3. INVOICES:
a. Invoices shall be submitted in arrears in accordance with: 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)

c. The VA has mandated electronic invoice submission to the Veterans Affairs Financial Services Center (VAFSC). VAFSC has partnered with Tungsten Network e-Invoicing network, for submissions of all electronic invoices to VA. Tungsten Network electronic invoicing is free to all VA vendors. In order to submit electronic invoices, all VA vendors must register with Tungsten Network by submitting an email to VA.Registration@Tungsten-Network.com or calling 1-877-752-0900 option 2 for Enrollment.
d. Contractor shall submit an electronic invoice by the tenth (10th) of the following month services were performed to the Veterans Affairs Financial Services Center (VAFSC) e-Invoice through the website at https://portal.Tungsten-Network.com/Login.aspx. For questions regarding the submission of VA electronic invoices, Tungsten Network customer service may be contacted at 1-877-489-6135.
e. All invoices shall reference the vendor name and address, customer name, contract number, appropriate obligation/funding order number, description of services provided, quantity, unit price, and total invoice amount (any additional info). Invoices shall also include any payment discount terms.
4. GOVERNMENT INVOICE ADDRESS (See SF1449 Block 18):
a. All invoices from the contractor shall be submitted electronically in accordance with 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)
b. Facsimile, e-mail, and scanned documents are not acceptable forms of submission for payment requests.
c. VA’s Electronic Invoice Presentment and Payment System – The Veterans Affairs Financial Services Center (VAFSC) uses a third-party contractor, Tungsten Network e-Invoice, to transition vendors from paper to electronic invoice submission. Please go to this website: http://www.tungsten-network.com/customer-campaigns/veterans-affairs-us/ to begin submitting electronic invoices, free of charge.
d. As of July 1, 2020, the VA10091 form has been eliminated and the VAFSC is requiring our vendors to update and add their information in the Customer Engagement Portal (CEP) at https://www.cep.fsc.va.gov/. This is for new vendors being added to FMS only.

5. ACQUISITION OF COMMERCIAL ITEMS:
This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 12 Acquisition of Commercial Items and FAR Part 13 Simplified Acquisition Procedures. FAR Part 15 will not be used. The procedures in Part 12 used in conjunction with Part 13 allow the Government to procure services resembling those in the commercial marketplace and streamline the procurement process. From this RFQ, the Government intends to award a firm fixed price contract where the contractor provides a price that is not subject to any adjustment on the basis of the contractor’s cost experience in performing the contract.
6. SITE VISIT:
It is strongly suggested and expected that the offeror inspect the campus to be serviced to fully understand the nature of the work and the conditions under which the work is to be performed. In no case shall failure to inspect the campus constitute grounds for a claim after contract award.
SITE VISIT IS SCHEDULED FOR MONDAY FEBRUARY 8, 2021AT 10:00 AM LOCAL TIME. INTERESTED PARTIES TO MEET COR BRENDAN MCLAUGHLIN (608-256-1901 X11157) AT FRONT ENTRANCE OF HOSPITAL. PLEASE BE PREPARED FOR COVID SCREENING UPON ENTERING BUILDING.
PLEASE NOTE: WHILE ON CAMPUS, ALL PERSONS WILL BE REQUIRED TO WEAR MASKS INSIDE ANY VAMC BUILDING AND WHILE TRAVELING IN VEHICLES FOR PURPOSES OF THE SITE VISIT.
7. RFQ QUESTIONS:
In order to maintain integrity of this solicitation and subsequent award date, all offerors are advised that any question and answer requests must be submitted in a written form via e-mail to the Contracting Officer SUSAN.PASHOLK@VA.GOV no later than TUESDAY FEBRUARY 9, 2021, AT NOON LOCAL TIME. Contracting will issue a consolidated response via solicitation amendment to https://beta.sam.gov Contract Opportunities within 2 business days to all parties
8. CONTRACTING OFFICER AUTHORITY:
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitment or issue changes that will affect price, quantity or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred thereof.
9. IDENTIFICATION, PARKING, SMOKING AND VA REGULATIONS:
a. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VA will not invalidate or make reimbursement for parking violations of the Contractor under any conditions.
b. All VA facilities are drug free.
c. All contractor personnel are required to adhere to all VA rules and regulations.
d. Smoking is prohibited on the entire campus, both inside and outside the VA Facility
e. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.
f. VHA DIRECTIVE 1192: This Veterans Health Administration (VHA) directive establishes policy and provides guidance for the prevention of seasonal influenza in VHA facilities through the vaccination or masking of health care personnel (HCP). AUTHORITY: Title 38 United States Code (U.S.C.) 7301(b), 7318(b). Contractor is responsible in ensuring that all aspects of this directive are implemented, including providing face masks and influenza vaccines for all employees working at a VA facility.
10. SECURITY & PRIVACY CONTROL:
The Authorization & Accreditation (A&A) requirements of VHA Handbook 6500.6 do not apply to this requirement, therefore, a Security Accreditation Package is not required. No A&A or MOU/ISA is required. Sensitive information exposure as part of this contract involves applicable security controls within the facility as part of the VA Information Security If there are any questions related to privacy, please have a member of your staff contact the VA Privacy Service at (202) 461-6309. Visitor Passes:
Issuance of visitor passes is the responsibility and at the discretion of each Facility Director. Visitor passes may be issued to individuals requiring short-term access to a VA facility for a period not to exceed 15 days in a 365-day period. Visitor passes are issued on a daily basis and expire at the end of the day of issuance. Visitor passes are issued following, at a minimum, review of one valid and current State or Federal government issued photo ID. Visitors must be escorted in areas restricted to the general public.

STATEMENT OF WORK
ELEVATOR INSPECTION SERVICE
Contractor shall furnish all supervision, labor, equipment, travel and supplies necessary to perform Semi-Annual periodic routine elevator dumbwaiter inspections, perform Annual Category 1 elevator dumbwaiter periodic inspection and safety tests and the Five Year Load Test of elevators and dumbwaiters located at the at the William S. Middleton Memorial Veterans Hospital, 2500 Overlook Terrace, Madison, WI 53705.
1. BACKGROUND AND OBJECTIVES: Madison VAH requires two complete periodic routine elevator inspections per year per all ASME codes. Periodic routine elevator inspections include the witness and certification of one (1) Semi-Annual periodic routine elevator /dumbwaiter inspection, one (1) Annual Category 1 periodic elevator/dumbwaiter inspection and safety tests per all ASME codes and the witness of one (1) Five Year Load Test of all of the elevators and dumbwaiters.
2. REQUIREMENTS AND QUALIFICATIONS OF ELEVATOR, DUMBWAITER and LIFT INSPECTORS: The inspectors and inspection supervisor shall meet the qualification requirements of the ASME QEI-1. Inspectors and inspection supervisors shall be certified by an organization accredited by ASME in accordance with the requirements of ASME QEI-1. All work performed under this contract shall be accomplished by a QEI-1 certified inspector employed on a permanent basis by the Contractor awarded this contract. The inspectors’ supervisor shall also be QEI-1 qualified. The inspector and inspection supervisor shall be employed on a permanent basis by the Contractor awarded this contract. The contractor must provide proof of QEI-1 certification for their inspectors and supervisors. Copies of the QEI-1 inspectors and supervisor’s certificates shall be provided with the proposal.
3. INSPECTIONS: There shall be two (2) inspections per year, six months apart, on all the equipment listed in the Schedule of Elevators and Dumbwaiters. Inspections include a Semi-Annual periodic routine elevator, dumbwaiter and lift inspection and the Annual Category 1 elevator, dumbwaiter and lift periodic inspection and annual safety test inspection. Inspections shall take place in April and October of each contract year, unless previously arranged by mutual agreement with the COR.
Inspections shall include but are not limited to; inside of the cars, elevator machine rooms and equipment, elevator control room and equipment, secondary equipment and machine rooms, tops of cars, hoist ways, shafts, elevator lobbies and elevator pits.
All inspections shall be conducted in accordance with the latest editions of ASME A17.1 and all current elevator requirements and standards. Inspections shall cover all items as stated in ASME 17.1, Section 8.11.
Inspections shall follow the recommended inspection and test intervals as specified in latest edition of Table N-1 of the ASME A17.1, Safety Code for Elevators.
All inspections and tests shall be in accordance with the latest editions of other applicable codes, such as, but not limited to:
ASME A17.1/CSA B44 Safety Code for Elevators and Escalators – 2013 ASME A17.1 Safety Code for Elevators and escalators – 2010.
ASME A17.2 Guide for Inspection of Elevators, Escalators and Moving Walks – 2012. ASME A17.3 Safety Code for Existing Elevator and Escalators.
ASME A17.4 Guide for Emergency Evacuation of Passenger from Elevators.
3.1 The Annual Category 1 elevator, dumbwaiter and lift periodic inspection and annual safety test inspection. To be performed during the October, shall include, in addition to all required inspections and safety tests, a review of the existing equipment for safety code compliance, including the firefighters' service functions, ADA requirements, as well as recommended upgrades, enhancements or modernization or needs based on long term life expectancy of the current existing equipment. These findings and recommendations shall be submitted in a separate narrative and included with the inspection report.
3.2 During any inspection, parts shall be removed where necessary to determine the exact condition of parts which could not otherwise be determined, and the equipment shall be cleaned where necessary to make the proper inspection. All parts shall he reinstalled after removal. Elevator inspector shall provide all necessary tools; door pressure gauge, stopwatch, calipers, flashlight and any other small hand tools necessary to conduct inspections.
3.3 All inspections shall be performed in the presence of the elevator maintenance technician, currently under contract as the VA elevator preventative maintenance contractor and a representative from the VA Hospital. The COR at the medical center shall accompany the contractor as an observer, if they choose, during the time of the inspections.
3.4 Inspections shall take place in April and October of each contract year, unless previously arranged by mutual agreement with the COR. All subsequent inspections in the following option years shall be on the same twice per year schedule.
3.5 Each site visit/inspection shall include an out brief meeting with the COR or VA Rep to discuss the inspection violations, findings, immediate needs, and necessary corrections.
3.6 In addition to using the standards in the A17.1 and A17.2 Codes, the inspection contractor shall assure that the maintenance contractor’s preventative maintenance check charts are up to date and correctly annotated.
3.7 Base Year requirement: Provide to the VA, the latest edition of ASME A17.4 Guide for Emergency Evacuation of Passengers from Elevators. Provide code book with the first base year report.
4. ANNUAL / SEMI-ANNUAL TEST: The elevator inspector shall be on site to witness and certify the Annual Category 1 safety tests performed with the VA elevator maintenance contractor. Elevator safeties shall be subjected to the annual running tests as required by ASME A17.2 latest edition. The Annual Category 1 safety tests and inspection will be scheduled to concur with one of the required bi-annual inspections. The elevator inspector shall be on site to witness and certify the Semi-Annual inspections and tests and cover all items as specified in the latest editions of ASME 17.2.
5.
5.1 PERFORMANCE EVALUATION DATA: In addition to the required ASME Annual & Semi-Annual inspections. The Inspection Contractor shall provide an elevator performance evaluation in report form for all elevators, dumbwaiters and lifts once per year.
6. FIVE-YEAR INSPECTION AND LOAD TEST: The elevator inspector shall be on site to witness and certify the five-year load & safety tests performed by the VA elevator maintenance contractor. The five-year inspection and load tests are performed during off tour hours, between 5:00pm on a Friday and 7:00am on Monday. The Contractor shall be notified in advance by the COR on date and times of the Five –Year Inspection & Test. It is anticipated that the five-year load & safety tests will occur in Base Year (2021) of this contact. The five-year inspection and load tests shall be performed during a weekend or holiday to reduce the impact to the facility.
7. DOCUMENTATION: Each inspection survey/evaluation shall include a comprehensive narrative/report. The narrative/report shall include:
7.1 Listing and narrative of the operating performance for each elevator/lift.
7.2 A narrative, which summarizes the inspection and tests conducted.
7.3 Explains and identifies any conditions found to exist, that require repair or replacement.
7.4 Provide recommendations as to what actions should be implemented to correct deficiencies and/or conditions identified.
7.5 Provide list of typical parts that are needed and are recommended for replacement to keep the equipment in good operating condition.
7.6 Contractor shall if necessary, clearly indicate any adjustments, repairs, or replacements necessary to correct troubles encountered.
7.7 Shall furnish specifications (if requested by the COR) enough to enable the VA to purchase the materials required for those purposes.
7.8 Indicate all items and locations inspected.
7.9 A narrative of all findings and recommendations of the existing equipment as detailed in paragraph 3.1. Narrative and recommendations listed above shall be in addition to standard ASME report/checklists and reports per the required ASME codes.
7.10 Reports shall be submitted electronically to the VA COR and the VA CO within two (2) weeks after completion of the inspection.
7.11 Provide the latest edition of ASME A17.4 Guide for Emergency Evacuation of Passengers from Elevators code book along with the base year report.
8. RECOGNIZED HOLIDAYS: Unless otherwise indicated, the contractor is not required to perform services on these Federal holidays.

9. HOURS OF OPERATION: Unless otherwise indicated (such as five-year load test), the contractor is responsible for conducting business, between the hours of 7:00AM and 5:00PM Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.
10. PLACE OF PERFORMANCE: The work to be performed under this contract will be performed at the following government facility:

11. SECURITY REQUIREMENTS: Contractor personnel performing work under this contract must check in with the VA Contracting Officer Representative and shall be escorted by the VA COR or VA rep and the VA elevator maintenance technician during all elevator inspections.
12. INTERFERENCE TO NORMAL FUNCTION: Contractor personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of patients, personnel, equipment, and carts.
13. SAFETY: The Contractor shall consider the clinical environment and instruct his/her employees on applicable safety precautions and specific requirements.
Contractor shall comply with OSHA General Industry and Construction Standards (29 CFR 1910 and 29 CFR 1926. Contractor to provide an OSHA 10 Hour General Industry Training certificate.
The contractor is responsible for compliance with all OSHA and other applicable safety regulations. Contractor is required to protect the immediate safety of persons and equipment in and about the elevator(s) during the inspection process.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 calendar days prior to the contract expiration. The “specified” rates under this clause will be those rates in effect under the contract each time an option is exercised under this clause.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 calendar days prior to the contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 calendar days prior to contract expiration. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed FIVE (5) years.

C.5 52.219-14 LIMITATIONS ON SUBCONTRATING (JUN 2020) (DEVIATION) (a) This clause does not apply to the unrestricted portion of a partial set-aside.
(b) Definition. “Similarly situated entity,” as used in this clause, means a first-tier subcontractor, including an independent contractor, that—
(1) Has the same small business program status as that which qualified the prime contractor for the award (e.g., for a small business set-aside contract, any small business concern, without regard to its socioeconomic status); and
(2) Is considered small for the size standard under the North American Industry Classification System (NAICS) code the prime contractor assigned to the subcontract.
(c) Applicability. This clause applies only to—
(1) Contracts that have been set aside for small business concerns or 8(a) participants;
(2) Part or parts of a multiple-award contract that have been set aside for small business concerns or 8(a) participants;
(3) Contracts that have been awarded on a sole-source basis in accordance with subpart 19.8;

(1) Services (except construction), it will not pay more than 50 percent of the amount paid by the Government for contract performance to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count toward the prime contractor’s 50 percent subcontract amount that cannot be exceeded. When a contract includes both services and supplies, the 50 percent limitation shall apply only to the service portion of the contract. Other direct costs are excluded to the extent they are not the principal purpose of the contract and cannot be obtained from small business concerns;
(2) Supplies (other than procurement from a nonmanufacturer of such supplies), it will not pay more than 50 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count toward the prime contractor’s 50 percent subcontract amount that cannot be exceeded. When a contract includes both supplies and services, the 50 percent limitation shall apply only to the supply portion of the contract;
(3) General construction, it will not pay more than 85 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. (4) Construction by special trade contractors, it will not pay more than 75 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count toward the prime contractor’s 75 percent subcontract amount that cannot be exceeded.
(f) A joint venture agrees that, in the performance of the contract, the applicable percentage specified in paragraph (e) of this clause will be performed by the aggregate of the joint venture participants.
(g) The Contractor shall comply with the limitations on subcontracting as follows:
