SCOPE OF WORK AND GENERAL SPECIFICATIONS
For Tile and Grout Restoration At Alexandria VA Health Care System
Pineville, La. 71360
1. Background: Alexandria VA Health Care System in Pineville, Louisiana, requires restoration of tile and grout in specified restrooms and showers throughout the Medical Center. The contract is to provide Tile and Grout Restorations Services to 11,333 Sq. Ft. of flooring in the public restrooms, lockers, and showers located at the Alexandria VA Healthcare System, Pineville, LA. The contractor shall provide the services to include all labor, materials, collection and removal of debris associated with the tile and grout restoration services within the master schedule identified in the Performance Work Statement (PWS) and meeting all minimum requirements specified herein.
2. Project Scope: The contractor shall restore the tile and grout in the rooms identified below:

3. Requirement Overview:
• Contractor shall furnish to the Government all necessary labor, supervision, material, equipment, and services necessary to restore tile and grout to a “Like New” condition using Permatect’s proprietary processes and chemistry or a product considered equal in quality meeting all salient characteristics identified below.
• The contractor shall adhere to Executive Order 13514 - Green Procurement Requirements - which requires that 95% of new contract actions including task and delivery orders, for products and services are energy-efficient, water-efficient, bio-based, environmentally preferable, non-ozone depleting, contain recycled content, or are non-toxic or less toxic alternatives, where such products and services meet agency performance requirements. Contractor to provide the processes and chemistry to remove, replace, and restore all tile and grout to “Like New” conditions. This product shall be inorganic with an oil, urine and water repellant compound with sufficient particle size of the emulsion and surfactant package to allow deep penetration of porous tiles substrates, and must have an adhesion mechanism which is both mechanical and chemical, and must carry a (4) four-year warranty against excessive wear, yellowing and de-lamination.
4. Coating Removal:
• Existing Polymer coatings will be removed using Permatect’s DCR (Durable Coating Remover) or a product proposed in the contractor’s quote considered by the Government to be of equal or greater quality based on meeting all the salient characteristics below. The contractor shall ensure 100% of the existing coating is removed to achieve successful bonding with new coatings.
• Salient Characteristics of durable coating remover are as follows: The contractor shall adhere to Executive Order 13514 - Green Procurement Requirements - which requires that 95% of new contract actions including task and delivery orders, for products and services are energy-efficient, water-efficient, bio-based, environmentally preferable, non-ozone depleting, contain recycled content, or are non-toxic or less toxic alternatives, where such products and services meet agency performance requirements. Removal of existing floor coatings to achieve successful bonding with new coatings.
5. Grout Restoration/Color Seal:
• Grout lines will be chemically and mechanically cleaned using a specialized cleaner that is specifically targeted to profile grout surface creating an anchor pattern for bonding with. The Grout Lines are then color sealed with Permatect’s “Channel Changer or a product proposed in the contractor’s quote considered by the Government to be of equal or greater quality equal in the description to all salient characteristics identified below to achieve consistency in appearance. A clear water floor rinse shall be applied to the floor and residue removed by suction.
• Salient characteristics for Grout Restoration/Color Seal are: Executive Order 13514 - Green Procurement Requirements - which requires that 95% of new contract actions including task and delivery orders, for products and services are energy-efficient, water-efficient, bio-based, environmentally preferable, non-ozone depleting, contain recycled content, or are non-toxic or less toxic alternatives, where such products and services meet agency performance requirements. Grout lines will be chemically and mechanically cleaned using a specialized cleaner that is specifically targeted to profile grout surface creating an anchor pattern for bonding with. Grout Lines are to be color sealed. A clear water floor rinse shall be applied to the floor and residue removed by suction.
• Missing grout shall be replaced with Permatect’s Channel Changer Grout replacement or a product equal in the description to all salient characteristics identified below.
• Salient characteristics for missing grout are: Executive Order 13514 - Green Procurement Requirements - which requires that 95% of new contract actions including task and delivery orders, for products and services are energy-efficient, water-efficient, bio-based, environmentally preferable, non-ozone depleting, contain recycled content, or are non-toxic or less toxic alternatives, where such products and services meet agency performance requirements. Replace missing grout with grout replacement for Coating to adhere for a high gloss appearance when dried.
6. Coating:
• Contractor shall apply interlocking siloxane inorganic protective barrier. All tile and grout shall be sealed with two coats of an Inorganic Siloxane based Permanent Barrier equivalent to Permatect’s “Proprius One” or a product equal to provide impermeable seal over the entire treated area. If an equivalent product is used, it must be included in the contractor’s quote and considered acceptable by Government meeting or exceeding all salient characteristics identified as follows.
• Salient characteristics for the protective barrier include: This shield shall have high gloss shine appearance when it is dried. This product shall be inorganic with an oil, urine and water repellant compound with sufficient particle size of the emulsion and surfactant package to allow deep penetration of porous tiles substrates, and must have an adhesion mechanism which is both mechanical and chemical, and must carry a (4) four-year warranty against excessive wear, yellowing and de-lamination.
7. Downtime:
• For all work outside the regular business hours 4:30pm – 6:00am, Monday through Friday, and weekends, the Contractor must provide written notification to the COR and the COR will schedule and provide written authorization in advance of the proposed work activity. If weekend work is required, it shall be coordinated with the COR and VA Police service.
• Patient areas will be coordinated with nurse managers.
• Affected work areas may not be immediately available and the Contractor must provide flexibility in the schedule.
8. Warranty and Support:
• The warranty and support must cover all components of the requirement in accordance with standard manufacturer’s warranty.
• Service and process must carry a 4-year warranty that covers excessive wear, yellowing and delimitation.

10. Installation:
All work and restoration will be coordinated with the COR. The COR will coordinate with the appropriate OIT and Biomedical Engineering groups. Phasing and work schedule will be provided and coordinated with the COR. A detailed installation schedule will be discussed and agreed upon during a project implementation kick-off meeting. The macro level phasing and completion schedule for the overall project is shown below. All work must be scheduled, performed, inspected and acceptance prior to the final completion date shown below:
With an average of five (5) bathrooms completed per week, 50 percent of the contracted restrooms should be completed in a 10-week timeframe. The remaining should be completed in another 10-week timeframe. The entire contract should encompass no longer than a five (5) month period from the date of award.
The vendor will confine operations (including storage of materials) on Government premises to areas authorized and approved by the Contracting Officer and/or COR. The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor’s performance. Working space and space available shall be as determined by the COR.
11. Operations and Storage Areas:
The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized and approved by the Contracting Officer and/or COR. The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
Working space and space available for storing materials and/or staging personnel and equipment shall be as determined by the COR.
The contractor shall remove all debris and trash resulting from these renovation services on a daily basis. The debris and trash shall be removed by contractor personnel using contractor provided containers with sealed lids to ensure all trash and debris is safely removed from the medical center without any spillage. The contractor is responsible for any temporary storage needed of the daily trash and debris (outside of the buildings) at locations as coordinated and approved by the COR and/or CO. Temporary outside debris and trash containers must be promptly emptied and/or removed from medical center property when full. All containers must be removed within seven (7) working days of final project completion unless otherwise directed by the COR and/or CO
Workmen are subject to rules of the MEDVAMC applicable to their conduct. All personal automobiles and contractor trucks shall be parked at the contractor's staging area or offsite of the MEDVAMC premises.
Execute work so as to interfere as little as possible with normal functioning of the MEDVAMC as a whole, including operations of utility services, fire protection systems and any existing equipment, with work being done by others. Do not store materials and equipment in other than the designated contractor storage areas.
Daily, the Contractor shall keep all work, storage, and staging areas clean and neat. Contractor shall provide sufficient trash containers so that there is no debris lying around.
12. Protection of Existing Structures, Equipment, Utilities, and Improvements:
The Contractor shall preserve and protect all structures and equipment on or adjacent to the work site. The Contractor shall replace at his own expense damage to such items to the satisfaction of the Contracting Officer.
Contractor shall take all measures and provide all materials necessary for protecting and preserving existing equipment and property in affected areas of installation against dust, debris and physical damage, so that equipment and affected areas to be used in MEDVAMC operations will not be hindered. Contractor shall permit access to VA personnel through installation areas as required for maintenance and normal MEDVAMC operations.
When the installation area is turned over to Contractor, Contractor shall accept entire responsibility therefore. Contractor shall maintain in operating condition, existing fire protection, alarm equipment and other operating equipment in the installation area. IT IS VERY
IMPORTANT THAT ESSENTIAL AND LIFE SAFETY SYSTEMS BE CONTINUOUSLY
MAINTAINED AND NOT INTERRUPTED WITHOUT TWO WEEKS PRIOR WRITTEN
NOTICE AND APPROVAL FROM THE VA MEDICAL CENTER.
13. Sign-in Procedures:
All Contractor workers are required to sign in and out at the VA Police Dispatch at the corresponding locations as directed by the COR, or designee at each facility. A valid state driver’s license or state identification card is mandatory for all employees to have access to these facilities. All contractor employees are required to wear the assigned VA badge at all times.

14. Material Safety Data Sheet (MSDS):
Contractor shall provide three (3) copies of each Material Safety Data Sheet for every product, chemical, etc. used on this project. MSDS sheets shall be provided for any material on the same day those materials arrive on VA property. At no time shall the Contractor have, or permit subcontractors to have, materials on station without MSDS sheets. All instructions for use shall be followed. Products will not be used until MSDS's are submitted to the COR.
The contractor shall maintain a current, green in color, loose-leaf notebook on the job site at all times, which is readily available for viewing by the COR or VA Safety Officer.
15. Work Hours:
Normal business hours are 7:30am to 4:30pm Monday through Friday excluding Federal Holidays.
For all work outside the regular business hours 4:30pm – 6:00am, Monday through Friday, and weekend, the Contractor must provide written notification to the COR and the COR will schedule and provide written authorization in advance of the proposed work activity. The VA requires that information submitted must contain: extent of work, workers involved, the affected areas, and estimated times of operation.
Requests for after hours work must be submitted in writing to the COR and CO. Coordinator two
(2) weeks prior to work. The VA requires that information submitted must contain: extent of work, workers involved, the affected areas, and the estimated times of operation.
All materials, tools, equipment, and supplies shall never be left unattended within work areas.
When working in common areas, there shall be a minimum of 2 individuals in the proximity of the work area to ensure that all materials, tools, equipment, and supplies are not accessible to patients or others.
16. ID Badges:
All workers are required to obtain a time-limited I. D. badge from the VA Police Service located in the main building, room 1A-74. This badge must be worn at all times when workers are on site.
17. Contractors Certifications:
Contractors must be licensed/certified to complete work where required by Federal/State/Local regulations within the State of Louisiana. The Contractor shall provide proof of license/certification where required by Federal/State/Local regulations; and liability insurance.
Contractor must be an authorized, certified, licensed distributor, and installer of the products provided.
