STATEMENT OF WORK

Patient Instructions

GENERAL INFORMATION

1. Title of Project: Purchase of LOGICARE Patient Instructions and initial deployment, integrated for the Bay Pines Veterans Healthcare System. License for use encompasses unlimited workstations in the Emergency Department and Acute Care.

2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. This is a request to purchase LOGICARE Patient Instructions which offers customizable instructions for diagnoses, medications, treatments, medical devices, recommended diets, and follow-up. The comprehensive library offers nearly 6,000 patient instructions for Emergency Departments, inpatient units and clinics. The automated process eliminates printing and scanning instructions into Computerized Patient Record System

(CPRS) and tailors the information given to the patient by the provider.

3. Background: LOGICARE can communicate with the Veterans Information Systems and technology Architecture (VISTA) and transfer patient information to CPRS as a signed

CPRS note. LOGICARE will automatically include the name of the patient’s primary care provider, next appointment date, any new medication ordered during the last encounter, as well as any other information BPVAHCS deems necessary, such as My HealtheVet, flu vaccination information, etc. LOGICARE can be initiated by any clinician providing health education and includes topics in the prevention, inpatient, ambulatory surgery, outpatient, and emergency room areas. Purchase of this system includes centralized training for staff as well as around-the-clock user, technical support services, and content updates.

4. Performance Period: This contract will be for a Firm fixed price with a base and four (4) option years.
