Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 36C246-24-AP-2089
1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office (NCO) 6, located at 100 Emancipation Drive, Hampton, VA 23667, in support of Richmond VA Medical Center at 1201 Broad Rock Blvd, Richmond, VA 23524. Purchase request number 652-24-2-165-0073.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: NCO 6 intends to award a new brand name only purchase order for Mizuho OSl's Trio Table to be used for spinal procedures, orthopedic trauma procedures, and radiolucent imaging. These table will replace the existing Mizuho table. The required items are: Estimated amount $184,076.71.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Mizuho OSI surgical tables are the only tables that will meet both the Neurosurgery and Orthopedic Specialty needs at the Richmond VAMC. These tables are unique and are the only truly modular table available that allows the interchanging of three different tops on one platform, or base. For example, the table can be used to do complex anterior posterior surgical cases for spine; it allows the surgeon to work on the patient’s anterior spine on a flat table allowing full spine radiolucent imaging, then, while the patient is still under anesthesia, a posterior spine frame is placed on top of the patient, the patient is turned 180 degrees until they are in the prone position allowing the surgeon access to the posterior portion of the spine. This table will convert from a spine frame to a flat top that can be used for both anterior spine cases and orthopedic fracture procedures, and to an ortho trauma fracture frame. There are no other modular table systems on the market that function like the Trios table, which cuts cost and saves space by allowing one table to do the function of what is typically three separate tables.
4. Description of market research conducted and results or statement why it was not conducted:
Searched VetCert, FPDS, NAC, GSA, and WWW. The VA Rule of Two cannot be met. There are at least two small business that can satisfy this requirement.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
