1. Contracting Activity: Network Contracting Office NCO 06, 100 Emancipation Dr, Hampton, Virginia 23667, for the Salisbury VAMC 1601 Brenner Avenue, Salisbury, NC

28144. PR NUMBER: 659-24-2-048-0121

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Salisbury's VAMC is requesting Philips IntelliVue MP5SC Pre-OP Monitors. These devices provide bedside monitoring of patient’s vital signs with battery backup.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

This is a brand name justification. The Philips IntelliVue MX550 equipment being procured will be an extension of a equipment already in use at the facility. The medical center is expanding and renovating the operating room. The medical center has standardized to this equipment throughout the facility for safety, competencies, maintenance, biomed training etc. The monitors being procured are compatible with the Philips receivers currently used throughout the hospital. If another brand of monitor were to be procured, it would not be compatible with the Philips receivers used throughout the facility.

4. Description of market research conducted and results or statement why it was not conducted:

Searches of SBA Vet Cert, GSA, NAC, and SAC were performed. The Original Equipment Manufacturer OEM (Philips Healthcare) is a Large Business. One SDVOSB and the OEM were identified that can provide the required equipment.

5. Contracting Officer's Certification: Purchase is approved in accordance with

FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
