Questions and Answers:
ELEVATORS INSPECTION AND CERTIFICATION
1. Does the Combined Synopsis/Solicitation posted as solicitation number 36C24819Q0144 on FBO contain requirements similar to an existing contract or will this be a new requirement?
There was a contract that already expired. Contract #VA248-13-C-0136.
2. Given that this is a VA contract through the U.S. Government (involving a VA facility owned by the U.S. Government), do inspectors have to be licensed in Puerto Rico (specifically) to be eligible to perform this work, or are U.S. state licenses acceptable?
Yes, although inspectors license in US can perform work under this contract, inspectors must be licensed in Puerto Rico to be in compliance with PR Occupational Safety and Health Administration (PROSHA) Regulation No. 18 and PR Department of Labor.
3. Is a QEI Elevator Inspector’s License required?
Preferable but not required.
4. Are annual (yearly) and periodic (every 6 months) inspections required?
Annual or every 5-year inspection, per PWS.
5. “Safety tests and other tests and inspections shall be performed by the Contractor.” Question – Please confirm responsibilities of this requirement?
Must be conducted along with the Elevator Preventive Maintenance Contractor and report any deficiencies in the annual report.
6. B.3 “Contractor shall perform all safety tests and other tests and inspections as recommended and required by ASME A17.1 Latest Edition, ANSI/ASME A17.2 Latest Edition and the Puerto Rico Occupational Safety and Health Administration (PR OSHA) Regulation Number.” Question - Are we to actually perform the “test” and witness the test, or is your elevator maintenance provider performing the test and we witness?
Elevator Maintenance and Repair contract will conduct load tests along with Elevator Preventive Maintenance Contractor. Elevator inspector will witness and validate that elevator is acceptable and is fully functional.
7. B.3 “All elevators provided with firefighters’ service shall be subjected monthly to Phase I recall and a minimum of one floor operations on Phase II to assure the system is maintained in proper operating order in accordance with A17.1 Code. A written record of findings on the operation shall be made by the Contractor and kept on the premises of said operation. (See Attachment C for firefighters’ test log). Testing shall be done on weekend, holidays or the hours before 06:30 AM or after 6:30 PM, at no additional cost to the Medical Center.” Question – Is our inspector required to be onsite monthly and to exercise Phase I and II, or is your elevator contractor performing and we verify the test log during our inspections? Phase I and Phase II Firefighting tests are required as part of the Annual Inspection Certification. This requirement will be applicable only during firefighter tests are conducted. Monthly requirement will apply only to Elevator Maintenance and Repair contractor.
8. B.3 “During the visit the Contractor shall be available to answer any doubts the COR could have regarding the Elevator being inspected or any other one under Contract.” Question – Please explain why there may be doubt of an inspection properly taking place. Should not all of the required Inspector’s checklists and documents provided at time of our completed inspection satisfy the requirement?
VACHS can have questions regarding the inspections and testing and contractor is require to response to these questions. If there is a need to clarify any situation encountered during the inspection for example, inspections cannot be conducted until deficiency is corrected.
9. B.3, 1., B, 16. “The Contractor shall verify and certify that the following performance levels shall be maintained at all times:
• Contract speed and brake to brake flight time shall be maintained as originally installed and adjusted.
• Leveling accuracy shall be maintained at all times.
• Opening and closing times, door close torques of all cars shall at all times be maintained within the limits of ASME A17.1 Code with a minimum of stand open time consistent with traffic demands at each floor.
• Door reversal on all elevators equipped with mechanical safety shoes shall always be initiated with the stroke of the shoe. Ensure all electric eyes or door detectors are working at all times.
• Variable car and hall door open times shall be maintained in accordance with original field adjustments. Deviations from this will not be permitted unless requested and/or approved by this COR.
• Upon satisfactory completion of this or subsequent readjustments, a computerized elevator analyzer or similar type event recorder shall be utilized to accurately record the hall waiting time at each landing and other data necessary to establish that the system is operating at peak efficiency. Results shall be submitted and be labeled as to floor, time, date and identification of all other data and shall be delivered together with an analysis of the tapes/disks to the COR.
Question – Please confirm these tasks associated are those required for this RFQ. These tasks are normally associated with an elevator service contract and not an inspection contract.
Tasks listed in the paragraph above correspond to Elevator Maintenance and Repair Services Contract. Please dismiss requirement. See revised PWS.
10. B.3. “Contractor must consider that the inspection could be done on weekdays after 4:00 pm or on weekends. The Contractor shall exclude Federal holidays, unless specifically instructed otherwise by the COR.” Question – Please indicate the hours we are allowed to perform all inspections.
Refer to the hours established in the scope of work. It is important to emphasize that inspections cannot interrupt normal hospital operations. However, a work schedule can be discussed with the CO and COR. Inspections must be conducted with VACHS Elevator Maintenance and Repair Contract.
11. B.3 “When planned work requires an elevator to be taken out of service, the Contractor is requested, when possible, to use the Federal holidays listed below.” - Question – All inspections are scheduled; will this require them to be performed only during the federal Holidays listed?
Contractor is required to coordinate with the COR prior to place out of order any elevator with is required including emergency situations. A work schedule can be discussed with the COR.
12. B.3 “The safety and buffer tests as required by the American National Standard Safety Code for Elevators (A17.1 and A17.2) and PROSHA regulations are to be performed by the Contractor.” Question – Please define “Contractor” – is this your elevator service provider or do you expect the inspector to perform these tasks?
Elevator Inspector must conduct safety tests and buffer tests.
13. B.3 items 33 and 34 “inspections” and” Testing” bullets in their entirety – Question - This language does not seem correct for an inspection contract. Please confirm.
Please refer to revised scope (Items 1.B.28 & 29)
14. Are there provisions for a re inspection fee? If an elevator fails a test or repairs are done that require an inspection outside of the scope of work and a return trip is necessary, how will this be handled?
Re-inspections cost can be included in the total cost per inspections.
15. B.3, #s 4 and 5: is there a separate registration process/form for each registration, or are these registrations one and the same? Please elaborate.
It’s the same registration. Please refer to revised scope (see item 1.B.5)
16. Section B.3(1)(B) – Scope of Work. Would you please specified clearly which are the edition of A17.1, A17.2, A17.3, and NFPA 70 which will govern this solicitation?
The latest edition-2016.
17. After the site visit I noticed that the elevator certificates posted in all elevators visited does not comply with the Puerto Rico Jurisdiction of the Department of Labor (DOL). They does not have the DOL logo neither have assigned a control number as per Regulation 18 of the DOL. On the other hand, the facilities where the inspection and test will be conducted are covered by the Federal Laws of the USA. Would you please specified clearly which regulation will govern this contract?
Under this new contract, this requirement will be met. Certification is required to state that meets compliance with PR Department of Labor, PR OSHA Regulation 18.
18. Item (5) and (6) requires that the “Contractor shall be registered at the Puerto Rico Department of Labor, Elevators and Boilers Division IAW PR DOL Regulation Number 18”. However, the RFQ does not require to be an Inspector with a license issued by the Department of Labor.
Inspector must be registered in the PR Department of Labor in order to meet PR OSHA Regulation 18.
19. Would you please specified clearly if the Contractor shall be licensed by the Department of Labor? Please, take in consideration the previous question (c) above to answer this question.
Under this new contract, this requirement will be met. Certification is required to state that meets compliance with PR Department of Labor, PR OSHA Regulation 18.
20. ASME A17.3 has not been adopted in Puerto Rico Jurisdiction, but it is required to comply by this RFQ. Would you please specified clearly if this standard (A17.3) will be part of this solicitation? Please, take in consideration the previous question (c) above to answer this question.
Only applicable standards are required for this solicitation. However, PR Department of Labor and PROSHA have adopted the ASME standards for Elevators.
21. Item (33) requires: “Periodic inspections of the elevators required by the National American Standard Safety Code for Elevators and Escalators (Latest Edition) will be performed under separated contract in accordance with existing San Juan VAMC policy. Upon completion of a routine inspection, the Contractor will be furnished with an inspection report listing deficiencies for which he is responsible to repair within 30 days. Items marked emergency should be corrected immediately. Under no circumstances, will the Contractor allow reported deficiencies to go uncorrected beyond the contract period.” Would you please specified clearly if this Contract Scope includes to make repairs of the equipment reported by other inspections?

22. Would you please specify clearly if the Contractor, according to this RFQ, is the same company or a different one from the elevator maintenance & repair contractor?
No. Elevator Maintenance and Repair Contractor will accompany the Third-Party Elevator Inspector to be selected under this contract.
23. What will be the formats to be used to carry out the annual inspections, load tests & inspection certificates?
An inspection report is expected with narrative, photographs and deficiencies including a checklist for all items to be inspected. Inspection certification will be similar from previous certifications but it is requirement to state that meet compliance with PR OSHA Regulation 18.
