SOLICITATION NOTICE OF TIERED EVALUATIONS. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C24818Q9680 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99-1 Effective 07-16-2018. This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127. TIERED EVALUATIONS LIMITED TO SDVOSB or VOSB: This solicitation is being issued as a tiered evaluation for SDVOSB concerns. If award cannot be made, the solicitation will be cancelled and the requirement resolicited.

The associated North American Industry Classification System (NAICS) Code for this acquisition is 339113, size standard 750 employees.

SALIENT CHARACTERISTICS for endoscope reprocessing sinks are:

A. Flushing Aid in Sink MUST be fitted with a Class II, 510K approved flushing aid per FDA Guidelines

1) Sink Basin size should be adequate to ensure the endoscope can be positioned without tight coiling 16”x30”. The basin should be about 8-11” deep

2) Sinks must fit in an area for 3 North. The space is about 117” or less for the large sinks, 3-5 basins (note the sink cannot be built fully as a 117” sink because we could not fit it in the elevator, best options would be two 3 basin sinks side by side or a three basin and two basin sink side by side, longest complete sink can’t be longer than 70”)

3) Sink on opposite side of the wall must not be more than 54” long, 2-3 basins

4) Sink for Cidex Decontamination area 120” or less, 3 basins (note the sink cannot be built fully as a 120” sink because we could not fit it in the elevator, longest complete sink can’t be longer than 70”)

5) Cidex Clean area sink must not be over 30” long, 1 basin

6) All sinks must be automated height adjustable sinks.

7) All sinks must be made of heavy duty stainless steel construction

8) All sinks must have a backsplash

9) Sinks must have a lifted edge around work surface to prevent fluid runoff

10) Sinks must be fully automated and have flushing and purging ports inside basin

11) Sinks must have etched gallon marking in sink bowls

12) Sinks must have either mounted storage shelves, removable baskets/shelves/bins or stain less steal peg boards; or combination of all the above

13) Must have overhead lighting above the sinks

14) Sink must have a bottom shelf for storage

15) Sink must come with tubing for all make of scopes used (Olympus and Pentax)

16) Sinks must have a hot and cold hook up

17) Sink must have a faucet and some type of spray nozzle

18) Sinks must come fully ready for our plumbing shop to install (this includes all electrical and pipe work) All deliveries are to be performed and accepted at the Department of Veteran Affairs, James A Haley VA Medical Center/associated Clinics, 13000 Bruce B. Downs Blvd., Tampa, FL

The following Provisions applies to this acquisition:

Provision 52.212-1, Instructions to Offerors—Commercial Items. Offerors must submit their technical and management approach that addresses all requirements pertaining to the required services.

Provision 52.212-2, Evaluation -- Commercial Items. Tiered Evaluation for SDVOSB concerns will be used first in accordance with 38 U.S C. 8127 (i). Upon completion of the tiered evaluation the comparative evaluation procedures will be used in accordance with FAR 13.106-2(b)(3).

Tiered Evaluation LIMITED TO SDVOSB The government will conduct tiered evaluations to a Service Disabled Veteran Owned Small Business with the following procedures.

Government will evaluate the quote, if any, submitted by SDVOSB concerns. If no quotes were submitted by SDVOSB concerns, or if none of the quotes would result in award at a fair and reasonable price that offers best value to the United States.

If any offer submitted by SDVOSBs are accepted, the Government will proceed with a comparative evaluation for the basis of award.

Comparative Evaluation The Government will award a Fixed Price contract to the responsible quoter using a comparative evaluation. The comparative evaluation will be performed in accordance with FAR 13.106-2(b)(3). The government reserves the right to select a quotation that provides benefit to the government that exceeds the minimum but is not required to do so. Quoters are advised that quotation may exceed the requirements but the government is not requesting or accepting alternate quotes; each response must respond to the solicitation requirement, stated in the RFQ.

The government will apply the following comparative evaluation process as the basis for award:

Assess the direct comparison of one quote with another using a uniform and fair approach to determine which quote provides the government what it needs, where and when in accordance with the SON and RFQ.

Once one quotation is found acceptable, it is compared to the remaining ones, and the “best” one is chosen for the best value to the government.

The Government may rely on internal documentation including the Federal Awardee Performance and Integrity Information System (FAPIIS) Past Performance Information Retrieval System (PPIRS) and contracting officer’s knowledge of and previous experience with supply or service being acquired for determining Past Performance. If no record of past performance is found in FAPIS or PPIRS, the contract shall not receive a favorable or unfavorable rating, but shall receive a rating of neutral.

Price shall be submitted using the enclosed Price Schedule.

Information not contained in a vendor’s quote will not be considered during the evaluation. The Government intends to make award selection without discussion, but may determine after evaluating submitted quotes that the elements stated in FAR 13.106-2(b) for evaluation procedures are necessary, and conduct them as appropriate. The Government reserves the right to make no award if no quotes meet the requirements of this solicitation.

Provisions 52.212-3 Offeror Representations and Certifications-Commercial Items. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal.

The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable.

Additional contract requirement(s) or terms and conditions necessary for this acquisition and consistent with customary commercial practices are as follows: Attachment A: VAAR: 852.246-73, 852.701-1, 852.273-74. Any questions concerning this solicitation shall be submitted to Frank Lloyd no later than August 28, 2018 via email to frank.lloyd@va.gov. All quotes shall be submitted to Frank Lloyd via email to frank.lloyd@va.gov not later than August 30, 2018, 3:00 p.m. EST.

Gray Market Prevention Language:

(a) Gray market items are Original Equipment Manufacturer’s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable.

(b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM’s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.

(c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA’s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
