Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Replacement of the booms in the six operating rooms at the William S. Middleton MVH in Madison, WI.

The requirement includes: demo and removal of existing Flexis anesthesia boom and pre-install work necessary to replace with a new boom in 6 operating rooms. Infection control, electrical, and med gas included.

Estimated Amount: $143,874.00

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Stryker booms are required because the William S. Middleton MVH operating rooms currently have all Stryker accessories such as the monitors and the printers that need to be compatible with the booms required for this procurement. Also, Stryker will be able to use existing mounts, video and integration cabling, which results in cost savings for the Government.

4. Description of market research conducted and results or statement why it was not conducted:

Results from VIP Query (Contracting Officers shall review the Vendor Information Pages (VIP) database at https://www.vip.vetbiz.gov): VIP search was conducted using the search criteria of NAICS Code:

339113, the search resulted in 361 matches. A second search was conducted using the search criteria of

NAICS Code: 339113 and the Keyword: Stryker, the search resulted in one (1) SDVOSB match. Vendor responded to e-mail indicating they are not able to provide this requirement.

Market Research Sources: Ability One / FSS Search / Intent to Sole Source

Market Research Findings:

Ability One website search was conducted; Ability One does not provide the required product.

FSS search conducted, resulting in no FSS contract holders providing the requirement.

Intent to Sole Source was posted onto FBO on 05/01/2018, with five (5) vendor responses received indicating they are capable of providing the brand name only requirement, one (1) Other Than Small

Business, one (1) Small Business and three (3) SDVOSB vendors.

Based on the information provided above, it is determined this procurement will be fulfilled by issuing a

Total Service Disabled Veteran Owned Small Business Set Aside, Brand Name Only RFQ onto FBO, i/a/w: VAAR 819.7005.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
