The purpose is to identify contractors available to complete the elevator repairs, NAICS 238290, size standard $15 million.

Statement of Work VA Ann Arbor Healthcare System
I. Location and Description of Work As a continuation of the pre-maintenance repair period the VA is requesting high callback and safety items (certain door locks, hanger roller wheels, and door guibs) be replaced with new. These, if left unattended will continue to cause service interruptions, shut downs, and possible entrapments.

II. Period of Performance Monday, Wednesday, Friday: Up to 10 weeks from award.
Safety Safety shall be the Contractor’s highest priority. OSHA Recordable Rate shall be no more than 1.0. Contractor shall be a certified member of ISN World Safety Charter Member or equivalent. The Contractor shall be solely responsible for compliance with (Federal) OSHA regulations. The Contractor shall provide and have in place Personal Protective Equipment (PPE), safety signage, cones, barricades etc., to protect the safety of VHSO patients, employees, visitors, volunteers, tenants and others. Contractor shall provide required safety items. Work shall be conducted in a manner and at times which would present the least amount of disruption to the VA healthcare facility. The Contractor shall provide full elevator service, in compliance with the ASME Safety Code requirements, manufacturer's recommendations, Elevator Industry Field Employees’ Safety Handbook, National Electrical Code, and all other applicable laws, regulations, rules, ordinances, codes, etc. Contractor shall provide a detailed explanation as to how the project will be implemented and managed to ensure compliance with all OSHA Construction Standards, Life Safety issues, and security measures. Explain approach to communicating, implementing and enforcing OSHA required safety training and requirements, as well as all other applicable state, local and Government safety requirements. Define and discuss the implementation of specific safety procedures including facility assessments, inspections, documentation and communications necessary to support assuring the mitigation of safety incidents over the life of the project. Contractor shall have a Site Safety and Health Officer (SSHO) assigned to the project to ensure their technicians are meeting OSHA requirements. The SSHO shall be well versed in OSHA requirements and regulations. The SSHO will perform regular safety inspections on the job site to ensure technician compliance and quality of work is at a high level that presents no risks to staff or patrons. Safety inspections will include fall protection and electrical hazards and end with job safety meetings with the entire crew.
Qualifications The successful contractor will have experience servicing all brands, makes and models of the elevators at all locations as well as experienced and qualified staff with outstanding quality and safety records. The North American Industry Classification Code (NAICS) for this procurement is 238290 with a small business size standard $15 million. QEI: Elevator Safety Inspector Qualifications: Pursuant to the American National Standards Institute/ American Society of Mechanical Engineers (ANSI/ASME) A17.1 section 1000, ASME A17.2 and ASME 17. 3, inspection contractor shall provide proof of Qualification of Elevator Inspectors (QE-1) certification. All inspectors or inspector’s assistants shall be an ASME/NAESA qualified elevator inspector. QEI Facility Assessment shall be performed by an established elevator company with at least 5 years experience as an elevator company with all requisite qualifications and certifications listed herein. QEI Inspector shall be made available within a two day period from notification for additional onsite consultation and to clarify questions that may arise as a result of inspections at the discretion of the Contracting Officer or representative at no additional charge. Inspector shall be available for phone, fax, or e-mail consultation during normal business hours. Contract Manager: The contract manager responsible for the management and scheduling of work to be performed under this contract shall possess at least 5 years of recent (within the past 7 years) experience in managing personnel responsible for the operation and maintenance of elevator equipment of the approximate size, supervisory control systems, complexity and other characteristics of the elevators to be operated and maintained under this contract. Supervisor: The supervisor responsible for directing the work to be accomplished under this contract shall possess at least 5 years of recent (within the past 7 years) experience in directing operation and maintenance of elevator equipment in a supervisory capacity for elevators of the approximate size, supervisory control systems and other characteristics of the elevators to be modernized, repaired and maintained under this contract. Journeyman Elevator Mechanics: All elevator mechanics shall be members of the International Union of Elevator Constructors (IUEC). Those engaged in the contract activities specified herein must be of a journeyman status as recognized by the industry and be licensed. The license must be of a grade or other level consistent with the requirement of the work being performed and/or as established by the referenced jurisdictions. A copy of the license for each employee shall be furnished to the CO or their designee no later than five (5) days after award. All licenses must be kept current for the duration of the contract. Each mechanic must possess at least 3 years (within the past 5 years), experience in the modernization, maintenance, repair, installation and/or inspection of elevators, elevator systems and equipment of equal or greater size, scope and complexity as those covered under this contract. All journeyman mechanics shall provide a certificate of completion indicating they have successfully passed an examination of a nationally recognized training program for VTE (National Elevator Industry Education Program (NEIEP). Elevator Mechanic Helpers: Elevator Mechanic Helpers may be utilized to perform work for which they are qualified, under the direct supervision of an Elevator Mechanic. Elevator mechanic helpers are not authorized to independently perform any of the work covered under this contract. Contractor shall provide evidence indicating mechanic helpers are involved in the nationally recognized training program for VTE, National Elevator Industry Education Program (NEIEP). Elevator expertise shall be validated based on participation in the NEIEP.
License and Certifications:
Describe in detail how the Contractor will provide personnel that meet experience requirements, assuring the Government all temporary or replacement employees will meet the experience and license requirements defined in this contract.
Past Performance Provide experience documentation of the Prime Contractor and/or their sub-contractors of specific projects (minimum of three) relating to statement of work specifications and drawings of this solicitation; successfully completed or in progress, that was coordinated and performed in the last 3 years of a similar size and magnitude, scope and complexity. Contractor shall provide the following information for each contract/project submitted for evaluation: Provide the names of the general contractor’s proposed project management personnel (i.e. Site Superintendent, Project Manager,) along with a description of the individual’s experience (i.e. certifications, resumes, training) and a list of successfully completed projects of similar size as stated in scope, and complexity within the last 3 years. Superintendent shall have a minimum of three (3) years of experience. Identify experience managing or supervising projects of similar size and scope. Offerors shall provide a list of sub-contractors that you intend to use for this proposed project and include all recent (within last 3 years) and relevant experience of the sub-contractors working on projects of similar size, scope, and complexity. Offerors shall provide information about the business or firm’s resource capabilities to ensure successful completion of this requirement. Past performance will be evaluated for quality, schedule (recent), price control, business relations, management, relevance. If no past performance information is readily available, the offeror may not be evaluated favorably or unfavorably on past performance. FAR 15.305 will be used to evaluate past performance of prospective offerors. Responsibility determination will be made in accordance with FAR 9.1 Responsible Prospective Contractors. Contractor shall provide completion of at least three (3) projects within the last 3 years ongoing or completed in direct performance of work similar in size, scope, magnitude and complexity to this requirement (Prime Contractor experience cannot be substituted with the experience of sub-contractors). The offeror shall have demonstrated ability to perform successfully in accordance with approved progress schedules under previous construction projects. Offerors shall prepare and forward to prior client(s) for submission to Contracting Office PAST AND PRESENT PERFORMANCE QUESTIONNAIRE for all the projects listed by offeror. This includes correspondence explaining the requirement to obtain an independent evaluation of prior contract performance and the Past and Present Performance Questionnaire for use in evaluating Past Performance. Proposals that do not include a “completed” past performance survey shall be rated neither favorably nor unfavorably. The performance risk shall be considered unknown and rated “neutral”. In order to be a “completed “ past performance survey, shall include a correct phone number, a correct email address and all requested information on the Survey Form Exhibit. Completed Past Performance Questionnaires shall be submitted to the Contracting Office so as to arrive not later than the closing date/time for receipt of proposals. Completed questionnaires shall be forwarded via email to [POC]. NOTE: Offeror shall not review the Past Performance Questionnaire prior to submission. Past Performance Questionnaire must be submitted by the client/evaluator directly to the Contracting Office. It is the Offeror’s responsibility to verify with each of their customers to ensure the completed questionnaire was provided to the Contracting Officer by proposal due date and time for receipt of offers. Offers shall address the following sub-factors in narrative. Offers that do not describe each topic in enough detail or provide the requested documentation will be rated unacceptable.
1. Provide documentation of specific elevator projects (minimum of three) successfully completed or in progress, that was coordinated and performed by the prime contractor in the last 3 years of a similar size in magnitude, scope and complexity, in the renovation as stated in SOW (provide all requested information).
2. Provide the names of the prime contractor’s proposed project management personnel (i.e. Site Superintendent, Project Manager,) along with a description of the individual’s experience and a list of successfully completed projects of similar size, scope, and complexity within the last 3 years.
3. Provide a list of sub-contractors that you intend to use for this proposed project and the percentage of labor they will perform.
4. Provide information about the business or firm’s resource capabilities to ensure successful completion of this requirement.
Best Value The offeror who demonstrates the “best overall value” to the government shall be selected for award. The Government intends to evaluate proposals and award a contract without discussions with offerors. The Government intends to award a single contract to the offeror deemed responsible in accordance with the Federal Acquisition Regulation (FAR) whose proposal conforms to the RFP’s requirements and judged to represent the “best value” to the Government and those proposals demonstrating an ability to exceed specified requirements may be rated higher in those areas than proposals demonstrating only the ability to meet requirements. The “best value” is the most beneficial (advantageous) offer, price and other factors considered, and consistent with the Government’s stated importance of evaluation criteria. This may result in award being made to a higher-rated, higher-priced offeror where the Contracting Officer determines that the Technical Evaluation of the higher-priced offeror outweighs the cost difference. To arrive at a Best Value decision, the Source Selection Evaluation Team (SSEB) shall integrate the evaluation of Technical Capability, Experience, Quality and Past Performance. While the Government and the SSEB shall strive for maximum objectivity, the evaluation process, by its nature, is subjective and therefore, professional judgment is implicit throughout the entire evaluation process. As such, offerors are encouraged to provide their best possible pricing initially as Final Proposal Revisions may not be requested. However, the Government reserves the right to request clarifications and/or conduct discussions if determined by the Contracting Officer to be in the best interest of the Government at any time leading up to contract award. Award of a contract shall be based on “best value” to the Government with technical capability and past performance being equal and when combined are significantly more important than price. An offeror must be determined to be technically acceptable and pricing determined to be reasonable based on fair market value for project cost.
