office needs thirty (30) 10 each 6FR Angio-Seal VIP 0.035 Wire to treat patients who experience symptoms of paroxysmal atrial fibrillation that require procedures to normalize the heartbeat. The 6Fr Angio Seal Is used in surgical procedures involving certain types of muscular ventricular septal defects when surgery is believed to be patient "high risk" based on anatomical conditions and/or based on overall medical condition. The Angio Seal is compatible with the interventional cardiology system owned and operated by MEDVAMC.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: MEDVAMC is currently using cardiology procedure systems that requires Terumo Angio-Seal VIP 0.035 for treatment of patients. Use of equivalent brands will required additional testing by VA medical personnel which will require additional resources to ensure execution of safe and effective patient surgical procedures which will effect patient safety by delaying care to the patients. In the event another brand was used the whole system would have to be replaced which is cost prohibitive, cause further delays in patient care, deter patient safety, and increase risk of staff error with equipment operational differences.

4. Description of market research conducted and results or statement why it was not conducted: A Sources sought was posted on 6/24/19 with responses due on 7/3/19. Only responded to the notice proposing a brand name Angio Seal.

An intent to sole source to was posted on 7/9/19 with a closing date of 7/15/19 with an archival date of 9/7/19. No responses were received. As part of Market Research, the following prices were surveyed as follows:

This requirement will be solicited from a single source pursuant to FAR 13.106-1 (b): soliciting from a single source and IAW VAAR 813.106 (a) pursuant to 38 U.S.C 8127(b):

use of Noncompetitive Procedures for Certain Small Business Contracts to VHSS as they are the only SDVOSB source that can provide this requirement per market research.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b) (1). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
