1. Contracting Activity: : U.S. Department of Veterans Affairs, VISN 12, Clement J Zablocki VA

Medical Center, Milwaukee, WI 2237: 695-19-4-969-0196

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Milo C. Huempfner VA Outpatient Clinic (OPC) in Green Bay, Wisconsin had an EMERGENCY, Brand-Name Only requirement for a Planmeca ProMAX Cone Beam CT (CBCT) System.

A total cost of $88,293.51

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Dental Service at the Green Bay OPC uses a Planmeca ProMAX CBCT System for their prosthetic treatments. The CBCT system at the facility is currently broken and unusable. The cost to repair the owned system over two thirds the cost of a total system replacement. This CBCT is necessary to provide proper patient care and the Associate Director authorized an “Emergency” purchase for this equipment. See contract file for authorization documents.

4. Description of market research conducted and results or statement why it was not conducted:

Market Research was conducted IAW current VA policy. Queries of the VetBiz, EPLS, FPDS, and SAM.Gov databases were conducted.

Included in the Acquisition Package that was submitted by the facility was authorization by the Associate Director to proceed with this request as an EMERGENCY procurement. No further Market Research was conducted and this request was processed and awarded immediately under the authority of FAR 6.302-2(a)(2) – Unusual and Compelling Urgency.

This document was prepared and submitted for approval within a reasonable time after award IAW

FAR 6.303-1(e).

Contracting Officer's Certification: This acquisition meets the requirements of FAR 6.302-2(a)(2) – Unusual and Compelling Urgency.
