1. Is it mandatory that the Assurance Service Program for the top 100 replacements all be Aesculap brand?
No.
2. On page 4 of the Price/Cost Schedule, Quantity is 6,4,4 for the 3 facilities, which would equate to 1 visit per Quarter at Fayetteville HCC and Wilmington HCC, and 1 day every other month at Fayetteville VAMC, but then on page 7, under "Scope" you request 2 times per week which would equate to 8 visits per month etc. Can you please clarify how many times a month services are needed?
The Quantity will remain 6 visits for Fayetteville VAMC, 4 visits for Fayetteville HCC and 4 visits for Wilmington HCC. The Performance Work Statement will be adjusted to reflect the Price/Cost Schedule requirements.
3. What is the previous spend in annualized dollars?
The current contract number for these services is V797P-4121B VA246-17-F-4426
4. How many Instrument Trays are there and what are the conditions?
There are approximately 250 instrument trays for all three facilities, and they are in good conditions.
5. How many Lap Trays are there and what are the conditions?
There are approximately 9 Lap trays for all three facilities, and they are in good conditions.
6. What is the overall quality/condition of the instrument tape, dip coating, etc.?
The conditions of this equipment are in overall good condition.
7. What are the expected days of service for the three facilities? It states two days per week, how is that determined or is that solely for the VA Hospital?
This has been updated under the scope section of the PWS. The work is expected to fall in accordance with the price schedule and will be scheduled with the contact at the Fayetteville VA Medical Center.
8. Is there a vendor currently in place to service this contract and are you able to share that information with me?
Please see the answer to question number 3.
9. We are not an instrument manufacturer yet, part of the requirement is to have instrumentation onboard to replace non-repairable (100 instrument patterns listed), and in the event of instruments having to be sent to our repair center, be covered at no additional cost. Is it acceptable to price our standard repairs accordingly to cover those costs since replacement and sending into our repair center incurs additional expenses, i.e., shipping, extended repair times, new parts, etc?
Everything must be completed on-site.
10. Is there an ISO 9000 requirement for this contract?
No.
Performance Work Statement Changes Original: Scope
The contractor shall provide all labor, materials and equipment necessary for maintenance and repair of several types of surgical instruments that are contained in various instrument sets and the refurbishment of containers/lids. This shall include all the surgical instruments currently in use at the Fayetteville VAMC and HCC. Additionally, the contractor shall provide replacement up to one-hundred patterns due to unserviceable instruments.
All surgical instruments and sets shall also be inspected and evaluated for continued best practice performance and restored to like new condition. The repair services are required twice a week and on as needed basis to ensure that we have the safest and most reliable instruments on hand to use on our Veteran patients.
· Each surgical set shall be inspected, repaired and refurbished at least twice annually.
· Ability to work harmoniously with SPS automated instrument tracking systems.
