B.3 PRICE/COST SCHEDULE

This solicitation uses a Brand Name or Equal Description of the product required. This permits prospective contractors to offer products other than those specifically referenced by brand name. All offers must work with existing equipment that has already been purchased and is currently in use at the station and also to meet standardization requirements throughout the hospital. If substituting other than equipment listed, must provide proof that substituted equipment will interact without issue with current equipment.

If offering other than brand name. The contractors must supply adequate descriptive literature to ascertain compatibility with the information provided on all the line items on this quote. Failure to provide adequate descriptive literature with the quote will be grounds for rejection of quotation. Please provide the RFQ number to the subject E-mail address. Minimal Technical Specifications

Item must be made of surgical grade stainless steel that is not manufactured in China or Pakistan. It has been determined by surgical staff that a Surgical Retractor system is the best value for the Government based on compatibility with the sets currently owned and procedural time savings which results in cost savings. Item to be used to perform surgical procedures

1.1 Stryker GoBed EX GoBed EX II (Model # FL28EX4) or Equal Medical/Surgical

Frames specific mandatory deliverables, tasks, salient characteristics:

1.1.1 Must be designed to be Med/Surg beds, Attachment A 36C24819Q1257

1.1.2 Must be designed for industrial use in a hospital,

1.1.3 Must hold weight capacity of 500lbs.

1.1.4 Must have a deck height range from 14” to 29”

1.1.5 Must have an alert system that is fully customizable and alarms when set parameters are compromised (e.g. brake settings, side rail positions, low bed height, etc.).

1.1.6 Must have a headboard height that is vertically adjustable to allow for a safe and ergonomic height for patient transport.

1.1.7 Beds must have the capability to accurately weigh and record patients’ weights while patients are in any position in the bed.

1.1.8 Brake pedal must be easily accessible when the bed or side rails are in any position to include 5th wheel for safer transport.

1.1.9 Bed must be able to monitor and track a patients’ position in at least three (3) zones of the bed.

1.1.10 Bed must include a trapeze system for ease of patient movement.

1.1.11 Pump rack and accessory outlets must be located away from the patient egress points (e.g. at the foot-end of the bed).

1.1.12 Bed must include SPR release, restraints and drainage hooks.

1.1.13 Bed must have a retractable frame for patient safety and safe patient handling.

1.1.14 Bed must have full up, full down, and intermediate side rails positions.

1.1.15 Side rails must have two or more patient grip points for safety.

1.1.16 Bed shall be able to wirelessly communicate with our nurse call and alarm management system.

1.1.17 Bed must be compatible with hospitals current evacuation devices.

1.1.18 Bed must have a one-button feature that helps patients’ move toward the head of the bed.

1.2 Fixed Removable 3-Stage IV Poles (FA64170) – QTY 85

1.3 IsoFlex LAL 2860 (2860-000-000) – QTY 85

1.3.1 Bed mattress must have a Shear management gel layer located in the patient surface area from head to toe to redistribute pressure by absorbing patient weight.

1.3.2 Bed mattress must have a core gel and shear gel to help while patient is raised.

1.3.3 Bed mattress must have a built-in low air loss tubing.

1.3.4 Bed mattress must have denser quality foam around edges to provide support when patient is sitting on edge of bed.

1.3.5 Bed mattress must have a 10-year warranty for the foam and gel part of the mattress

1.8 Vendor delivery, installation and turn-in requirements

1.8.1 Upon award vendor is required to coordinate delivery schedule with Logistics, Biomed and Nursing Services. Scheduled deliveries shall occur over a maximum duration of a 3-week period. Weekdays, weekends and holidays will all be used inconsideration for delivery of beds to minimize impact on patient care.

1.8.2 Upon delivery of the beds the vendor is required to perform a 1-for-1 swap to include installation of the new bed/mattress and removal of the turned-in bed/mattress on the same business day. C.W. Bill Young VAMC employees will not be responsible for the storage of newly procured beds or removal of turned-in beds

B.4 DELIVERY SCHEDULE

Additional Instructions to offerors concerning Gray Market items-

The VHA (Veterans Health Administration) will not accept “Grey Market Goods”.

Grey market goods are defined as: Gray Market Goods also known as parallel market, is the trade of a commodity through distribution channels which, while legal, are unofficial, unauthorized, or unintended by the original manufacturer.

Products that are purchased outside of our authorized distributor network on the gray market generate a number of concerns such as: counterfeit or adulterated devices, inadequate storage conditions, tracking issues in case of a recall, loss of warrantee, and most importantly patient safety. In some instances, unauthorized distributors procure products from an authorized distributor for resale.

1. Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

2. This procurement is for new Equipment ONLY; no remanufactured or "gray market" items. All

Equipment must be covered by the manufacturer's warranty.

NOTE: Offerors/quoters that are not the OEM must provide the following information as a part of their quote: Submit original documentation from the manufacturer stating that they are an authorized distributor for the specific items being procured (this letter should be on the manufacturers’ official letter head).
