STATEMENT OF WORK
1. GENERAL: C.W. Bill Young Veterans Medical Center has a requirement for the Replacement of the Emergency Call System. The contractor will provide all labor, tools, materials, transportation, and supervision to replace the station emergency call system. This is to be a turn-key system. The Towers shall have two phone lines as designated by campus safety and OI&T.
2. SCOPE: This Statement of Work is for the replacement of the emergency call stations located at the C.W. Bill Young VA Medical Center. Contractor shall conduct the following work:
a) The contractor shall supply all supervision, labor, and materials for this job. (Note, items due to become obsolete or not supported by the manufacturer are prohibited).
b) Contractor is not to shut down or perform work without authorization from the COR.
c) The contractor shall apply Lock Out Tag Out to all sources of energy.
d) The contractor will complete all necessary tasks and present an itemized list of materials/parts will be presented to engineering for approval. This is to cover any unforeseen conditions that arise during the replacement process.
e) The contractor shall coordinate with OI&T for any necessary phone lines or SIM cards and ensure proper operation.
f) The contractor shall remove existing (8) emergency phone towers and (5) wall mounted emergency phones.
g) The contractor shall be responsible for replacing any land lines found not in operation.
h) The contractor shall provide and install (8) emergency cellular phone towers.
i) The contractor shall supply and install (5) emergency hard wired emergency phones in the parking garage.
j) The contractor, when ready to test, shall communicate with the VA for performance verification.
k) The contractor shall test out all call stations and verify with OI&T correct phone numbers.
l) The contractor shall provide the results of the test to the COR.
m) The contractor shall perform a “clean Sweep” after the work is completed.
3. CONTRACTOR REQUIREMENTS: Contractor shall be a certified telecommunications installation mechanic and shall provide evidence of certification prior to award. THE CONTRACTOR SHALL FOLLOW ALL GUIDELINES REFERENCED IN THE LATEST EDITION OF THE NEC, NFPA 70E, OSHA STANDARD 29 CFR 1910 SUBPARTS I & S AND MANUFACTURE’S RECOMMENDATIONS.
a) COORDINATION: The contractor shall attend a kickoff meeting with the Electric Shop, Contracting Officer’s Representative, and other applicable personnel to discuss procedures and OSHA requirements. Contractor shall not manipulate any equipment owned by the facility.
b) RIGGING: Contractor shall be responsible for providing rigging, if necessary, in accordance with all OSHA regulations. A rigging plan shall be submitted to the COR 3 weeks in advance for approval prior to beginning work on site.
c) UTILITY OUTAGE: Contractor shall provide a written work plan outlining the plan and steps for accomplishing the work. This shall be submitted to the COR at least 3 weeks in advance. Work plan will be submitted to the Utility Outage Meeting 2 weeks in advance. This group meets weekly on Mondays from 230PM to 3:30PM. Contractor shall be available for this meeting if requested by the COR.
4. CONTRACTOR QUALIFICATIONS:
a) Contractors shall be experienced Contractors, having properly trained and licensed permanent staff required to perform the proposed work. Mechanical installations, electrical installations, Telecommunications installations, Photovoltaic power systems, and transformers. (i.e., the rebuilding, installation, set up, repair, and maintenance) shall be the primary interest/specialty of the Contractor’s business.
b) Contractor shall be certified by the International Electrical Testing Association (NETA), NFPA-70E, NFPA-99. or equivalent certifying organization as Certified Technicians with each having completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training and Supervisors having OSHA 30 hr. training.
c) Contractors shall have technical training and demonstratable track records of working experience in Telecommunications, inspection, repair and testing of emergency call towers, systems and related components in healthcare, industrial, educational, and commercial facilities for a minimum of five (5) continuous years on components on which the Contractors have experience shall include, but not limited to, AC electrical power systems, DC power systems, telephone systems, cellular phone communications, land line phone communications, etc.
d) Contractors shall have safety trainings – either on-the-job or class-room type – in electrical safety outlined on the OSHA Standard 29 Code of federal Regulations (CFR)1910 Subpart S – Electrical, and the NFPA 70E – Standard of Electrical Safety in the Workplace. Training certification shall be provided indicating each technician is a Qualified Person as defined by NFPA 70E. Training certificates shall be submitted to the VA Contracting Officer prior to work. If no training certifications are available, the contractors’ Employer shall certify that he/she has met this requirement in writing and submit it to the VA Contracting Officer prior to work.
e).Contractors should have ready access to the latest versions of the following references:
I. NFPA 70, National Electrical Code.
II. NFPA 70B, Recommended Practice for Electrical Equipment Maintenance.
III. NFPA 70E, Standard for Electrical Safety for the Workplace.
IV. OSHA Standard 29 CFR 1910, Subparts I & S.
V. Operating/Maintenance manuals, and specifications of the electrical/electronic equipment to be used during maintenance and testing.
f) Contractors shall have and provide all necessary supervision, tools, equipment, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Tools, equipment, and PPE shall comply with the requirements of OSHA Standard 29 CFR 1910, Subpart I, and NFPA 70E. Prior to working initiating work, Contractor shall provide documentation that all instruments, test equipment, tools and PPE have current calibrations. Dated calibrations labels shall be visible on all instruments, test equipment, tools, and PPE as appropriate.
5. PLACE OF PERFORMANCE/WORK HOURS: Campus wide as noted on the campus map. C.W. Bill Young, Bay Pines VA Hospital, BAY PINES, FL 33744. (a)(1) The North American Industry Classification System (NAICS) code for this acquisition is 238210.
(2) The small business size standard is $19 Million.
(3) The small business size standard for a concern that submits an offer, other than on a construction or service acquisition, but proposes to furnish an end item that it did not itself manufacture, process, or produce is 500 employees, or 150 employees for information technology value-added resellers under NAICS code 541519, if the acquisition—

The Contractor shall maintain registration in SAM during contract performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement. The Contractor is responsible for the currency, accuracy, and completeness of the data within SAM, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in SAM after the initial registration, the Contractor is required to review and update on an annual basis, from the date of initial registration or subsequent updates, its information in SAM to ensure it is current, accurate and complete. Updating information in SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document.
C.3 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (MAY 2024) ALTERNATE I (FEB 2024) The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v) of this provision.
(a) Definitions. As used in this provision— Covered telecommunications equipment or services has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.
Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR 127, and the concern is identified by SBA or an approved third-party certifier in accordance with 13 CFR 127.300. It automatically qualifies as a women-owned small business eligible under the WOSB Program.
Forced or indentured child labor means all work or service—
(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.
Reasonable inquiry has the meaning provided in the clause 52.204–25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.
Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;

Sensitive technology—
(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically—
(i) To restrict the free flow of unbiased information in Iran; or
(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and
(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).
Service-disabled veteran-owned small business (SDVOSB) concern means a small business concern— (1)(i) Not less than 51 percent of which is owned and controlled by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran; or

Service-disabled veteran-owned small business (SDVOSB) concern eligible under the SDVOSB Program means an SDVOSB concern that—
(1) Effective January 1, 2024, is designated in the System for Award Management (SAM) as certified by the Small Business Administration (SBA) in accordance with 13 CFR 128.300; or
(2) Has represented that it is an SDVOSB concern in SAM and submitted a complete application for certification to SBA on or before December 31, 2023.
Service-disabled veteran-owned small business (SDVOSB) Program means a program that authorizes contracting officers to limit competition, including award on a sole-source basis, to SDVOSB concerns eligible under the SDVOSB Program.
Small business concern—
(1) Means a concern, including its affiliates, that is independently owned and operated, not dominant in its field of operation, and qualified as a small business under the criteria in 13 CFR part 121 and size standards in this solicitation.
(2) Affiliates, as used in this definition, means business concerns, one of whom directly or indirectly controls or has the power to control the others, or a third party or parties control or have the power to control the others. Subsidiary means an entity in which more than 50 percent of the entity is owned—

Successor means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term “successor” does not include new offices/divisions of the same company or a company that only changes its name. The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
Veteran-owned small business concern means a small business concern—
(1) Not less than 51 percent of which is owned and controlled by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
