Statement of Work Offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.
*All Interested Contractors: One site visit is scheduled for March 15th, 2018 at 12PM EST.* Location: Ralph H. Johnson VAMC 109 Bee Street Charleston, SC 29401

Scope: The current dish washing machine is used intermittently 13.5 hours daily in the service of 3 meals per day. The contractor(s) shall furnish and install as itemized below per specification listed.
Schedule of Items: The contractor will be expected to install quoted piece of equipment including accessory parts as outlined below:
1. One (1) Electric Dish Washing Machine
· Hobart Model No. CLPS86EN-ADV+BUILDUP
· 1 ea CLPS86EN‐ADVELE0CD 480v/60/3‐ph electric
· 1 ea CLPS86EN‐ADVHTE15K Electric tank heat 15kW wash/10kW rinse

· 1 ea BDELRCD‐STDDOM Standard height, domestic, Blower Dryer Electric, 480/60/3 L‐R, includes (1) 4” X 16” vent stack
· 1 ea CLPS86EN‐ADVHGTSTD Standard height
· 1 ea CLPS86EN‐ADVFETSTD Standard feet
· 1 ea EXTHD/E‐DOM E‐series extended hood dom
· 1 ea CLE/TBL‐SWITCH Table LMT switch CLE‐Series
2. One (1) Clean Dish Table
- Nichols Custom Stainless Model No. CLEAN77
- Clean dish table measures 7'‐7‐1/2" long x 30" wide x 34" high
3. One (1) Soiled Dish Table
- Nichols Custom Stainless Model No. TSOIL
- T shaped soil dish table measures 10'‐0" long x 3’‐3‐1/4 long x varies 28‐ 36" wide x 34" high.

5. One (1) Pre-rinse Faucet Assembly
- T & S Brass model No. B-0169
- Spray unit, single control valve with vacuum breaker
- 5 ‘ flexible hose, stainless steel
- Angled spray valve with automatic shut off
- ½” IPS female water inlet
Installation:
-Pre-installation site survey to determine space, electric, drain and water requirements are met -Installation of dish washing machine, clean dish table, soiled dish table/trough, waste collector, and spray arm
- Installation of trough to soiled dish table, to include optional gusher assembly, 2” tank drain and recirculation plumbing.
-Installation of spray arm faucet to soiled dish table
Training:
- In-service training provided to include use, features, cleaning and maintenance of equipment -One operations manual -Point of Contact for Warranty Service
Deliverables/Deliverable Schedule: The contractor shall remove all existing equipment and deliver to RHJ VAMC loading dock. Contractor will uncrate, set in place and install all new equipment to include all existing utility connections. After contacting NFS requisition managers at the VA, a specific time will be scheduled for the delivery of the product and the installation. Product will be installed during normal working hours, between 7:00am and 3:30pm, Monday through Friday. Contractor will be responsible for removing all cardboard, refuse, debris, etc. from site daily during installation period. Documentation necessary includes a statement acknowledgement/invoice and receipt of delivery.
Contractor Responsibilities:
1. Contractor(s) shall provide all labor, equipment and materials necessary to complete installation of equipment and accomplish work as stated.
2. Disconnect existing equipment to include electrical, plumbing, and/or venting, and move to facility loading dock.
3. Receive all equipment and deliver to the facility.
4. Uncrate and set into place.
5. Provide all equipment labor and materials to reconnect existing, proper sized, electrical service to new equipment as required to meet new equipment connection points.
6. Reconnect water supply lines as required to meet new equipment connection points.
7. Installation of trough, gush assembly, spray arm, waste collector, tank drain and recirculation plumbing to soiled dish table/dish washing machine.
8. Perform startup and test, and adjust machine as needed for proper operation.
9. Contractor will dispose of any debris at onsite dumpster.
10. No additional work shall be required on the part of the government.

Period of Performance: Contractor will have 5 working days to complete installation of the unit after the delivery date and remove all debris and leftover materials from site.
