Statement of Work
1. INTRODUCTION
This contract is to provide on call pickup and delivery of medications FROM Lexington VA Pharmacy located at 1101 Veterans Drive, Lexington, KY 40502 TO designated sites at the Leestown campus of the Lexington VA Medical center at 2250 Leestown Road, Lexington, KY 40511. Service is required to be available 24 hours a day, 7 days a week, including holidays.
Most trips will be outside of normal business hours. (After 4:30 p.m., Saturdays, Sundays, and Holidays). Additional stops are required at different nurse’s stations within the same facility.
The following is additional clarification regarding CLIN 0003, Additional Delivery Stops:
1. This CLIN will not be in use by itself. It will always be in conjunction with either CLIN 0001 or CLIN 0002 if additional deliveries are required.
1. The additional stops are nurses’ stations that are within the same building as the initial delivery destination which is associated with CLIN 0001 or CLIN 0002. The delivery driver to drive to another location.
The medications will be transported in a bag as supplied by the pharmacy to the nursing station identified at Leestown campus. Driver will sign pick up and drop off log for each bag. Controlled substances will not be transported by this contract.
VA IT Security System Certification & Accreditation requirements do not apply therefore a Security Accreditation Package is not required.
Invoicing shall be done monthly in arrears, and each invoice shall be itemized by delivery event.
2. REQUIREMENTS
The contractor will provide a driver and vehicle service between two locations on-call.
Patient information must be protected in a confidential manner in compliance with Health Insurance Portability & Accountability Act (HIPAA) regulations.
Drivers must be fully licensed and insured.
3. DELIVERY
Pickups shall be made at Pharmacy call window located in the facility at 1101 Veterans Drive, entry through main entrance. A log shall be signed with time and location. The delivery address will be noted on each bag by building number.
Pickup and delivery will be on an on-call/as-needed basis. Pick up following a request shall occur within 60 minutes and the delivery shall arrive within 30 minutes of pickup time. Contractor shall perform these services (pick-up and delivery) in these stated timeframes at least 90% of the time.
Courier contactor personnel responsible for performing each pickup and delivery shall signify completion of the task by signing the delivery ledger.
The contractor courier shall provide telephone contact information so that pharmacy personnel can inquire about the disposition/status of pickup and delivery events.
