STATEMENT OF WORK (SOW)

Jackson Patient Room Chairs

1. Contract Title: Jackson Case Goods

2. Background: The government requires new Seating for various inpatient rooms. This scope of work will be performed at the VA Medical Center Jackson MS.

This is a furniture requirement.

3. Scope. The project scope includes furniture for at the VA Medical Center. All furniture within project scope is to be installed by vendor.

The RFQ is for contract furniture and the vendor is responsible for transportation and installation of said product.

4. Specifications/Characteristics:

1. Furniture must have lifetime warranty

1. Furniture must be SCS Indoor Advantage Gold for Indoor Air Quality and FSC certified to substantiate the LEED Materials & Resources Credit 7 for Certified Wood and have BIFMA Level Certification

1. Upholstery must be commercially rated and meet NFPA 701 VA designer will select finishes with awarded vendor

1. Sleep chair from LZB. Needs to have all Silica on it. casters.

5. Delivery.

1. Delivery and installation scheduling will need to be coordinated with VA COR and end users.

1. Preferred form of communications is email.

1. Products will need to be scheduled for delivery once arrived and inspected.

1. The product delivery is 8 weeks after contract award

1. A project manager or project designer and installation foreman must be onsite during installation.

1. Secure VA Badge and security clearance for all contractors, scheduled through VAMC Human Resources.

1. Leave all work areas in clean and orderly condition.

1. Place of Performance – VAMC Jackson, MS or local leased space.

1. Period of Performance - The Contractor shall perform all work during normal business hours (Monday-Friday, 8:00 AM – 4:30 PM), unless otherwise stipulated by the COR.

1. Special Considerations. Vendor is required to have receiving paper work signed by Interior Designer after completing contracted services. If COR is not available, the vendor shall contact Engineering Department.

1. Install should be coordinated at least 2 weeks in advance with VA Interior Designer.

1. Contractor Furnished Materials. This term refers to all labor, materials, and equipment to perform the job. When "furnish," "provide,” "install," or a similar term is used, it shall mean complete installation, ready for use.

1. Smoking Policy. The Contractor shall not allow smoking in any building. Smoking is only permitted in designated outdoor areas, 25 feet away from buildings.

1. Items to be received and stored at vendor warehouse for up to 90 days due to project completions.

1. Delivery will be made on up to 10 different selected dates.
