PERFORMANCE BASED WORK STATEMENT

Inspection and Testing of Fire Protection Equipment

1. GENERAL:

The Contractor shall provide all plant, labor, equipment, materials, supervision, repair/replacement parts, and transportation, necessary to perform annual fire extinguisher inspections, provide annual inspection of Fire Extinguishers to include testing of the En-gauge electronic monitoring system to insure functionality and replacement of batteries for wireless devices, semi-annual kitchen hood inspection (appropriate replacement of fusible links and extinguishing canister materials), and fire extinguisher six and twelve year maintenance testing and certification at the Veterans Health Administration, Ralph H. Johnson, Medical Center in Charleston, South Carolina and designated offsite locations.

2. BACKGROUND:

The RHJ VAMC - 133-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located at 109 Bee Street, Charleston, South Carolina, 29401 and provides primary, secondary, and some tertiary care.

4. PLACE OF PERFORMANCE:

All buildings on RHJ VAMC campus 109 Bee Street, Charleston SC, Trident MH, 9202 University Blvd., North Charleston, SC 29406:, Trident OPC, 9237 University Blvd., North Charleston, SC 29406, Myrtle Beach Clinic, 3381 Phillips Blvd., Myrtle Beach, SC, C&P Clinic, Joint Base Charleston-Weapons Station, Building 754 Pulaski St., Goose Creek, SC 29445, Joint Base Charleston-Weapons Station, Bldg. #746 Pulaski St., Goose Creek, SC 29445, Fisher House, 150 Wentworth St., Charleston, SC 29401.

5. SCOPE:

a. The Contractor shall provide all labor, equipment, materials, supervision, repair/replacement parts, and transportation necessary to perform monthly and annual fire extinguisher inspections for up to 308 extinguishers (Class A, B, C & K and Non Ferrous extinguishers for Radiology Departments), at locations listed in paragraph 4.

b. The Contractor shall provide all labor and materials, supervision, repair/replacement parts, and transportation necessary to perform up to 308 six and twelve year maintenance testing and inspections of both 2.5, 5lb, 10 and 20lb fire extinguishers per the attached rate schedule below total quantity to be determined by the VA at the time of the inspection. Hydrostatic testing of approximately 40 fire extinguishers each year, and replacement of approximately 40 fire extinguishers each year. Repair/replace up to 50 en-Gauge fire extinguisher monitoring devices each year.

c. The Contractor shall provide all labor, equipment, materials, supervision, repair/replacement parts, and transportation necessary to perform semi-annual/annual inspection automatic extinguishing systems testing (hoods/tanks), inspection and certification of the kitchen hood suppression systems and replace fusible links, located within RHJ VAMC (main hospital): Dietetics Kitchen (1), Canteen Kitchen (2), CLC (nursing home)(1).

d. (NOTE: most ABC extinguishers were purchased June 2013 therefore will be due hydrostatic testing at the same time:

e. The Contractor shall test and certify all associated with systems and provide documentation of inspections in accordance with current applicable NFPA (10, 17, 25, 72, 96 and 101), VA, and OSHA requirements.

f. The Contractor shall provide a detailed report documenting test results and findings by location.

g. The contractor shall make minor repairs fire extinguishers and equipment as necessary to insure compliance with NFPA (10, 17, 25, 72, 96 and 101), VA and OSHA requirements.

h. All parts provided by the contractor shall be new and compatible with the original equipment manufacturer for the specific equipment, as required.

i. The contractor shall contact Michael Tucker, Occupational Health and Safety Specialist, to schedule inspection, testing, and certification work.

j. During the progress of the work and upon completion, all work shall be subject to inspection by the Contracting Officer and the COTR or the designated representatives.

k. The contractor shall provide emergency repair and service as necessary with 24 hours of contact.

l. The contractor shall be located within a 120 mile radius of the Charleston VA Medical Center located at 109 Bee Street, Charleston SC.

m. The contractor shall be responsible for providing a human point of contact for emergency calls 24 hours per day.

n. The contractor shall be responsible to perform the semi-annual hood inspections and automatic extinguishing system inspections during the months of October and April respectively (unless written changes are agreed upon in advance). These inspections shall be performed all in the same day if possible and shall be scheduled to prevent interfering with serving patients food. The contractor shall provide written notice two full weeks in advance to the COTR to allow for scheduling with all parties involved.

o. No work is to be performed on the site on Saturdays, Sundays or legal holidays unless otherwise specified and the COTR is duly notified in writing no less than forty-eight hours in advance of the contemplated work and written approval for such is obtained. The following are National Legal Holidays:

6. CONTRACT PERFORMANCE MONITORING:

The government reserves the right to monitor services in accordance with Performance Based Matrix.

7. INVOICES:

Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer’s Technical Representative (COTR), and submitted by appropriate method (online billing) to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714.

A properly prepared invoice will contain:

· Invoice Number and Date

· Contractor’s Name and Address

· Accurate Purchase Order Number

· Supply or Service provided

· Total Amount Due
