Statement of Work
1. GENERAL INFORMATION
1. The Surgical Service Line requires the purchase of eight (8) complete comprehensive eye lanes with refraction desks, equipment and one (1) Haag-Streit Octopus 900 Perimetry room, set-up in support of the Atlanta VA Medical Center’s mission to expand new patient care eye services to a new North Fulton Eye Clinic facility.
1. Scope of Work: The vendor/contractor shall provide the requested equipment to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall deliver and install equipment and furniture to set up eight (8) complete comprehensive eye lanes and one (1) Haag-Streit Octopus 900 room, to the North Fulton Eye Clinic, and meeting all applicable codes and regulations, drawings, and is subject to the terms and condition of the contract.
1. Background: The new North Fulton Eye Clinic requires (8) complete comprehensive eye lanes and one (1) Haag-Streit Octopus 900 room. This equipment is needed to equip the new Comprehensive Eye Clinic and the Technology-based Eye Care Services central station located at the new North Fulton Eye Clinic location, 2309 Parklake Drive, Atlanta, GA 30345.
1. Performance Period: The contractor shall complete the work required under this SOW in 90 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor’s proposal, the contractor’s proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO.

1. Installation
5. All equipment, one (1) Haag-Streit Octopus 900 Perimeter and eight (8) Omega refraction desks, and furniture shall be installed by the vendor. Omega Refractions Desks are custom made specifically for ophthalmology and therefore require additional time to be installed.
1. Maintenance
6. Equipment, one (1) Haag-Streit Octopus 900 Perimeter, eight (8) Omega refraction desks and furniture shall be serviceable and routinely maintainable by vendor per warranty. After warranty has expired vendor will respond when Bio-med is not able to repair.
1. CONTRACT AWARD MEETING
The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived.
1. GENERAL REQUIREMENTS
For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan.
All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms.
Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within _90__ calendar days from the date of receipt. The contractor shall have _90__ calendar days to deliver the final deliverable from date of receipt of the Government’s comments.
1. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES
Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW.
Task One: The contractor will build to exact specifications provided by the Eye Clinic and deliver eight (8) eight Omega style Refraction desks, auxiliary equipment for eight (8) fully equipped comprehensive eye lanes, and (1) Haag-Streit Octopus 900 Perimeter to the North Fulton Eye Clinic. Logistics will need 14-day notice to schedule delivery by a specific date and time.
Deliverable One: Verification of proper, fully functioning equipment and furniture for eight (8) comprehensive eye lanes to the North Fulton Eye Clinic.
Task Two: The contractor will install eight (8) Omega refraction desks including all auxiliary equipment that will complete eight (8) fully equipped comprehensive eye lanes and install one (1) Haag-Streit Octopus 900 Perimeter to the North Fulton Eye Clinic.
Deliverable Two: Verification of proper, fully functional and installed equipment and furniture of eight (8) Omega refractions desks, eight (8) comprehensive eye lanes and one (1) Haag-Streit Octopus 900 Perimeter to the North Fulton Eye Clinic. Contractor will ensure Omega refractions desks are electrically wired to the room for proper room illumination during the patient examination.
Task Three: Provide full service literature, preferably in electronic format, including as-built drawings and installation data to Atlanta VAMC Biomedical Engineering department.
Deliverable Three: Verification of receipt of full-service literature (Operator and Service Manuals), preferably in electronic format, including as-built drawings and installation data.
Deliver to: Atlanta VA Northlake Location Attention: Cheri Boudreaux (Eye Clinic) and Building Lessor 2309 Park lake Drive Atlanta, GA 30345
1. CHANGES TO STATEMENT OF WORK
Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
1. INFORMATION TECHOLOGY (IT)
· The contractor does not require IT access. The contractor shall not be granted access to VA sensitive information.
· The C&A requirements do not apply.
· The Security Accreditation Package is not required.
1. ESCORT REQUIRED
1. The vendor will be escorted by the COR/Ophthalmology Clinic Manager (Cheri Boudreaux) and the Lessor of the leased space during installation of furniture and equipment.

*Contractors providing equal products shall provide descriptive literature describing equal characteristics and must be compatible with current system.
Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable.
Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.
The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA selection, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
