INVOICES:
Payments shall be made in arrears upon receipt of a properly prepared invoice.
In order to comply with the Improper Payment Elimination and Recovery Act of 2010 (IPERA), the VA has mandated electronic invoice submission to the Veterans Affairs Financial Services Center (VAFSC). VAFSC has partnered with Tungsten Network e-Invoicing network, for submissions of all electronic invoices to VA. Tungsten Network electronic invoicing is free to all VA vendors. In order to submit electronic invoices, all VA vendors must register with Tungsten Network by submitting an email to VA.Registration@ob10.com or calling 1-877-752-0900 option 2 for Enrollment.
Contractor shall submit an electronic invoice by the tenth (10th) of the following month services were performed to the Veterans Affairs Financial Services Center (VAFSC) e-Invoice through the website at https://portal.ob10.com/Login.aspx. For questions regarding the submission of VA electronic invoices, Tungsten Network customer service may be contacted at 1-877-489-6135.
All invoices shall reference the vendor name and address, customer name, contract number, appropriate obligation/funding order number, description of services provided, quantity, unit price, and total invoice amount (any additional info). Invoices shall also include any payment discount terms.

SPECIAL NOTES
In order to maintain integrity of this solicitation and subsequent award date, all offerors are advised that any question and answer requests must be submitted in a written form via e-mail to the Contracting Officer susan.pasholk@va.gov no later than WEDNESDAY MAY 29, 2019 AT NOON CENTRAL. Contracting will issue a consolidated response via solicitation amendment to www.fbo.gov within 2 business days to all parties.
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitment or issue changes that will affect price, quantity or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred thereof.
SECURITY & PRIVACY CONTROL:
The Certification & Accreditation (C&A) requirements of VHA Handbook 6500.6 do not apply to this requirement, therefore, a Security Accreditation Package is not required. No C&A or MOU/ISA is required. Sensitive information exposure as part of this contract involves applicable security controls within the facility as part of the VA Information Security

AUTHORITY
This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 13.5 for Certain Commercial Items in which simplified acquisition procedures in FAR Part 13 will be used. FAR Part 14 and 15 will not be used.
The Lowest Price Technically Acceptable source selection process will be used for this FAR 13.5 procurement. This means award will be made on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. See 52.212-2 Evaluation – Commercial Items (Section E) for a description of the factors in addition to price that the Government will use to evaluate offers. Prospective contractors must furnish the information listed herein with their quotations to ensure they receive full consideration.
SITE VISIT
It is strongly suggested and expected that the offeror inspect the campus to be serviced to fully understand the nature of the work and the conditions under which the work is to be performed. In no case shall failure to inspect the campus constitute grounds for a claim after contract award. SITE VISIT IS SCHEDULED FOR THURSDAY MAY 23, 2019 AT 10:00 AM CENTRAL. STATEMENT OF WORK
GENERATOR LOAD BANK TESTING
Contractor shall provide all labor, parts, equipment, tools, supplies, transportation and supervision necessary to provide testing, in July of each contract year, in accordance with requirements for standby generator testing, Attachment A, VHA Directive 1028 “Electrical Power Distribution Systems” and all applicable NFPA (National Fire Protection Association), Joint Commission, Federal, State and Local regulations.
1. SCOPE:
A. Testing shall be performed on nine (9) emergency standby generators located at the Tomah VA (Veterans Administration) Medical Center, Tomah, WI. as listed on Attachment B.
B. The emergency standby generators shall be load bank tested on an annual basis for a period of two (2) continuous hours in the following intervals:
1) 25% of name plate rating for 30 minutes, followed by,
2) 50% of name plate rating for 30 minutes, followed by,
3) 75% of name plate rating for 60 minutes.
C. During the triennial load bank test, which will occur in 2021 (Option Year 2) of this contract, the emergency standby generators shall be tested for a period of four and one half (4.5) continuous hours in the following intervals:
1) 25% of name plate rating for 30 minutes, followed by,
2) 50% of name plate rating for 30 minutes, followed by,
3) 75% of name plate rating for 3.5 hours.
D. Contractor shall perform annual laboratory fuel testing of all standby generator underground/above ground fuel supply tanks and send testing report to COR (Contracting Officer Representative) per Section 2, “Contract Deliverables” below. Fuel shall be tested to all NFPA 99 and NFPA 110 standards and in accordance with VHA Directive 1028 and Joint Commission requirements and any other Federal, State and Local regulations.
E. Contractor shall visually check general condition of the EPPS (Electrical Power Supply System) and note any unusual condition or vibration, leakages, noises, temperatures or general deterioration of overall system. Any discrepancies shall be noted and be included as a contract deliverable per Section 2 below.
F. Contractor shall check operation of day tank, to include, operational test of day tank float switch, transfer pump operation, solenoid valve operation (if so equipped) and inspection of all fuel lines and fuel line connectors for indication of wear or fuel leaks.
G. Contractor shall restore system to automatic operation upon completion of all required testing services.
H. Any changes to this SOW (Statement of Work) shall be authorized and approved only through written correspondence from the Contracting Officer in coordination with the Contracting Officers Representative. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor.
I. Notwithstanding, the Contractor shall perform no additional work without the prior written authorization of the Contracting Officer. Any additional work, which may be performed by the Contractor without this approval, will be performed by the Contractor at no additional cost to the Government.
2. CONTRACT DELIVERABLES:
Contractor shall provide, to the COR after completion of work, at the minimum, the following deliverables:
A. The following information, which shall be recorded, during each increment of testing as defined in Section 1, Part B, Line Items 1 through 3 and Section 1, Part C, Line Items 1 through 3.

13) Initials of Technician Performing Testing B. Contractor shall provide COR with results of annual laboratory testing of fuel in all standby generator, above ground and below ground fuel tanks.
C. Above information shall be provided to the COR in both Digital and Hardcopy formats and within 30 days upon completion of testing.
3. GENERAL REQUIREMENTS:
A. The contractor’s FST (Field Service Technician) shall contact the COR/POC prior to performing service at this location and shall communicate to the COR of their presence on the Tomah VA Medical Center campus.
B. The contractor shall obtain all other necessary licenses and/or permits required to perform all work under this contract. All technicians performing work under this contract shall have completed OSHA 10-hour safety training.
C. Contractor is responsible to replace any items damaged by the contractor, sub-contractor, their employees or equipment at no extra charge to the Government.
D. Contractor shall not interrupt any utility service while performing under this contract.
E. Contractor personnel shall conduct their work in a manner that does not interfere with the normal functions of the facility and will stop work if asked by the CO/COR as required if interruptions do occur.
F. The Contractor is responsible for supplying, completing, and submitting all reports required or requested by any additional Federal, State, or local codes as they pertain to any work performed under this contract.
G. No hot electrical work shall be performed. Any electrical work that requires any interruption in electricity must be coordinated with the COR and the facility electricians. All OSHA (Occupational Safety and Health Administration) and Facility lockout/tagout procedures shall be followed.
4. CONTRACTING OFFICER’S REPRESENTATIVE (COR):
Scheduling of work shall be arranged with at least one, or all the Point of Contacts below and in the following order; contract COR, contract ACOR (Alternate Contracting Officers Representative) or Tomah VAMC Industrial Equipment Mechanic(s).

5. FACILITY SECURITY REQUIREMENTS:
A. All Contractor FST’s (Field Service Technician(s)) shall obtain contractor ID badges from the COR. Identification shall be worn, visible always by the Contractor and all Service technicians while on premises. Contractor shall coordinate with COR to acquire said identification badges.
B. During normal working hours of operation, Monday through Friday, the contractor shall check in with the COR when arriving on campus to perform work and check out with the COR when work day is complete. Check-in and check-out can be either in person or with a phone call and will be left to the discretion of the COR.
6. RISK CONTROL:
A. The Contractor shall take all necessary precautions to protect government property and shall repair any damage caused by their efforts.
B. All work affecting patients shall be coordinated in advance by the Government and the Contractor to have minimal impact on the patients.
7. COMPETENCY OF FIELD PERSONNEL:
A. Each respondent must have an established business, with an office and full-time staff. The staff shall include a “fully qualified” FST (Field Service Technician(s)) and “fully qualified” FST (Field Service Technician(s)) who will serve as the backup.
B. “Fully Qualified” is based upon training and on experience in the field. For training, the FST must have a minimum of two years of experience (except for equipment newly on the market) with respect to scheduled and unscheduled preventative maintenance on emergency standby generators.
C. The FST’s shall be authorized by the Contractor to perform all maintenance services required in above Scope of Work. All work shall be performed by a “fully qualified” competent FST. The Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FST’s for each make and model of generator the Contractor services at the Tomah VA Medical Center. The CO (Contracting Officer)/COR may authenticate the training requirements, request training certificates or credentials from the Contractor at any time for any personnel who are servicing or installing VA equipment. The CO/COR specifically reserve the right reject any of the Contractor’s personnel and refuse them permission to work on Tomah VA Medical Center standby generators and associated equipment.
D. If a Subcontractor(s) is used, in accordance with Clause 852.219-74 LIMITATIONS ON SUBCONTRACTING, Subcontractor must be approved by the CO/COR. The Contractor shall submit any proposed changes in Subcontractor(s) to the CO for approval.
8. PLACE OF PERFORMANCE:
Tomah VA Medical Center, 500 E. Veterans St., Tomah, WI. 54660
9. PERIOD OF PERFORMANCE:
A. Within fifteen (15) days of the award of this contract, the Contractor and the Government shall agree upon a date for work to take place.
B. All work is to take place during the normal hours of operation defined herein, unless agreed upon by the COR in advance, and at no additional cost to the Government.
C. All testing shall take place in July of each contract year and during normal business hours as indicated in Section 10 below.
10. SERVICE HOURS AND FEDERAL HOLIDAYS:
A. Normal hours of coverage are Monday through Friday from 7:30 am to 4:30 pm, excluding holidays. All work shall be performed during normal hours of coverage unless otherwise requested or approved by the COR or their designee.
B. All contract work shall be on a date and time mutually agreed upon between the Contractor and the COR.

11. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS (For any onsite services):
1. The Contractor's employees and/or subcontractors approved by the COR shall wear visible identification at all time while on the premises of the VAMC. Identification shall include, but not be limited to a company identification card, which shall include the company name, employee name, title, and a photograph of the employee.
2. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VAMC Police Section. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions.
3. Any interior or exterior doors that shall be unlocked to gain access shall be kept locked during the performance of work unless it results in the creation of a safety hazard. In those instances, the Contractor is responsible to take necessary actions to maintain the same level of security prior to gaining access/performing work. Should any other devices need to be unlocked like an entry gate, those devices shall be locked immediately upon entry or exit. At no time shall the Contractor allow any person into a locked area or loan keys/access cards to another person. Contractor may be held in default for failure to comply with this requirement and subject the Contractor and employees to contract and other legal remedies.
4. The Contractor agrees that none of its officers or employees shall use or reveal any research, statistical, medical, or security information which may be obtained during the performance of the work or as a result their presence on Government premises without the written consent of the Government.
5. Smoking is prohibited inside any buildings at the VAMC.
6. Possession of weapons is prohibited.
7. Enclosed containers, including tool kits, may be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.

The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor’s compliance with the limitations on subcontracting or percentage of work performance requirement.

C.7 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)
(a) Definitions. As used in this clause-
(1) Contract financing payment has the meaning given in FAR 32.001.
(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, e-mail, and scanned documents are not acceptable electronic forms for submission of payment requests;
(4) Invoice payment has the meaning given in FAR 32.001; and
(5) Payment request means any request for contract financing payment or invoice payment submitted by the Contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the Contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A Contractor must ensure that the data transmission method and format are through one of the following:
(1) VA’s Electronic Invoice Presentment and Payment System at the current website address provided in the contract.
(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI)
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances below, the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for—
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of WISCONSIN. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.
