SPECIFICATIONS/DESCRIPTION/WORK STATEMENT

(FY 2020 MILWAUKEE VAMC SECURITY SYSTEMS EXPANSION PROJECTS)

BACKGROUND

The Veteran Affairs Medical Center, located in Milwaukee, Wisconsin to include the Community Based Out-Patient Clinics in Green Bay, Wisconsin, Appleton, Wisconsin, Union Grove, Wisconsin, Cleveland, Wisconsin, The CRRC (Community Referral and Resource Center) VA leased space, and the Great Lakes Acquisition Center and Human Resource leased space are equipped with high level security systems. The security systems consist of but not limited to, Intrusion alarms, wired and wireless panic alarms, Intrusion Detection systems, multiple camera and recording devices, phone and intercom systems, personal identification verification systems and physical access control systems.

The Milwaukee Veteran Affairs Police Service currently has a service contract with a security company that provides preventive maintenance, emergency services, and limited upgrades to the current security features. Due to the certain levels of security requirements, to include continuous vulnerability assessment to each location, expansion and other upgrades are detrimental to the entire security network system and our facility is expected to provide security to control the integrity of our environment.

STATEMENT OF WORK

The contractor shall provide management, supervision, proposals, labor, equipment, supplies, installations, and final completion for all expansion projects requested by the Contracting Officer Representative or designee. All work with the proper approval for the building security systems will include Green Bay, Appleton, Cleveland, Union Grove Out-Patient Clinics; Community Referral Resource Center leased by the VA; Honey Creek Corporate Building leased by the VA and the Veteran Affairs Milwaukee Medical Center. All expansion projects must have required certifications for the security systems components and OSHA requirements.

This is a statement of work for security systems and is for the VAMC, Green Bay Clinic, Appleton Clinic, Cleveland Clinic, Union Grove Clinic, CRRC, GLAC, and Human Resource Honey Creek and must provide proposal for services, programming, upgrades, licensing, to all approved proposals for security system expansion projects to include as-built when the project is completed.

All statements of work for the expansion projects below will have the requirements needed to complete each project.

WORK REQUIREMENTS

Line Item 0001:

Bosch Transponders

Contractor will provide and install Bosch wireless panic alarm systems in Building 70 2nd floor Facility Management and Building 70 1st floor Adult Day Health Care. This install will include wireless receivers and transmitters that will annunciate to VA Police Operations through the Bosch man down system. Contractor will provide all programming and mapping to ensure the system provides 24/7 capabilities.

Line Item 0002:

CCTV Building 70

Contractor will provide and install five (5) fixed cameras to the Building #70 Adult Day Health Care clinic as follows; (1) one four-way fixed camera facing the entry doors, elevator, 70 E wing North, and facing towards the ADHC patio exit from the building 70 hallway intersection, (1) one fixed camera facing the entry doors from within the clinic at approximately the mid-point of the main clinic hallway, (1) one fixed camera facing the emergency exit stairwell door within the clinic, (1) one fixed camera facing the emergency exit door from within the stairwell area, and one (1) fixed camera in the Building 70 1st floor hallway facing the ADHC patio doors. (5) five milestone licenses will be needed for operation of the cameras. Contractor will provide all programing, mapping and proper recording to all devices. The camera will have the ability to be viewed at VA Police Operations, with a minimum 60-day record capability and the basic requirements of 15 frames per second. Contractor will interface cameras to Wandergard system to allow for camera call ups.

Line Item 0003:

Green Bay CCTV

Contractor will provide and install two (2) fixed cameras to the Green Bay Community Based Outpatient Clinic Veterans Canteen Service area as follows; (1) one three-way Multiview camera inside the entrance of the double doors to the Retail Store, and one (1) two -way Multiview camera in the food court facing the cashier and the glass display cases South of the food service area. (2) two milestone licenses will be needed for operation of the cameras. Contractor will provide all programing, mapping and proper recording to all devices. The camera will have the ability to be viewed at VA Police Operations, with a minimum 60-day record capability and the basic requirements of 15 frames per second.

Line Item 0004:

Milestone Workstations

Contractor will provide 4 Milestone Workstations for replacement of current outdated workstations as follows; (1) one in Milwaukee VA Police Operations (2) two in Green Bay VA Police Operations and (1) one in Appleton VA Police Operations These workstations will be programmed and mapped to function as upgraded working Milestone recording servers to ensure effective 24/7 monitoring and record capability of Milestone CCTV systems within the entire Milwaukee VAMC complex, to include the Green Bay, Appleton, Cleveland, Union Grove and CRRC Community Based Outpatient Clinics.

Line Item 0005:

Blue/Yellow Clinic/Adult Day Health Care Alarm Upgrade

Contractor will provide and install fifty-three (53) upgraded panic alarm systems to the Building 111 Blue and Yellow Clinics and Building 70 Adult Day Health Care, utilizing Honeywell 270R hard wired hold up switch. This will require the replacement of 53 hard wired panic alarms in the following locations of Building 111; rooms; 1501, 1502, 1503, 1504, 1505, 1506, 1507, 1510, 1512, 1514, 1516, 1518, 1520, 1522, 1524, 1527, 1528, 1530, 1531, 1532, 1533, 1536, 1537, 1537A, 1538, 1539, 1540, 1541, 1543, 1544, 1545, 1545A, 1546, 1547, 1549, 1550, 1551, 1552, 1552A, 1553, 1554, 1555, 1557, 1558, 1565, 1566, 1567, and 1568. Building 70 Adult Day Health Care rooms 70-E-126, 70-E-128, 70-E-132, 70-E-133A and 70-E-136. These alarms will require key reset capabilities and will annunciate in Police Operations through the P2000 with audible alarm and pop up notifications. Contractor will provide all programing, mapping and proper recording to all devices.

Line Item 0006:

PLS 4 and 7 IDS and Panic Alarms

Contractor will provide and install upgraded panic alarm systems to the parking structure 4 and 7 Milestone recording server rooms utilizing Honeywell 269R hard wired hold up switch. These alarms will require key reset capabilities and will annunciate in Police Operations through the P2000 with audible alarm and pop up notifications. Contractor will also install Intrusion Detection within those rooms to ensure detection of unauthorized access. These Intrusion detection devices will annunciate to police operations with an audible alarm and pop-up notification. Contractor will provide all programing, mapping to all devices.

Line Item 0007:

Upgrade multiple analog cameras on the Milwaukee Campus

Contractor will provide and install 55 total fixed cameras in multiple locations across campus to upgrade analog camera systems. The locations are as follows;

Building 111 locations of; 3CN #22 Restraint Room, 3CN #21 Seclusion Room, 3CN #11 Dining Room 3, 3CN #13 Intake Room 3608, 3CN #20 Sally Port, 3CN #16 Nurses Station, 3CN #19 Visitor Lockers, 3CN #15 Stairwell Hallway by Room 3626, 3CN #10 Dining Room 2, 3CN #2 Sun Room, 3CN #9 Dining Room 1, 3CN #23 C Bank Elevators, 3CN #14 Patient Effects, 3CN #7 Main Door Hallway, 3CN #6 North Visitors Lounge, 3CN #18 Day Room 2, 3CN #3 Computer Room, 3CN #17 Day Room 1, 3CN #1 Quiet Room, 3CN #5 North South Door, 3CN #8 Patient Phones, 3CN #12 Dining Room 4, 3CN #4 Laundry Room, Chapel Offering, Chapel West door, Loading dock trash room, Loading dock recycling room, Loading dock interior, warehouse entrance, warehouse rear entrance, Building 70 locations of; Loading dock camera 1, Loading dock camera 2, Loading dock camera 3, Loading dock camera 4, West entrance (192.168.2.100), Courtyard East (192.168.2.100), Volunteer entrance (192.168.2.101), West entrance (192.168.2.101), and Chapel alter (192.168.2.101), Building 123 locations of; A wing lounge 3, D wing lounge 1, B wing lounge 3, A wing lounge 1, A wing lounge NE exit, B wing lounge 1, B wing lounge 2, Dining area (192.168.1.37), TV area, A wing (192.168.1.38), Dining area (192.168.1.38), Back door in vestibule, and E wing garage area.

The cameras will have the ability to be viewed at VA Police Operations with a minimum 30-day record capability. The cameras have existing Milestone corporate licenses. The cameras will be installed onto the milestone server/management server and licensed with 55 licenses under the corporate level for police services. Contractor will provide all programing, mapping and proper recording for the devices to include the basic requirements of 15 frames per second.

Line Item 0008:

Building 146, 147, 148 CCTV

Contractor will provide and install (1) new fixed camera in CLC Building 146 Pantry, building 147 Pantry, and Building 148 Pantry. Contractor will make every effort to use existing cabling. Three (3) Milestone corporate license will be needed for the installation and use of the camera. Contractor will provide all programing, mapping and proper recording to all devices. The camera will have the ability to be viewed at VA Police Operations, with a minimum 30-day record capability and the basic requirements of 15 frames per second.

Contractor will make every effort to use existing cabling. Contractor will ensure all proper programming, mapping and testing of the installed system.

Line Item 0009:

Pharmacy Monitors

Contractor will provide and install a monitor to view pharmacy camera feeds in (2) two pharmacy offices. Contractor to furnish thin client to allow for camera viewing. Contractor to program camera views to view up to sixteen cameras per monitor. Contractor will provide all programing and mapping to all devices. Contractor to utilize existing camera feeds from Milestone system for monitors.

Line Item 0010:

CRRC Server

Contractor will provide and install new Milestone recording server to replace failed server at CRRC. Server will be installed in the IT Server room to have proper recording and 60 day of saved video coverage.

Line Item 0011:

Intrusion System, BIOMED

Contractor will provide and install Motion Intrusion system, to include local alarms and upgrade the current card reader to high insurance and a panic alarm. The install is for the in the BIOMED Server room building 70. All features will be programmed in the P-2000 System.

Line Item 0012:

Panic Alarm, Hematology

Provide and install (1) new panic alarm in Hematology clinic to include all programming, commissioning, system test and checkout.

Place of performance will be:

Milwaukee VAMC 5000 West National Avenue Milwaukee, Wisconsin 53295

Period of performance: Estimated projects completion timeline is date of award through 12 months.

1. CONTRACT EFFORT REQUIRED

A. General: It is of utmost importance that the contractor utilizes adequately skilled and productive labor in order to furnish the required level of services specified in this contract.

Failure on the part of the contractor to perform satisfactory work and obtain satisfactory results shall necessitate Government action to adjust the contractor’s monthly invoices accordingly and/or pursue contract termination.

B. Contractor’s personnel are required to sign-in immediately upon arrival on-site, at the Police Operation Center (Building 111), and sign-out at time of departure.

C. Contractor will coordinate all actions with the Project Manager with regards to any door lock or door modifications and installations. This guidance will also apply to any window modifications.

Environment of Care Program – Consists of basic policies and procedures, VAMC specific, to be provided to the contractor in order to familiarize contractor’s personnel with the work environment before the contractor begins on-site work activities. Purpose is to promote safe and effective on-site work performance.

3. SERVICES REQUIRED

A. As a minimum, inspections, preventive maintenance and all expansions shall be completed as required by and in accordance with the following:

1. National Fire Protection Association – National Electrical Code, NFPA 70-latest edition

2. National Fire Protection Association – Electrical Equipment Maintenance, NFPA 70B-latest edition

3. National Fire Protection Association – Electrical Safety Requirements for Employee Workplaces, NFPA 70E-latest edition

4. National Fire Protection Association – Health Care Facilities, NFPA 99-latest edition

B. Expansion installations services shall be performed within the normal workweek. No non-emergency services shall be performed at other than the normal workweek unless prior approval of the COR (or designee) is obtained.

C. All approved proposals will be coordinated with the Police to ensure all components are met for the installation.

D. Acquisition and installation of software updates/upgrades. Perform database back-ups must be coordinated with the VA Police.

E. Provide consultative services to conduct and document performance improvement reviews and resolve system operational problems not covered by other contracts.

F. Security upgrades and expansions shall be completed upon approval unless the contractor requests and obtains approval to perform work at a later date and time. Requests shall be submitted, in writing, to the COR and state:

- Reason for requested delay in performance of approved proposal work:

- Equipment and functions affected by the delay;

- Date and time project are requested to begin:

- Duration of project; and

- Operational status of equipment during performance of the upgrade.

4. MATERIALS AND PARTS TO BE FURNISHED, INSTALLED, AND/OR REPAIRED BY

THE CONTRACTOR

A. Solvents and lubricants shall be of the type specifically approved by the manufacturer of the equipment.

B. Parts/components shall be approved for use by the manufacturer of the building security system to include approval from the COR or designee.

C. Software and hardware updates/upgrades shall be approved for use by the manufacturer of the building security system to include approval from the COR or designee.

5. TEST EQUIPMENT

A. Prior to commencement of work on this contract, the contractor shall provide the VAMC with a copy of the current calibration certification of all test equipment, which is to be used by the contractor on VAMC’s equipment. After each proposal is completed by the contractor it must be tested and verified through the Physical Security Specialist. Test equipment calibration shall be traceable to a national standard.

6. UTILITIES FURNISHED BY THE GOVERNMENT

A. Electrical power at existing outlets for the contractor to operate such equipment as is necessary for the performance of work required by this contract.

7. SERVICE MANUALS/TOOLS/EQUIPMENT

A. The VAMC shall not provide tools, (test) equipment, service manuals or service diagnostic software to the contractor. The contractor shall obtain, have on file, and make available to its service personnel all operational and technical documentation (such as operational and service manuals, schematics, and parts list) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name and/or the manuals themselves shall be provided to the CO upon request.

8. FACILITY BUILDING SECURITY SYSTEMS COMPONENTS INCLUDED WITHIN THE

SCOPE OF THE CONTRACT

A. All items noted above are the facility building systems components and are within the scope of work, unless otherwise noted.

B. Contractor must be able to access systems remotely.

C. Contractor must rapidly resolve problems to minimize the impact of safety and business operations.

D. Contractor must be able to provide a service team that is certified and has knowledge of all system and component needs.

E. Contractor must provide written reports/assessments for testing on all expansions and status of normal status of any upgrade/expansions and as-builts.

F. Contractor shall advise the VA of changes in code requirements, reliability standards, and performance improvements as recommended by the manufacturer(s).

9. SERVICE NOT COVERED

This contract does not include the performance of any work, as determined solely by the VA, required as a result of negligence, improper use, accident, vandalism, power line fluctuations, or damage by work performed by others for which the contractor is not directly responsible. The contractor shall not supply nor install new attachments as may be recommended or directed by inspection firms or by State, Municipal, or other Federal Government authorities, unless approved in writing by the Contracting Officer or designee.

10. COMPLIANCE WITH SECURITY REQUIREMENTS

A. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS:

The Contractor's employees and/or subcontractors approved by the COR, shall wear visible identification at all times while on the premises of the VAMC. Identification shall include but not be limited to a company identification card, which shall include the company name, employee name, title, and a photograph of the employee. All employees of the Contractor or Subcontractor providing services to the Government must wear a company uniform identified with the company name and/or logo. Uniforms must be neat and clean at all times while working the VA premises and will be approved by the COR.

Contractor’s personnel may be required to possess, and wear VA issued identification cards in addition to the above requirements.

B. It is the responsibility of the Contractor to park in the appropriate designated parking areas.

Information on parking is available from the VA Police Section. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.

C. The Contractor will be provided keys in order to access areas in the building. The

Contractor shall be responsible for any lost keys, card keys. Associated hardware and any inherent damages (i.e., re-keying of whole building). This cost shall be withheld from payment(s). The decision to re-key the whole facility is solely that of the Government.

D. Any interior or exterior doors that must be unlocked to gain access shall be kept locked during the performance of work. Should any other devices need to be unlocked like an entry gate, those devices must be locked immediately upon entry or exit. At no time shall the contractor allow any person into a locked area or loan keys/access cards to another person. Contractor may be held in default for failure to comply with this requirement and subject the Contractor and employees to contract and other legal remedies.

G. The Contractor agrees that none of its officers or employees shall use or reveal any research, statistical, medical, patient specific, or security information obtained in the performance of the work without the written consent of the Government.

11. COMPLIANCE WITH THE ENVIRONMENT OF CARE PROGRAM (SAFETY

REGULATIONS)

In performance of this contract, the Contractor shall train its employees to follow the basic VAMC Environment of Care policies/procedures and standard safe work practices, and takes such safety precautions as the Safety Manager or his designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer or his/her designee will notify the Contractor of any noncompliance, and the Contractor shall immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his representative at the site of the work, shall be deemed sufficient for the purposes aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue and order halting of all or any part of the work, and Contractor may be held in default.

Contractor shall provide to the Contracting Officer, prior to contract start date, Material Safety Data Sheets (MSDS) for all potentially hazardous materials (lubricants, cleaners, working fluids, etc.) to be used in the facility in performance of the contract, and will not use, in the facility, such materials which have not been so cleared in advance with the VAMC. MSDS for new chemicals shall be furnished concurrently with arrival of the chemicals on site. Additionally, a copy of all required MSDS shall be maintained at the chemical storage site in a location, readily accessible to VA personnel.

NOTE: OSHA requires used rags and flammable materials to be stored in fireproof containers (provided by the contractor).

12. INVOICES AND PAYMENT

A. Invoices will be submitted when completion of proposal work is done in arrears to the Government Office designated in the solicitation and include the original and one copy.

B. To constitute a proper invoice, it must include, as a minimum the following information:

1. Name of the business concern and invoice date.

2. Contract and purchase order numbers or other authorization for rendering services. Failure to cite these numbers may cause a delay in processing payment.

3. Description, price, and quantity of property and services delivered or rendered.

4. Name (where practicable), title, phone number, and complete mailing address of responsible official to whom payment is to be sent and any other substantiating documentation or information as required by contract.

13. INSURANCE

A. Worker compensation and employer’s liability. Contractors are required to comply with applicable Federal and State Worker Compensation and occupational disease statutes.

B. General Liability. Contractors are required to have Bodily Injury Liability Insurance coverage written on the comprehensive form of policy of at least $1,000,000 per occurrence.

C. Property Damage Liability. Contractors are required to have Property Damage Liability insurance coverage of at least $1,000,000.

14. UNAUTHORIZED USE OF GOVERNMENT EQUIPMENT

The Contractor shall not allow his/her employees, including Subcontractors’ employees, to open desk drawers, cabinets, or to use office equipment, including the use of non-pay telephones for any purpose other than a local emergency call.

15. TAX EXEMPTION

The Federal Government is exempt from most taxes. Exemption Certificates indicating the Medical Center’s tax exempt status will be furnished by the Contracting Officer upon request.
