a. Under this contract, unless otherwise delegated in writing to the Contracting Officer’s Representative (COR), the Contracting Officer is the only person authorized to approve changes or modify any of the requirements. The contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction ,of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. The Contracting Officer is responsible for the overall administration of the contract.
b. Representatives of Contracting Officers in accordance with VAAR 852.270-1, January 2008:
The Contracting Officer reserves the right to designate representative(s) to act for him/her in furnishing technical guidance and advice or generally supervise the work to be performed under this contract. Such designation will be in writing and will define the scope and limitations of the designee's authority. A copy of the designation shall be furnished to the contractor.

B.2 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND
COMPLIANCE (JUN 2011)
This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause.
To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

REPLACE DOOR KNOBS AND HANDRAILS
Target Completion Date: No later than 270 days after award
I. PROJECT GOAL:
This project requires a contractor to replace existing door knobs and handrails at the Bruce W. Carter VA Medical Center (BWCVAMC) located at 1201 NW 16th Street Miami, FL.
II. STATEMENT OF WORK (SOW):
1. The contractor shall replace existing door knobs to levers and existing handrails at specified location in the Main Building of the BWCVAMC in accordance with all federal, state, local or any other governing body – policy procedure, legal or otherwise doctrine for the proper intended performance of the equipment.
2. The contractor shall ensure the following 240 (+/- 10) door knobs are replaced with Schlage ND53LD RHO 626 levers at the following locations:

3. The contractor shall remove handrails, crash rails and base rails in corridors to the following floors:
12, 11, 10CD, 9, 8, 7, 6, 3 and 2.
4. The contractor shall install new handrails, crash rails and base rails exactly like those currently installed on the first floor of the main building for uniformity. (See Section D.2 for model information.)
5. The contractor shall remove existing wall coverings and patch and paint wall corridors to accommodate for the new handrails.
6. Contractor shall furnish sufficient technical, supervisory and administrative personnel to ensure the expeditious accomplishment of the work as specified in this SOW.
7. Contractor shall establish barriers to prevent pedestrian travel while is in progress.
8. Contractor shall ensure only authorize personnel enter the containment area in the proper personal protection equipment.
9. All work performed shall be fully documented by the Contractor.

10. Contractor shall apply environmentally-friendly technology and practices as much as feasible, including recycling/reclamation of the devices removed.
III. SPECIAL REQUIREMENTS:
1. VA Police will issue identification badges to contractors, employees and subcontractors which must be worn always when on VA Property.
2. Contractor shall provide a list of all personnel who will participate on the project. Personnel shall be subjected to a background investigation before commencement. After background check, has been completed identification badges will be issued.
3. Contractor shall keep the worksite cleaned daily and haul away all debris. Do not leave tools unattended outside of BWCVAMC. All materials and debris shall be in covered cart when entering and exiting the building.
4. Utility Shutdowns/Outages: All shutdowns involving temporary impairment of utilities shall be requested by the Contractor at least 14 calendar days in advance if time permits.
IV. QUALIFICATION/CERTIFICATION:
1. All work shall be performed at the BWCVAMC and shall conform with current codes and standards as they relate to the facility.
2. All required licenses and permits shall be the responsibility of the Contractor.
3. Contractor and all contractor employees must have taken the Occupational Safety and Health
Administration (OSHA) Certified 30/ 10-hour construction safety course and show documented proof. The contractor shall designate a “competent person” in accordance with 29 CFR 1926. 32(f) which states, “One who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous or dangerous to employees and who has authorization to take prompt corrective measures to eliminate them”. The contractor shall instruct each of his employees in the recognition and avoidance of unsafe conditions and the regulations applicable to this work environment to control or eliminate any hazards or other exposure to illness or injury.
V. SAFETY REQUIREMENTS:
1. Comply with all project specific Safety, Health and Fire measures per BWCVAMC requirements.
2. Maintain site and building access free and unobstructed to facility emergency services.
3. All temporary worksite partitions shall meet Safety, Health and Fire measures according VA requirements.
4. Temporary heating and electrical installations shall comply with OSHA 1926, National Fire
Protection Association (NFPA) 241 and NFPA 70.
5. Means of egress shall not be blocked.
6. Egress routes for contractor’s workers shall be maintained free and unobstructed.
7. Contractor furnished fire extinguishers must be provided and maintained in work areas and in temporary storage areas.
8. Flammable and combustible liquids must be stored, disposed and used in accordance with OSHA
1926, NFPA 241 and NFPA 30.
9. Acquire and maintain all construction permits per BWCVAMC Safety and Health During
Construction Activities Policy 001SEM-36-13 and Construction Policy 001SEM-50-13. All permits shall be visible from commencement to end of project.
10. Smoking inside BWCVAMC is prohibited.
11. Waste and debris from work areas shall be disposed daily.
VI. EQUIPMENT:
1. All Equipment, tools, etc., shall be maintained while on the project and shall be adequate, in quality and capacity for proper execution of work.
2. Under no circumstances tools or equipment shall be left unattended.
VII. SCHEDULING AND HOURS OF WORK:
All work in areas involved shall be scheduled with the BWCVAMC Engineering Contracting Officer’s Representative (COR). Work shall be scheduled for proper execution to completion of this contract.
VIII. SAFETY REGULATIONS:
1. All work performed under this contract shall be in accordance with Safety and Health during Construction Activities Policy 001SEM-36-13 and Construction Policy 001SEM-50-13. Provide a written certified statement indicating that the project meets the life safety code requirements. For those maintenance and repair projects where the life / safety code requirements are not applicable, a written statement shall also be provided accordingly.
2. The BWCVAMC Safety Officer and COR shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract.
3. Provide all necessary barricades / signage where required.
4. Isolate the area to prevent the spread of debris into adjacent space and maintain the area negatively pressurized when required.
5. Contractor shall perform field test and participate on final acceptance tests for new equipment or systems installed when applicable.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.
Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

E.1 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
This is a competitive Request for Quote (RFQ) conducted under FAR Part 13.106-2(b)(3) - Comparative Evaluation source selection process.
The Government will conduct comparative evaluations of all quotes received, and award to the response which represents the best overall benefit to the Government. Each response must at minimum meet the solicitation requirement statement. Responses may include, but not limited to: any/all applicable Federal, State and Local certifications; the procedural plan that indicates the time and sequence, experience, past performance, and price for the completion of the requirement.
Quotes will be compared to other responses received to achieve the best overall benefit to the Government. Once the Government determines that there is a quoter that provides the best overall benefit it may communicate with only that quoter to address any remaining issues.
The government will conduct a comparative assessment of offeror s responses based upon the following criteria:
Delivery – Offeror s clearly demonstrates in its response a clear understanding of the Government’s requirements for scheduling and completion of requirement with PWS.
Offeror provides a clear and effective approach in providing the required services within the time frame.
Qualifications: Offeror s clearly demonstrates in its response a clear understanding of the Government’s requirements for qualification and certifications to any/all applicable Federal, State and Local certifications. offeror provides certification of applicable Federal, State and Local certifications.
Experience: Offeror s clearly demonstrates in their response of detailing their experience in a successful competition of similar type and size requirement.
Past Performance: Offeror must have a record of satisfactory or neutral past performance.
If no record of past performance is found the Offeror will receive a rating of neutral. The Government will utilize internal documentation including Federal Awardee Performance and Integrity Information System (FAPIIS) and Past Performance Information Retrieval System (PPIRS) for determining the Past Performance of Offeror.
The Government will review the price schedule for completeness and accuracy and will evaluate the reasonableness of the quoted prices. Normally, adequate price competition establishes a fair and reasonable price. Government reserves the right to award to a offeror with other than the lowest price.
Award will be made to the offeror whose quote represents the best overall value to the government.
The Government reserves the right to select a response that provides benefit to the Government that exceeds the minimum. The Government is not required to select a response that exceeds the minimum.
The Government is not accepting alternate quotes; only one (1) quote shall be submitted by the quoter. Be sure to respond to any information the Government is requesting within this Request for Quote.
All quoters who are Veteran concerns must be certified as Service Disabled Veteran Owned Small Business (SDVOSB) in Vetbiz/VIP: https://www.vip.vetbiz.gov/ at the time of solicitation closing. All quoters must be fully registered and active in SAM: https://www.sam.gov/ at solicitation closing.
(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

E.2 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS—
COMMERCIAL ITEMS (NOV 2017)
The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site located at https://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision.
(a) Definitions. As used in this provision— Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.
Forced or indentured child labor means all work or service—
(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or
(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.
Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.
Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.
Inverted domestic corporation means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.
Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174).
Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;
(3) Consist of providing goods or services to marginalized populations of Sudan;
(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;
(5) Consist of providing goods or services that are used only to promote health or education; or
(6) Have been voluntarily suspended.
“Sensitive technology”—
(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically—
(i) To restrict the free flow of unbiased information in Iran; or
(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and
(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).
Service-disabled veteran-owned small business concern—
(1) Means a small business concern—
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.
(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
Small business concern means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.
Small disadvantaged business concern, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that—
(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by—
(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and
(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.
Subsidiary means an entity in which more than 50 percent of the entity is owned—
(1) Directly by a parent corporation; or
(2) Through another subsidiary of a parent corporation.
Successor means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term “successor” does not include new offices/divisions of the same company or a company that only changes its name.
The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
Veteran-owned small business concern means a small business concern—
(1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C.
101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
Women-owned business concern means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of its stock is owned by one or more women; and whose management and daily business operations are controlled by one or more women.
Women-owned small business concern means a small business concern—
(1) That is at least 51 percent owned by one or more women; or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
(2) Whose management and daily business operations are controlled by one or more women.
Women-owned small business (WOSB) concern eligible under the WOSB Program (in accordance with 13 CFR part 127), means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States.
(b)(1) Annual Representations and Certifications. Any changes provided by the offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications posted on the SAM website.
(2) The offeror has completed the annual representations and certifications electronically via the SAM website access through http://www.acquisition.gov.
