590-3630152-413-841100-2580 0100401P4 X The Purpose of this Amendment is to submit Technical Questions and Answers, Update the Pricing Worksheet to reflect a change in Pickup and Delivery to the CBOCs and As Needed locations, and Extend the Response Due Date.
1. Technical Questions and Answers are hereby submitted.
2. Pickup and Delivery from the Hampton VAMC to the local CBOCS is hereby changed from daily to three, 3, times a week.
3. Pickup and Delivery from the Hampton VAMC to the Elizabeth City CBOC remains once per week.
4. Pickup and Delivery from the Hampton VAMC to the As Needed locations is up to three, 3, times a week.
5. The Response date to submit your quotes is hereby extended to Wednesday, August 2, 2023 at 15:30.
6. Q&A begins on Page 2.

1. I would like to know if this is a daily route, or an on-call situation?
a. Three, 3, times per week to and from the Hampton VA Medical center to local CBOCS
b. Weekly (Once per week) to and from the Hampton VA Medical Center to the Elizabeth City CBOC in NC
c. All other locations are as needed up to three, 3, times per week from the Hampton VA Medical center to the Richmond VA Medical Center and affiliated medical centers (Must be flexible)
2. Also, if the packages will require multiple vehicles or just one?
a. Just one sedan sized vehicle
3. What will be the frequency at which the packages will need to be transported and how large the packages will be?
a. Three, 3, times per week to the local CBOCS in Chesapeake, VA Beach, and Portsmouth
b. Once a week to the Elizabeth City CBOC in NC
c. Small packages no larger than 50 lbs.
4. Will the packages need to be transported in something larger than a van?
a. No
5. On the worksheet, is each line item the cost x mileage for 1 trip?
a. To simplify, the formula is removed.
b. Submit your price based on the required pickup/delivery for each location.

a. Round trip

24. Is billing to the VA determined by the daily/weekly rate provided in the Updated Hampton Pricing Worksheet by the bidder/awardee?
a. Quotes will be submitted as a total lot, LT, amount on B.2 Price/Cost Schedule on the RFQ
b. Complete and submit breakdown of costs provided in the pricing worksheet with RFQ
c. Invoices will be billed/submitted monthly, in arrears.
