Attachment B: Statement of Work and Salient Characteristics
1. BACKGROUND
The Atlanta VA Health Care System/ Regional Telehealth Services, Technology-based Eye Care Services (TECS) Division requires Four (4) Visual Field Perimetry Machines with printer, wheelchair table, DICOM, and Four (4) middleware license that will assist facility staff with the care of patients in support of the VA Mission. VA Biomed will install Visual Field Perimetry Machines at the Austell Clinic and other Atlanta VA Healthcare Clinics.
1. SCOPE
The vendor/contractor shall provide Four (4) Visual Field Perimetry Machines with printer, wheelchair table, DICOM, and Four (4) middleware license. The contractor shall provide operator manuals and service manuals. The contractor shall also provide a 12- month warranty on all products and 24 months on lasers. Training on the use of the products is also included.
1. VENDOR/CONTRACTOR REQUIREMENTS
The vendor/contractor shall ensure the Visual Field Perimetry Machines, device and connectivity license meets specifications In Accordance With (IAW) the specifics identified in this SOW.
1. DELIVERABLES
Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW.
Visual Field Perimetry Machine:
Task One: Deliver four (4) visual field perimetry machine with printer, wheelchair table, DICOM, and four (4) middleware licenses to the Atlanta Healthcare System for the Austell and Atlanta VA Clinic eye screening stations.
Deliverable One: Verification of four (4) proper, fully functional and installed visual field perimetry machines with printer.
Task Two: Provide full service literature, preferably in electronic format, including as-built drawings and installation data to Atlanta VAMC Biomedical Engineering department.
Deliverable Two: Verification of receipt of full-service literature (Operator and Service Manuals), preferably in electronic format, including as-built drawings and installation data.
Task Three: Provide the appropriate DICOM and four (4) middleware licenses and/or image capture/review software to connect the Visual Field Perimetry Machines to VA Vista Imaging and the Eye Clinic’s middleware, pre-existing image management system.
Deliverable Three: Verification of receipt of DICOM and four (4) middleware licenses and that the visual field perimeter can connect and transfer images to VA Vista Imaging and the Eye Clinic’s middleware, pre-existing, image management system.
1. PERIOD OF PERFORMANCE
June 14, 2019 through September 28, 2019. The contractor shall complete the work required under this SOW in 90 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO).

All equipment:
1. Must be DICOM compliant as per the DICOM Conformance Statement
1. The image must be captured using a DICOM interface for transfer into Vista Imaging.
1. Should be on approved VA list: a listing of tele retinal image acquisition systems that have been validated by the VA is located at below website link on page 29.(ophthalmic photography) http://www.va.gov/HEALTH/imaging/docs/VistA_Imaging_DICOM_Modality_Interfaces.pdf
1. Should be able to connect to Atlanta main eye clinic’s existing software management system “middleware” (Zeiss Forum).
1. Technology for Visual Field Perimetry Machine
0. Perimeter should have capability to connect to main Atlanta VA Eye Clinic’s pre-existing image management software system at a high level such that the image management software can manipulate the raw data derived from the perimeter. Eye Clinic currently owns Zeiss Forum as the image management system/middleware.
0. Visual Field perimetry machine should be a summary measurement of the patient’s visual field status, expressed as a percent of normal age-adjusted visual field.
0. Threshold test library needs to include multiple tests including 60-4, 24-2, 30-2, 10-2, and macula.
0. Machine should have a blind spot monitor, gaze and head tracking.
0. Multiple stimulus colors – White on White, and red or blue on white.
0. Must perform foveal threshold testing and automatic pupil measurement
0. Should produce results that are directly comparable to perimetry machines that already exist at the Atlanta VA Eye Clinic (currently clinic owns 5 Zeiss Humphrey Visual Field Machines).
0. Progression analysis algorithms on the perimeter should utilize evidence-based medicine protocols derived from glaucoma studies such as the Early Manifest Glaucoma Trial.
0. Progression analysis algorithms should be FDA approved to detect progression and not just provide trends.
