Question 3: Will the Government please clarify the relative importance of the evaluation factors (e.g., technical capabilities and past performance when combined are more important than price; OR price is more important than other factors; etc.)?
Answer 3: Per Far 13.106-1(a)(2)(iii) Solicitations are not required to state the relative importance assigned to each evaluation factor and subfactor, nor are they required to include subfactors.
Question 4: Will the Government consider past performance other than CPARS, and if so, how?
Answer 4: Past performance evaluations will be conducted using information provided with the Quote, information obtained from references, information obtained from the Contractor Performance Assessment Reporting System (CPARS), and information from any other sources deemed appropriate.
Question 5: In the RFQ it states “Contractor shall provide training to Engineering personnel on requirements and responsibilities within one (1) week of contract award”. Could you give specifics about what this training is?
Answer 5: The training specified includes all sampling procedures required by the testing lab.
Question 6: The RFQ it states that the “Tester shall utilize all Personal Protective Equipment (PPE) appropriate for the location.” Outside of normal sample collection PPE (glove, goggles, gown, mask) is there any special PPE needed?
Answer 6: The sampling shall be conducted in areas of patient care or areas adjacent to patient care. There are no special PPE requirements outside of those commonly used; intensive care areas will be included on the sampling list, but no sampling will be conducted in occupied intensive care patient rooms.
Question 7: The RFQ states "At least six (6) normal samples taken each quarter shall be retested with all procedures and requirements used for the original samples. Any samples to be retested shall be coordinated with the COR before any work is performed.” Retest outlet selection: Who selects the specific outlets that are to be retested? Coordination with the COR: What specific details or documentation are expected when coordinating with the COR prior to performing the retests? Purpose and Timing: Is the retesting primarily intended as a quality assurance measure, and is there an expected timeline for when these retests should be conducted following the original tests?
Answer 7: Please see the corrected statement in the SOW in section Specific Tasks for Testing #15 (attached).
STATEMENT OF WORK (SOW)
SOW Title: RLRVAMC Legionella Testing Background: The Richard L Roudebush VA Medical Center is required to complete testing in facility water systems that may potentially contain Legionella bacteria. The medical center requires the validation of engineering controls applied to potable water sources within Building 1 and the Veteran’s House at Cold Spring Road. This validation is accomplished by regular microbiological testing of water from sources that affect patient care areas.
Scope: The Richard L Roudebush VA Medical Center, Indianapolis requires microbiological testing for the presence of Legionella bacteria to meet VA Directive 1061, Centers for Disease Control (CDC) Elite Protocol, and applicable standards from state and federal organizations. This is located at the Richard L. Roudebush VA Medical Center, located at 1481 W. 10th Street, Indianapolis, IN 46202 and the Veteran’s House located at 2669 Cold Spring Rd, Indianapolis, IN 46222. Services shall be in accordance with the terms, conditions, provisions, and schedule of the contract.
Specific Tasks For Testing:
1. Contractor shall provide all labor, materials, equipment, transportation, lodging and supervision necessary to collect appropriate samples and deliver the samples to a laboratory for testing.
2. The laboratory used to test samples shall be accredited by a recognized regional, national, or international accrediting body.
3. The laboratory used shall demonstrate proficiency for microbial testing of potable water by culture methodology according to the Centers for Disease Control and Prevention (CDC) Environmental Legionella Isolation Techniques Evaluation (ELITE) program or the Public Health England (PHE) Legionella External Quality Assessment (EQA).
4. Polymerase Chain Reaction (PCR) testing methodology shall not be used.
5. Contractor shall provide training to Engineering personnel on requirements and responsibilities within one (1) week of contract award.
6. Contractor shall provide same tester each time to avoid the need of an escort. Escort shall be provided for the first two (2) quarters of testing. Following the first two (2) quarters of testing, the contractor shall be familiar enough with the facility’s layout to perform testing without an escort.
7. Tester shall be proficient in microbiological sampling methods and procedures. All documentation of tester’s training shall be provided to the VA within one (1) week of contract award.
8. Tester shall utilize all Personal Protective Equipment (PPE) appropriate for the location of each sample.
9. Tester shall immediately contact the Contract Officer’s Representative (COR), or alternate contact if COR is unavailable, if access to a testing source is blocked, denied, or inaccessible.
10. All positive Legionella results shall be communicated to the COR, and other VA personnel designated by the COR, as soon as testing results are available.
11. All samples taken from each source shall be the first draw; taken before allowing the water to substantially flush prior to taking the sample.
12. After the first draw has been taken from a source, the contractor shall also measure the initial temperature, temperature after one (1) minute of flow, pH, and two levels of biocide from that source. The levels of biocide to be measured and reported include the Total Chlorine and the Free Chlorine.
13. At least two (2) samples taken each quarter shall be from a hot water source with no mixing valve if such water outlets are available. Hot water tests shall also include the temperature of a sample from that source after one (1) minute of constant flow.
14. At least two (2) samples taken each quarter shall be from an ice machine or other pure cold water source with no mixing valve.
15. Contractor shall retest any sample locations that produce positive Legionella results above acceptable limits. All retesting samples shall be collected within one (1) week of the notification of positive results. All retests shall be completed in accordance with the procedures contained within the facility's Water Safety Management Plan and VHA Directive 1061
16. Hot water, cold water, and mixed water samples shall be taken in accordance with the sample requirements of the laboratory used. The contractor shall be responsible for verifying and complying with these requirements.
17. All samples shall be stored and transported according to the testing requirements of the laboratory used for testing. Laboratory delivery deadlines may change if ice packs or refrigeration are used when storing samples.
18. Tester shall coordinate sample locations with the COR each quarter before testing occurs. General locations and testing requirements are shown in the table below.
19. Contractor shall validate and certify the facility’s Water Safety Management Plan during each year of contract performance.

Performance:
1. The Government shall document performance. Any report shall become a part of the supporting documentation for any contractual action.
2. The laboratory used shall be able to determine if the Legionella detected by culture in environmental samples is the species Legionella pneumophila and, if so, if it is L. pneumophila serogroup 1.
3. The test results included in the report shall show if cultures detected are of Legionella Pneumophila Serogroup 1, Legionella Pneumophila of a Serogroup other than 1, or Legionella non-Pneumophila.
Security Requirements: Contractor shall not have access to VA Sensitive Information, patient records, or data. The Certification and Accreditation (C&A) requirements do not apply, and a Security Accreditation Package is not required.
General Requirements
1. All on site work shall be coordinated with Sean Palmer (317) 988-2503, sean.palmer@va.gov, or any other VA personnel designated by the COR, at least seven (7) days prior to performance of services.
2. For testing services performed during normal work hours, 7:30 a.m. to 4:00 p.m. at the Richard L. Roudebush VA Medical Center, contractor’s tester shall report to the Operations and Maintenance Supervisor, Building 1, Room A-B117. If unavailable, report to Facilities Engineering, Building 5, Room 110.
3. The contractor shall respond to routine communications within 48 hours.
4. The contractor shall provide emergency response, including a site visit if requested, within 4 hours of the request, 24 hours a day, and 365 days per year.
5. Situations requiring more frequent analysis and/or special testing other than the specified requirements may arise. Contractor shall not perform any extra cost work without written authorization from the contracting officer.
6. Any deficiencies or problems noted with VA test results or with the water treatment system test results shall be reported immediately to the COR. If problems are identified, the contractor shall recommend corrective actions immediately.
Deliverables
1. The contractor shall submit an electronic version of the complete laboratory report each quarter.
a. The reports shall include:
i. Results of each test
ii. Date and time of each test
iii. Field testing parameters
iv. Location of sample
v. Recommendations for remediation
vi. Chain of custody information
b. For any products utilized in the testing process, the following shall be documented on the report:
i. Name, ingredients, control range, test procedures, physical state (liquid, powder, etc.), container size, method of delivery, compatibility with present materials being used, catalog data, and Safety Data Sheets (SDSs).
2. The reports shall be sent to the COR, Sean Palmer (sean.palmer@va.gov), and any other VA personnel designated by the COR.
Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI): The Richard L Roudebush VA Medical Center shall not furnish parts and/or any other test equipment for the performance of this contract. It is the responsibility of the contractor to bring the appropriate equipment; and/or supplies necessary to complete the work as required per this contract.
Other Pertinent Information or Special Considerations: All work shall be performed by personnel qualified to work on the specific equipment. All work performed shall be compliant with VA Directive 1061 guidelines and shall be designed to assure that the Richard L Roudebush VA Medical Center is providing the best possible engineering controls to inhibit the growth of Legionella bacteria.
