This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 13.5, as supplemented with additional information included in this notice. The Department of Veterans Affairs intends to issue a Firm-Fixed Price (FFP) contract. This announcement constitutes the only Solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation 36C24818Q0234 is issued as a Request for Quotation (RFQ) and incorporates the provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-96, effective: November 6, 2017.
This procurement is being solicited as a 100% SDVOSB set-aside under North American Industrial Classification System (NAICS) code 492110 with size standard of 1,500 Employees. This procurement will be awarded using the Simplified Acquisition Procedures (SAP) outlined in FAR Part 13.5. This solicitation sets forth the requirement for providing Blood Courier Services for the VA Medical Centers and facilities/locations listed in the Statement of Work (SOW).
All offerors must be registered in System for Award Management (SAM). Information concerning SAM registration requirements may be viewed via the Internet at https://www.sam.gov.
To receive consideration as SDVOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov at the time of award.
The Department will award a contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to this notice will be the most advantageous and representing the best value to the Department. A firm-fixed-price contract will be awarded to the offeror with the Lowest Price Technically Acceptable (LPTA) quote. The quote will be evaluated as acceptable or not acceptable based on the Offeror’s quote meeting the Evaluation Criteria Factor specifications of this combined synopsis/solicitation. The failure of an Offeror to provide information sufficient to determine conformance with the combined synopsis/solicitation may result in the Offeror being ineligible for award.
The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition.

52.217-9 Option to Extend the Term of the Contract
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 Days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months.

Schedule of Services Contractors shall quote prices for all of the Contract Line Item Numbers, to include a Total price/cost, on the attached Price/Cost Schedule.
Evaluation Criteria:
A firm-fixed price contract will be awarded to the Lowest Priced Technically Acceptable (LPTA) offer. The Technical and Past Performance evaluation will be a determination based on information furnished by the offeror. The Government is not responsible for locating or securing any information which is not identified in the offer.
The following factors, listed in descending order of importance, will be considered in the evaluation of quotes received under this solicitation. To be considered technically acceptable, the quote must include all items as described in Factors 1 & 2, and in the requirements of the solicitation.
FACTOR 1: TECHNICAL & MANAGEMENT APPROACH- The Offeror's quote shall disclose the technical ability in as much detail as possible, including, but not limited to, the requirements in this section of the RFQ. The technical ability should be specific, detailed and complete enough to clearly and fully demonstrate that the offeror thoroughly understands the intent of the Statement of Work (SOW). Stating that, the offeror understands and shall comply with the SOW, or paraphrasing the SOW, or parts thereof, is considered inadequate.
0. Provide information on your company’s ability to perform the services as described in the SOW (to include proposed # of drivers and their location).
0. Provide detailed information how your company will ensure specimens remain within the required temperature ranges, as described in the SOW.
0. Submit a copy of liability insurance.
0. Provide organizational chart, resumes of key personnel, and certification and/or licenses required to perform the services.
0. If a subcontractor is utilized, include with your quote a letter identifying a full understanding of the following clauses, to include percentages for the prime contractor and any anticipated subcontractor(s):

FACTOR 2: TOTAL EVALUATED PRICE - All pricing will be evaluated on a total price basis to include option years of the contract. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
Statement of Work
Contractor shall provide all parts, materials, supervision, services, specialty services, testing, and inspection to successfully perform in accordance with the attached SOW.
Payment CONTRACTOR REMITTANCE ADDRESS: Effective January 1, 1999 all payments by the Government to the contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer-- Other than Central Contractor Registration (31 U.S.C. 3332).
Payments will be made monthly in arrears upon receipt of properly prepared invoices which must show reference to the correct purchase order number issued upon contract award. Contractors must use OB-10 for submission of invoices. Register at the following website, www.ob10.com/veteransaffairs
Clarification Questions:
Submit all clarification questions concerning this solicitation in writing via E-Mail to Todd Pappas, todd.pappas@va.gov, (352)381-5702. Questions must be submitted by 10:00 AM EST, 29 December 2017. After this date, the VA does not guarantee that a response will be given.
Submitting a Quote Contractor shall submit their quote by completing the attached price/cost schedule; including any offered discounts; name, address, and telephone number of the offeror; terms of any express warranty, a completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and the SAM document. Interested contractors must also submit a copy of any relevant documents, certifications and licenses along with their quote, as described in the Evaluation Criteria Factors.
Quotes are due no later than 10:00 AM EST January 8, 2017. Quotes will only be accepted by e-mail to todd.pappas@va.gov. Offers received after the exact time specified in this solicitation are considered late and may be considered if the action would not unduly delay the acquisition.

This Request for Quote is not considered to be a commitment by the Government to award a contract nor will the Government pay for any information provided; no basis for claim against the Government shall arise as a result of a response to this Request for Quote or use of any information provided. Failure to submit information in sufficient detail may result in considering a company as non-responsive and may influence competition and set-aside decisions. Regardless of the information obtained, the Government reserves the right to consider any arrangement as deemed appropriated for this requirement. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted.
