Brand Name Justification for Single Source Awards IAW FAR 13.106-1

For Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K)

Acquisition Plan Action ID:__VA256-18-AP-4209__________

1. Contracting Activity: Department of Veterans Affairs, VISN 16, NCO 16, Alexandria VA Health Care

System, Pineville, LA. Purchase Request # 564-18-2-1099-0039 - Medivator Automated Endoscope Reprocessing Unit for the Veterans Healthcare System of the Ozarks, Fayetteville, AR.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The “Brand Name” Medivators Endoscope re-processor unit completely reprocess the endoscope which includes high level disinfection, cleaning and leak test. The Medivator re-processor is directly compatible to the Olympus 190 endoscopes which the medical center and CBOC currently has in use. The VHCS of the Ozarks is requesting 2 reprocessing endoscopes for the Springfield CBOC. The estimated value is $114,200.00.

3. Unique characteristics that limit availability to only one source and brand name, with the reason no other supplies or services can be used:

The scope currently in use at the medical center is the Olympus 190 endoscope that have been validated for that device (Medivators Endoscope Reprocessor). The reprocessing accessories are only compatible with the device and it includes chemicals and hookup devices (i.e. blocks).

Purchasing another “brand” of re-processors would require the medical center to purchase all new endoscope with accessories which would cost the medical over an estimated $200K. In addition, the medical center currently has in place an existing preventative maintenance contract for these Medivators Advantage processing device. The SPS technicians have valid SOP and competence training and using another device would require additional SOP competence to include validation and staff training. The reprocessing capacity for this device meets in-customer demand and finally the VISN 16 lead for SPS has mandated the standardization of reprocessing equipment which is the Medivator Endoscope equipment.

Using another “brand” that is not compatible with existing scope may result in system malfunction, additional replacement cost and the equipment unavailable to meet patient care needs.

4. Description of market research conducted and results or statement why it was not conducted:

Market research indicates that Medivator Inc is the only known manufacturer with limited distributors.
