DESCRIPTION

The Department of Veterans Affairs, Network Contracting Office (NCO) 6 has a requirement for Walk Off/Step Off Mats Services. Services include providing and servicing mats. The Contractor will also be required to swap out and clean mats on a weekly basis. Services will be accomplished on Thursdays, 52 times per year.

The Department of Veterans Affairs, Network Contracting Office (NCO 6) requires establishing a new multiple year (Base and four Option years) contract for the Walk-Off/Step-Off Mat Service The Contractor shall provide all transportation, material hardware and parts, tools, specialized equipment, labor, supervision and technical expertise to complete the services. The solicitation shall be conducted in accordance with FAR Part 12 and FAR Part 13

The Contractor will provide all transportation, tools, specialized equipment, personnel protective equipment, supervision and technical expertise to provide and deliver customized step-off mats to various locations on the main hospital campus.

The Contractor will also be required to swap out and clean mats on a weekly basis.

Place of Performance Hampton VA Medical Center (VAMC) located at 100 Emancipation Drive, Hampton, VA. This requirement does not include Hampton VAMC’s functions/activities located off the main campus.

Period of Performance 1 October 2021 to 30 September 2022 for the Base Year. Total Period of Performance, which includes Option Years, will be 1 October 2021 to 30 September 2026 in strict accordance with all schedules, specifications, terms, and conditions and provisions of the contract solicitation.

Type of Set-Aside: This acquisition is a 100 % Service-Disabled Veteran Owned Small Business (SDVOSB) set-aside. The applicable NAICS is NAICS code is 561740 (Carpet and Upholstery Cleaning Services) and the size standard is $6 Million.

Contract Type: The Government intends to award a Firm Fixed Price Contract in accordance with all terms, conditions, provisions, and specifications of the solicitation.

All potential offerors are responsible for monitoring and downloading all documents including amendments at the Beta/SAM website without further notification from the Contracting Officer. To be eligible for award, contractors must be registered in the following:

3.1.Carpets are to be of commercial heavy-duty grade as per industry standards and must meet or exceed requirements outlined in ANSI/NFSI B101.6-2012. B.3 PERFORMANCE WORK STATEMENT

Annual Service Contract

Walk-Off/Step-Off Mat

Environment Management Service (EMS)

l. General Requirements. Environmental Management Service (EMS) requires establishing a new multiple year (Base and four Option years) annual Walk-Off/Step-Off Mat Service contract for the Hampton VA Medical Center (VAMC) located at 100 Emancipation Drive, Hampton, VA. This requirement does not include VAMC functions/activities located off the main campus.

1.1. Upon award of contract, the Contractor will provide all transportation, tools, specialized equipment, personnel protective equipment, supervision and technical expertise to provide and deliver customized step-off mats to various locations on the main hospital campus.

1.2. The Contractor will also be required to swap out and clean mats on a weekly basis. Services will be accomplished on Thursdays, 52 times per year.

1.3. Period of Performance:

Base Year 1 October 2021 to 30 September 2022 Option Year 1 1 October 2022 to 30 September 2023 Option Year 2 1 October 2023 to 30 September 2024 Option Year 3 1 October 2024 to 30 September 2025 Option Year 4 1 October 2025 to 30 September 2026

Total Period of Performance, which includes Option Years, will be 1 October 2021 to 30 September 2026.

1.4. This service will be Turn-key with the Contractor providing all goods and services.

1.5. This contract will be firm-fixed for the Base and all four (4) Option years. The Contractor will invoice monthly for services.

2. Coordination. A Contracting Officer's Representative (COR) from EMS will serve as the focal point and point-of-contact (POC) for this contract.

2.1. Contractors will conduct a site visit prior to submitting their proposal. Site visits will be formally pre-scheduled and pre-coordinated through the Contracting Officer (CO). Unannounced site visits will not be honored. The Federal Government will not incur any costs associated with site visits.

2.2. Work on this contract will be accomplished on Thursdays of each week. All work will be performed during normal duty hours 07:00AM - 3:00PM, excluding Federal Holidays. Work outside these hours must be pre-scheduled and approved by the COR. If the regularly scheduled exchange service day falls on a Federal Holiday, the Contractor can perform the service either the day prior to, or the day after the holiday, and will inform the COR.

NOTE: The Federal Holidays are New Year's Day, Martin Luther King, Jr. Day, President's Day, Memorial Day, Juneteenth, Fourth of July, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day.

2.3. The Contractor will check-in with the COR prior to accomplishing any services. The Contractor will check-out in the same manner.

2.4. Contractor personnel will also check-in and pick-up visitor badges from Police Services at Building 36. Badges will be returned daily by 3:30 PM.

2.5 If necessary, station maps may be provided to assist Contractor personnel.

2.6. The Contractor will provide the CO and COR a key personnel contact telephone listing.

3.2. Some mats will be custom made with graphic logos. The logo will be provided by COR.

3.3. The Contractor will deliver clean, dry and serviceable mats weekly. After the initial start-up, replacement mats will be provided to the Federal Government at no charge.

4. Specific Requirements. The Contractor is responsible for providing all transportation, tools, materials, labor, personnel protective equipment, supervision and technical expertise to implement all aspects of a full-service Walk-Off/Step-Off Mat contract. Services will be accomplished on Thursdays, 52 times per year. Specifically, the Contractor will accomplish the following:

4.1. Manufacture and/or provide mats as indicated above in paragraph 3. New mats will be inspected and accepted prior to being placed in specified locations.

4.2. Prior to the first delivery, orient personnel with the Hampton VAMC grounds and mat delivery locations.

4.3. On delivery Thursday, check-in with the EMS COR and accomplish badging through Police Services.

4.4. Accomplish mat swap out at all locations indicated above. Unless approved by the COR, there will be no deviations to the provided list.

Note: Incoming mats will be delivered clean, completely dry and serviceable.

4.5. Upon service completion, check-out with, and have the EMS COR inspect and accept all services.

4.6. Check-out and return badges to Police Services.

4.7. Clean and service dirty mats.

4.8. Accomplish invoicing monthly in accordance with the instructions provided in the formal contract.

5. Government Provided Equipment, Services or Office Space. With the exception of station maps, the Federal Government will not provide any equipment, services or office space during the execution of this contract.

6. Other Station Requirements. The following station requirements will be adhered to.

6.1. Contractor is responsible for maintaining liability insurance for the duration of the contract and for following all pertinent VA, Federal and State regulations.

6.2. Contractor personnel must maintain a valid Driver's License and all ancillary permits required to legally operate company vehicles.

6.3. While on VA Medical Center property, Contractors will wear a company uniform and a visible identification badge which contains a photograph of the employee. This company badge is in addition to the badging accomplished through Police Service.

6.4. Contractor motor vehicles will be clearly identified with the company's name.

6.5. Contractor personnel will not operate cellular phones while performing services within VA buildings.

6.6. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds. Failure to follow these regulations can result in removal of the worker or Contractor from the facility and/or a U.S. Magistrate' s ticket and fine.

6.7. Parking will be in designated lots. Consult with the COR as to allowable parking and material drop-off.

6.8. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect Federal facilities. The Contractor will provide a list of employees expected to work on this contract. This list will be on company letterhead and contain the employees' name, driver’s license number, address and nationality. This list will be verified and signed by a corporate officer. These personnel are subject to a background check and fingerprinting to be eligible to receive a security badge.

6.9. The U.S. Department of Veterans Affairs and the U.S. Department of Health and Human Services requires that all Contractor personnel coming on this facility to complete the Veterans Health Administration’s Privacy Policy Training prior to coming on the facility. The Contractor will submit the certificate, to be completed, at the end of the Microsoft PowerPoint Presentation for each person assigned to this contract who comes on this facility to work. A company roster certifying course completion may be substituted for individual certificates.

B.4 SPECIAL CONTRACT REQUIREMENTS

1. NO SMOKING

Per VHA Directive 1085 Dated 3/15/19 All VA Health Care Facilities shall be smoke free. This directive applies to all patients, visitors, contractors, volunteers, and vendors.

2. BACKGROUND INVESTIGATIONS

A full background check is not required for this position; however, all contract employees must bring photo identification and advance notice from the requiring service to obtain a VA Badge in order to work on a VA facility. Upon award, the CO shall provide the contractor with detailed instructions on fulfilling security requirements.

3. BILLING AND PAYMENT

Contractor shall submit invoice, monthly in arrears, via payment request in electronic form via VA’s Electronic Invoice Presentment and Payment system http://www.fsc.va.gov/fsc/einvoice.asp. The following information must be included on all submitted invoices. Invoices submitted without the following information may be rejected for payment. Invoices shall include at a minimum:

• Date of invoice

• Invoice number

• Contract number

• Obligation number

• Vendor name

• CLIN and dates of service

4. CONTRACT ADMINISTRATION

Notwithstanding the Contractor's responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor.

The Contracting Officer is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that shall affect price, quantity or quality of performance of this contract.

The COR shall be responsible for the overall technical administration of this contract.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 Days.

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 Days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. (End of Clause) C.6 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING—MONITORING AND COMPLIANCE (JUL 2018)

(a) This solicitation includes FAR 52.219-4 Notice of Price Evaluation Preference for HubZone Small Business Concerns.

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

(c) All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.

(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

D.2 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1.1. Contract Description: The Contractor will provide all transportation, tools, specialized equipment, personnel protective equipment, supervision and technical expertise to provide and deliver customized step-off mats to various locations on the main hospital campus.

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

· What will be monitored

· How monitoring will take place.

· Who will conduct the monitoring

· How monitoring efforts and results will be documented.

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. The primary interest is in the final product/service that the Contractor is providing and not in the details of how the contractor is accomplishing the task. The COR may evaluate work at any time during the Contractor’s work performance. This plan documents the methods to be used for measuring each contract requirement, the scheduling of inspections, and the results of the surveillance program.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.

2. GOVERNMENT ROLES AND RESPONSIBILITIES

The following personnel shall oversee and coordinate surveillance activities.

a. Contracting Officer (CO) - The CO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The CO shall also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The CO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.
