B.2 PRICE/COST SCHEDULE

STATEMENT OF WORK

Provide all necessary labor, material, tools, and equipment to perform: semiannual maintenance/inspection, annual maintenance/inspection, and two-hour and four-hour load test—including associated certification, on emergency generators at the Department of Veterans Affairs Eastern Kansas HCS listed herein in accordance with all terms, conditions, provisions, specifications, and schedules of this solicitation.

SECTION C - DESCRIPTION/SPECIFICATIONS/WORK STATEMENT

1. GENERAL:

1.1 The work under this contract shall be performed under supervision of a Registered Professional Engineer. The Registered Professional Engineer shall be on site initially to discuss with the Contracting Officer’s Technical Representative (COTR) the procedures used to test equipment listed under this contract. The Registered Professional Engineer shall supervise the services personally, review the final report and discuss with the COTR the results of the services, any recommendations, future actions and professional opinions, which are contained in the final report.

1.2 The Supervising Service Engineer must be accredited as a Certified Electrical Test Technologist by the NICET (National Institute for Certification of Engineering Technologists) or the NETA (National Electrical Testing Association) or similar certification and have a minimum of five (5) years of experience on similar major testing projects. Contractor shall provide proof of Supervising Service Engineer’s certification prior to the performance of work.

1.3 Field service work shall be performed directly by Field Service Engineer who has successfully demonstrated their knowledge and experience in the Field of Electrical Power Distribution Systems Testing. The Field Service Engineer shall be capable of the following: (1) testing, assessing, evaluating, servicing and reconditioning components, (2) assuring that the equipment on which work has been performed is safe, reliable and acceptable for its intended purpose, and (3) identifying defective equipment and potential safety problems, environmental hazards or code violations.

All other testing technicians assigned to the project must be certified by NICET, NETA, or must have an equivalent qualification certification. Contractor shall provide proof of technicians’ certification prior to the technician performing work. Test assistants and apprentices may be assigned to the project as assistants to certified technicians at a ratio not to exceed two certified to one noncertified assistant or apprentice.

1.4 All work performed shall be fully documented by the Contractor. Testing shall be performed in compliance with manufacturers' recommendations and applicable NEMA, NETA, ANSI, ASTM, ASA, OSHA 1919.269, NFPA 70, NFPA 70E, NFPA 99, NFPA 110, JACHO and VA Standards.

Refer to the Comprehensive Accreditation Manual for Hospitals: The Official Handbook, Parts EC.7.40.1 and EC.7.40.7, as set forth by the Joint Commission on Accreditation of Healthcare Organizations (JACHO), for compliance requirement.

1.5 Conformity to Regulations: The Contractor shall conform to all regulations, Federal, State, and local, governing emergency generator electrical testing and certification.

1.6 The cost of travel and/or travel time to and from the job site will be at the Contractor’s expense, at no cost to the Government

1.7 Contractor shall submit maintenance/inspection form, load bank field test form, and final inspection/test report format to the COTR for approval prior to commencing testing. Testing shall not start prior to approval of form formats by the COTR.

1.8 Contractor shall provide a comprehensive written final report to the Engineering Service-line Manager, which summarizes: maintenance/inspection findings, field test findings, and provide a statement as to whether the Emergency Generator Systems reflect compliance or lack of compliance with codes and standards cited in paragraph directly below, list all anomalies found, and suggest recommendations to correct any noted deficiencies. Final report shall clearly state whether compliance occurred with respect to JACHO EC.7.40.1 and NFPA 110.

1.9 The contractor’s representative shall report to the Engineering Service Office, prior to commencing work and at the completion of each workday.

1.10 Contractor shall check in with the VA’s Engineering personnel each day before starting work to obtain a badge and authorization to perform service that day. The Contractor must wear the badge in plain visible site while on station and return that badge at the conclusion of each day.

1.11 Keys: The COTR shall provide the Contractor access to all buildings and rooms required in the provisions of this contract. The Contractor shall notify the COTR when leaving these areas.

1.12 Lights/Windows: All lights and/or windows shall be turned off or closed and locked respectively in unoccupied areas, unless otherwise directed by the COTR. If any difficulties are encountered in keeping areas locked, or windows closed, the COTR shall be notified immediately.

1.13 All materials and equipment will be removed from the facility or stored properly at the end of the workday and secured during the workday. Under no circumstances will the Contractor allow any equipment to be left unattended for any length of time.

1.14 Smoking is permitted only in those specific areas designated for smoking.

1.15 All service tickets will show the Make, Model, Serial, and PM numbers of the generator and prime mover being tested.

1.16 Time Constraints:

1.16.1 Maintenance, inspection and testing shall be performed between 8:00 a.m. and 4:00 p.m., Monday through Friday, excluding federal holidays. Due to the critical nature of emergency generators serving health care buildings, testing of generators shall not occur should stormy weather conditions exist.

1.16.2 Topeka: Testing and maintenance by Contractor of generator, located in Building 54, will only be performed with the concurrence of Surgery Service. Allowable time may be after-hours or on weekends. Maintenance supervisor will help in establishing time for the testing and maintenance of this generator.

2. MAINTENANCE AND INSPECTION WORK: Provide all necessary labor, material, tools and equipment to perform maintenance on emergency generators designated in this section. The work described herein is to be performed at multiple sites. Read Scope of Work carefully.

2.1 Semiannual Maintenance and Inspection:

Semiannual maintenance shall to be performed on all emergency generators as described in this portion of the statement of work and designate in the tables of section 2.3.

(a) ENGINE LUBRICATING SYSTEM:

1. Oil analysis to check for the presence of metals, water, or breakdown of engine oil.

2. Change oil, filter, and by-pass filter (if sufficient hours). Oil and filter to be furnished by contractor.

3. Clean crankcase breather.

(b) FUEL SYSTEM:

1. General inspection of all components.

2. Change or clean air filter (filter to be furnished by contractor).

3. Change fuel filter (filter to be furnished by contractor).

(c) GOVERNOR:

1. Check all linkages and ball joints.

2. Clean hydraulic oil.

3. Observe for oil leakage.

(d) COOLING SYSTEM:

1. General inspection of all components.

2. Clean exterior of all radiators.

3. Check condition of all hoses and connections, replace as needed.

4. Adjust and replace belts if necessary.

(e) GENERATOR:

1. Check brush length and pressure.

2. Check appearance of slip ring and clean if necessary.

3. Blow out with clean dry compressed air.

(f) SERVICE BATTERIES:

1. Clean and check connections.

2. Make specific gravity test on lead acid battery.

3. Make load test of each nickel cadmium cell where applicable.

4. Add distilled water to maintain proper level.

5. Check charging rate.

6. Battery trickle charging circuit operating properly.

(g) GENERATOR OPERATION:

1. Run generator under load to check instruments including voltage.

2. Check automatic shutdown and transfer of load from commercial power to emergency power.

3. Check automatic shutdown and transfer of emergency power to commercial power.

4. Check running cool down time.

(h) REPORTING REQUIREMENTS:

1. Provide inspection report to the Chief of Engineering Services, or his designee, advising of any conditions warranting services or other recommendations.

2.2 Annual Maintenance and Inspection:

Annual maintenance shall to be performed on all emergency generators as described in this portion of the statement of work and designate in the tables of section 2.3.

During the month of October, Contractor shall perform the following maintenance services:

(a) FUEL SYSTEM:

1. General inspection of all components.

2. Change fuel filter, (Filter to be furnished by contractor).

3. Change or clean air filter (filter to be furnished by contractor).

4. Analyze diesel fuel for condition, (request fuel be replaced if required).

5. Check entire unit for leaks.

6. Adjust valves and injectors.

(b) LUBRICATING SYSTEM:

1. Change oil and by-pass filter (oil to be furnished by contractor). Oil must meet unit manufacturer’s recommendation.

2. Change oil filter, (Filter to be furnished by the contractor).

3. Check entire unit for oil leaks.

4. Clean crankcase breather.

5. Lubricate all parts as required by manufacturer’s service manual.

6. Operation of lubricating oil heater (if installed).

7. Check oil pressure for proper pressure.

(c) COOLING SYSTEM:

1. Check condition and rod out heat exchange if necessary.

2. Change antifreeze solution (antifreeze to be furnished by contractor). Antifreeze to be changed on even numbered years (every other year).

3. Cooling system will be protected to –20 degree Fahrenheit.

4. Clean exterior of all radiators.

5. Check all engine water pumps and circulating pump.

6. Examine all ductwork for looseness.

7. Clean and check motor operated louvers.

8. Check condition of all hoses and connections. Replace as needed (to be furnished by contractor).

9. Adjust and replace if necessary all belts (to be furnished by contractor).

(d) SERVICE BATTERIES:

1. Clean and check connections.

2. Make specific gravity test on lead acid battery.

3. Make load test of each nickel cadmium cell where applicable.

4. Add distilled water to maintain proper level.

5. Check charging rate.

6. Battery trickle charging circuit operating properly.

(e) EXHAUST SYSTEM:

1. Check condition of muffler; exhaust lines, supports and connections.

2. Check for repair, as needed all exhaust leaks.

3. Check and exhaust manifold and turbo-charge cap screws.

(f) GENERATORS:

1. Clean generator winding.

2. Check generator bearing.

3. Measure and record resistance reading of generator winding using insulation tester (Megger).

4. Inspect transfer switch and make repairs or replacement if indicted.

5. On main switchgear and generator switchgear operate every circuit breaker manually; visually check bas bar, and feeder connection for cleanliness and signs of overheating.

6. No unusual sounds.

7. Vacuum inside the transfer switched and generators control panels.

8. Check brush length and pressure.

9. Check appearance of slip rings, if necessary.

10. Blow out with clean dry compressed air.

(g) SYSTEMS CONTROLS:

1. Re-evaluate the setting of the voltage sensing and time delay relays.

2. Check out and record the time intervals of the various increments of the automatic start-up and shutdown sequences.

(h) ENGINE SAFETY CONTROLS:

1. Check operation of all engine operating alarms and safety shutdown devices (general will not be under load during this check).

2. Run generator under load, to check instruments including voltage, frequency, amperage, and make the necessary adjustments.

3. Check automatic transfer switch for proper operation, check operation. check contracts, trickle charger system, voltage at contractor, phase protection relays and start relays.

4. Tune engine as required by manufacturer’s service manual.

(i) GOVERNOR:

1. Check all linkage and ball joints.

2. Check oil level, if applicable.

3. Observe for oil leakage.

ANNUAL 2-HR LOAD BANK TESTING:

Annual load bank testing is to be performed on all emergency generators operating under 30% of their nameplate rating as described in this portion of the statement of work. Provide all necessary labor, material, tools and equipment to perform load bank testing on generators designated in tables of this section.

3.1 The VA will provide switching to isolate generators and assist with connections at the load test terminals.

3.2 In accordance with NFPA 110 Standards for Emergency & Standby Power Systems, 2002 Chapter 8 Routine Maintenance and Operational Testing, the service technician will hook external load bank at generator breaker. The technician will then place a dynamic and/or static load on each generator of 25% of nameplate KW rating for 30 minutes followed by 50% of nameplate KW rating for 30 minutes, followed by 75% of nameplate KW rating for 60 minutes for two continuous hours of testing. The technician will also check all safeties on each generator.

3.3 Individual emergency generator report forms are to indicate initial start-up time, load at each phase of load bank testing, and total run-time. For the 25% and 50% load bank testing phases, not less than three reading are to be taken and entered onto the test form. For the 75% load bank test, the initial reading is to be recorded with reading taken at 15-minute intervals thereafter. Test form is to delineate each phase of testing. Initial load bank test start and final stop times are to be shown on the test form. The technician’s signature, along with date each emergency generator was tested, is to appear on test form. Failure to provide test information, recording the minimum test loads levels, and testing intervals for less than the prescribed durations on the report form will require retesting of the generator(s) in question by the Contractor at no expense to the government.

3.4 Failure to provide a load bank of sufficient size, for generators list in tables below, and to test each emergency generator at or above KW loads for the prescribed time intervals cited in paragraph 3.2, shall result in Contractor performing the test again at no additional charge to the VA.

3.5 In the event that a generator, generator engine, or associated emergency power equipment failure occurs during the 2-hour load test, JACHO (article EC.7.40.7 of the accreditation manual) requires that generating unit be retested after necessary repairs or corrections are made. Unit pricing is to be provided with contractor’s proposal for retesting of a generating unit, should failure occur during testing, under the following scenarios:

3.5.1 Repairs and/or corrections can be made prior to demobilization from the job site, or

3.5.2 If demobilization/mobilization expenses are incurred, provide unit price to perform a retest of the failed generating unit.

3.6 Submit a complete test report to the Facilities Management Service-line Manager, advising of any conditions warranting services or other recommendations.

3.7 Load bank testing shall be performed annually, during the month of October, on the following emergency generators:

4. TRI-ANNUAL 4-HOUR LOAD TESTING OF EMERGENCY GENERATORS:

Contractor is to provide all necessary labor, material, tools and equipment to perform tri-annual load testing on emergency generators. Starting in October of the year 2010, and every three years thereafter, Contractor shall perform a 4-hour load test on generators listed in tables below.

4.1 The VA will provide switching to isolate generators and assist with connections at the load test terminals.

5. PRICING:

6.TESTING AND REPORT SCHEDULE:
6.1Start/Finish Dates: Contractor shall notify each medical center’s staff at least one week in advance of scheduling any proposed maintenance or testing. Medical Center staff shall approve proposed schedule for maintenance and or testing of emergency generators prior to commencing work. Contractor shall complete annual scheduled emergency generator testing, at each site, not more than one week from its starting date. Due to the combined duration of testing, that occurs tri-annually, this one-week testing completion restriction for testing of all generators at a given site is waived. Contractor shall jointly develop a comprehensive test schedule, with each medical center’s staff, that is acceptable to all parties.

Copies of generator field test form, completed each business day, shall be provided to the COTR at the location in question. All maintenance and inspections reports are due within two weeks of completing maintenance of the last generator at each site. All emergency generator field test form submissions and final test report are due within 30 day of testing completion at each site. Any deficiencies that are discovered during maintenance, inspection or testing, that might affect the reliable operation of any generator, are to be reported immediately to the Chief of Engineering at each site.

7. DAMAGE AND EQUIPMENT LOSS

All tasks accomplished by the Contractor personnel will be performed to preclude damage or disfigurement of government-owned furnishing, fixtures, equipment and architectural or building structures. Contractor will report any damage or disfigurement to these items when caused by the Contractor’s personnel and will perform repairs or replace government-owned equipment, fixtures, furnishings, grounds and architectural or building structures to their previously existing condition.

Contractor is required to provide proof of insurance which meets or exceeds the terms of FAR 52.228-5 and the Schedule of Insurance Coverage listed herein prior to the commencement of work at any VA Medical Center.

In accordance with FAR Subpart 28.307-2, Liability:

7.1Workers' compensation and employer's liability. Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
7.2General liability. (1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. (2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.
8.INTERFERENCE TO NORMAL FUNCTION

Contractor’s personnel are required to interrupt their work at any time to avoid interference with patient care procedures and the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts.

9. VEHICLE SECURITY: While performing under this contract, all vehicles used by the contractor shall be locked and the keys removed when not in use. This is intended to protect the contractor’s property and the safety of the patients at the medical center.

10. PAYMENTS: Payment will be made after receipt of properly prepared invoices, provided that the following has been met:

10.1 Only invoices will be certified for payment. Carbons or photocopies will not be accepted.

10.2 In order to administer this contract, it is mandatory that all technicians report to the respective Facilities Management office, with the work/repair ticket after the work have been completed. This procedure is necessary to insure that internal accountability is maintained for all repairs. Failure to follow this procedure may jeopardize the payment of maintenance work and the contractor’s technician who ignores this procedure does so at his/her own risk.

11. SAFETY REQUIREMENTS: In the performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or his designee may determine to be reasonably necessary to protect the lives and health of the occupant of any building. The Contracting Officer or his designee will notify the contractor of any noncompliance with the foregoing provisions and action to be taken. After receipt of such notice, the contractor shall immediately correct the conditions to which attention has been directed. Such notice, when served on the contractor, his representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or any apart of the work and hold the contractor in default as provided for elsewhere in this contract.

The contractor shall provide to the COR and the contracting officer a quote to perform repairs before performing repairs for SUB-CLINS 0002 AA, 0003 AA, 0004 AA, 0005 AA, 0006 AA, 0007 AA, 0008 AA, 0009 AA. Only the contracting officer may authorize services under these Sub-clins. A modification to the task order shall be executed for these repairs.

Note 2: The contractor shall provide labor rates and discounts to those rates as an attachment to the Request for Quotes that will become a permanent part of the awarded.
