Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Need 2 brand name only Mar Cor Millenium HX Dialysis which is portable water unit for dialysis. The ICU will be doubling the capacity to 10 beds increasing the demand for acute dialysis treatment. The new state-of-the-art ICU has been relocated to 4th floor making it essential to physically keep one dialysis unit in the ICU.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Acute dialysis services is fully staffed and managed by SBY. The acute service provides dialysis support to patients admitted to the ICU and to the in-patient service in Bldg. 2. All ICU rooms are now equipped with proper hook-ups for water supply and drainage needs to provide bedside dialysis treatment to critically ill patients. Frequent transportation of units from 3rd floor to 4th floor can lead to breakdowns and increased wear and tear. The HD and RO units are not built to withstand the bumps and jerks from recurrent transport through elevators. Currently have 3 HD machines and 3 portable RO supporting the services. There are frequent breakdowns of these machines and often have come very close to shutting down services due to equipment failure.

The additional breakdowns due to frequent transportation of equipment can significantly limit capabilities to continue providing timely care.

4. Description of market research conducted and results or statement why it was not conducted:

Emailed Bruce Farrow on 007/20/2021 and he stated that MAR COR is the sole manufacturer and distributor.

No vendors were contacted due to the manufacturer being the sole distributor for this brand name only item.

National Strategic Source Listing search with keyword “dialysis” revealed 1 vendor that can provide the name brand item, but the contract expired 12/30/2020.

National Acquisition Center search with keyword “dialysis” revealed several vendors but none provide this specific brand.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
