Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity:
Department of Veterans Affairs, St. Louis VA Medical Center, 915 N. Grand Blvd., St. Louis, MO.
63106 -1621 and NCO 15 3450 S. 4th Street Leavenworth, KS 66048.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The STERIS/AMSCO Innowave PCF Ultrasonic equipment and accessories will be used for cleaning and disinfecting all SI robotic endowrist devices used on patients undergoing robotic urology surgical procedures in the Operating Room.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
STANDARDIZATION: The facility is currently using numerous Steris equipment inhouse to include 5 Steris Steam sterilizers and other Steris equipment. The facility has maintenance and repair contract with Steris Corp who will also provide these services to include annual and as needed training/services for the seasoned and new SPS personnel. The standardization significantly reduces the maintenance expenses, testing material, detergent expenses, quality control process, and inventory management by having the same equipment brand.
4. Description of market research conducted and results or statement why it was not conducted:
Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
