STATEMENT OF WORK

Commercial Weight-Based Inventory Management System (July 16, 2018)

1. Background

Veterans Health Administration (VHA) directive 1761(1), dated October 24, 2016, provides policy, mandatory procedures, and operational requirements for implementing an effective Supply Chain Management (SCM) program at Department of Veterans Affairs (VA) medical facilities, Community Based Outpatient Clinics (CBOCs)/Outpatient Clinics (OPCs), and Consolidated Mail Outpatient Pharmacies (CMOPs).

The VHA Office of Business Oversight (OBO) performs ongoing Logistics Business Reviews (LBR) of VHA Medical Center Inventory Management programs. The report number 14-02-LOG-NR-001, OBO LBR Summary, dated January 07, 2015, recommended that SCM staff “improve the accuracy rate of inventory on hand and the retention of physical inventory documentation.”

The VA medical facility Director is responsible for ensuring that proper resources (i.e., space, staffing, and technology) are allocated to the facility SCM program to meet the requirements of these directives and working with the Facility Chief Supply Chain Officer (CSCO) to implement SCM initiatives that seek to optimize customer service outcomes.

The CSCO is responsible for “promoting efficient utilization of supplies by ensuring that proper items and levels are set within inventory points.”

The Generic Inventory Package (GIP) is the current software being utilized for inventory management of Expendable Supplies. The GIP was created in 1977. It is used to manage the receipt, distribution, and maintenance of stock items received for the supply warehouse from outside vendors and distributed to primary inventory points. The system also manages receipt and distribution of items from 11 primary inventory points to 130 secondary inventory/distribution points. SCM staff are tasked with ensuring that the physical inventory count of stocked supplies matches the inventory system on hand value, eliminating excess inventory, and purchasing supplies in appropriate quantities and at appropriate intervals to avoid shortages. Considerable time and effort is required to monitor the distribution of expendable supplies in secondary inventory/distribution points. Current processes require a Supply Technician to physically report to secondary inventory/distribution points, count/scan expendable supplies and enter a distribution order to replenish stocks. It is not currently possible to inventory each item in every secondary inventory/distribution point with any frequency. Staffing levels, space constraints and rate of use determine frequency of “visits” to secondary inventory/distribution points. Human error is the leading cause of physical to electronic inventory mismatches, inaccurate data, and stock outs. Stock outs can and have caused medical procedures to be delayed or canceled.

PAR Excellence’s weight-based inventory system will allow SCM staff to drastically reduce or eliminate stock outs, inventory mismatches and data errors. PAR technology provides real time data on what clinicians are using, how much SCM staff need to restock or reorder, and how much SCM staff should keep on hand to minimize costs, overstock, shortage and expiration. PAR Excellence offers a capitalizable platform with an expected life of 20+ years which will have an immediate, positive impact on supply chain labor. It removes non-value-added activity allowing staff to focus their time and effort on the support of the clinicians’ needs. The Return on Investment (ROI) on PAR Bin has proven to be predictable and consistent with each location where it is deployed paying for itself in 23-27 months.

The following DVA/VHA facilities have deployed/are currently using PAR Excellence:

-Veterans Health Care System of the Ozarks – Fayetteville, AK – (VHAFAV)
-VA Portland Health Care System – Portland, OR – (VHAPOR)
-Mann-Grandstaff VA Medical Center – Spokane, WA – (VHASPO)
-VA Puget Sound Health Care System – Seattle, WA – (VHAPUG)
-Boise VA Medical Center – Boise, ID – (VHABOI)

2. Scope The contractor shall provide all labor, software licenses, interface development, PAR Bin products (scales, controllers, set up kits), suture solution products (scales, kits), storage options (storage bins, louvers, shelving, bridges), 3rd party Information Technology (IT) support and 3rd party general site preparation support required for installation and deployment of an inventory control system in all secondary distribution points and the MEDICAL/SURGICAL primary stock point. The contractor shall provide turn key installation and comprehensive training to SCM staff. The contractor shall, to the extent possible, use existing PAR Excellence equipment previously installed in secondary distribution points, provided equipment is serviceable/functioning, and decrement the value/cost of that equipment from the PAR Excellence Systems quote. This inventory control system shall be a commercial off-the-shelf combination of software and storage systems with demonstrated capabilities in a healthcare medical supply environment to perform the data capture, tracking and search capabilities needed. The system shall be a stand-alone, computerized, weight-based inventory tracking system utilizing scales and software to record supply issues (removal) and supply receipts (additions) to the inventory point. The system shall have weight-sensors, which automatically identify inventory decrements by user or staff removal of items and when warranted, create critical alerts. The system shall be capable of continually communicating system status and notify staff personnel of specific system failures. The system shall be capable of updating and reporting open orders, emergency/critical item levels and negative quantities on hand. The system shall be capable of reporting average month usage, consumption analysis as well as consumption deviation. The system shall be able to produce enterprise reports providing quantity on hand, value on hand, transactions and item history reports. The system shall be capable of remote support via site-to-site Virtual Private Network (VPN) access to the application server. The system shall be accessible by multiple users simultaneously. The system shall operate as a stand-alone system not reliant on any VA, VHA or Durham VA Medical Center (DVAMC) assets other than Internet connectivity and space. The PAR Bin system will be interfaced with GIP. The primary purpose of the interface is to identify out of balance on hand quantities and values between GIP and the PAR Bin system. PAR Bin will identify any out of balance on hand quantities and values, accounting for any due outs and due-ins. If the out of balance condition is caused by a pending due-in or due out it will be displayed separately to easily identify. PAR Bin due-ins and due-outs will automatically be removed when the GIP due-in and due-out are "cleared". The system shall be capable of producing reports in Excel or text delimited format using commercially available software. Contractor employees shall be fully trained, qualified and licensed (if applicable) to install this system. The client license shall operate on existing VA PCs and support all inpatient and outpatient operations.

3. Specific Tasks

3.1. Delivery of Equipment. The contractor shall furnish all primary inventory point and secondary distribution point equipment, supplies and software detailed in Para. 2. necessary to achieve a functional system. The contractor shall coordinate with DVAMC SCM staff to ensure adequate warehouse storage space is available before equipment and/or supplies are shipped. The contractor shall maintain adequate quantities of parts including unique and/or high mortality replacement parts in such a manner as to be available for shipment within 24 consecutive hours of request. All parts supplied shall be compatible with existing equipment. Contract parts coverage shall include all system options, third party pass thru devices, such as compact disk devices, remote workstations, etc. and will include all manufacturer supplied software including that used by third party devices. The contractor shall use Original Equipment Manufacturer (OEM) new or rebuilt parts. All parts shall perform identically to the original equipment specifications.

3.2. Test Equipment. Prior to commencement of work on this contract, the contractor shall provide the DVAMC with a copy of the current calibration certification of all test equipment to be used by the contractor on DVAMC equipment. This certification shall also be provided on a periodic basis when requested by the DVAMC. Test equipment calibration shall be traceable to a national standard.

3.3. Installation. The contractor shall be responsible for the installation of the system on-site in each primary inventory point and secondary distribution point in Building 1 as detailed in attached technical drawings to include 3rd party Information Technology (IT) support and 3rd party general site preparation support. Consideration shall be given to current privacy, safety and security measures in place at the DVAMC. The contractor shall provide a one year warranty on all parts and labor. The warranty shall include all travel and shipping costs associated with any warranty repair. Warranty period shall begin from date of owner acceptance.

3.4. Equipment Modifications/Updates. Equipment modifications are divided into two kinds; 1) Normal functional improvements that assure that the equipment is operating at the peak of performance and 2) Enhancing improvements - optional improvements that change the equipment’s original design to allow for supplementary or additional functions. Normal functional improvements shall be provided at no cost to VA within six (6) months after each version is released. Installation shall be done at no additional cost to the government during normal working hours, unless requested by the Acquisition Program Specialist (APS). Enhancing improvements are optional to VA. If the APS has determined that VA requires such enhancing improvements, it shall be considered outside of the scope of this contract and processed as a separate procurement. All modifications, upgrades, updates, enhancements, etc., must be scheduled in advance with the APS. The Contracting Officer (CO) and APS must receive notice of at least five (5) full business days in advance before any equipment updates, upgrades, enhancements, and modifications are begun along with justification (if vendor-suggested modification), estimated costs (if applicable), probable effect on equipment operation, and needs for new or additional in-service training.

3.5. Operator Training. Education/training shall be provided to employees after installation is complete. A training plan shall be submitted by the contractor no later than ten (10) days after date of award. Training shall include, but not be limited to:

4. Performance Monitoring

An APS will monitor contractor performance daily via routine inspection of their work to assure it is completed in accordance with the awarded contract. An APS will be available to the contractor during their period of installation and will assist with their needs, providing a liaison between the contractor and CO.

5. Security Requirements

5.1. Contractor personnel performing installation may be required to complete a Department of Veterans Affairs security screening. All contractors shall not enter the work area without the appropriate badge. Badges shall be worn at all times while on DVAMC property. Contractors must present themselves to the DVAMC PIV office to obtain a contractor’s badge. DVAMC SCM staff will facilitate/provide a sponsor. Contractors may be subject to inspection of their personal effects when entering or leaving the work area.

5.2. The DVAMC will provide the vendor/contractor with expendable medical supply information required to populate the inventory system and computer files for intended use. With respect to data analysis, reporting, and programming services, DVAMC is responsible for their selection to achieve the intended results. No access will be required or requested to sensitive patient information.

6. Risk Control Contractors may be working in areas of the facility that are under construction. Contractors will be working in or near patient care areas requiring them to be cognizant of clinical staff and patient movements. Contractors shall have no patient contact or access to sensitive patient information. An APS will be available for risk intervention should the need arise.

7. Place of Performance

Work will be performed at the Durham VA Medical Center, 508 Fulton Street, Durham, NC 27705 in Building 1.

8. Period of Performance The contractor shall have 120 days from the notice to proceed to complete all work necessary for the set-up/installation of the system. Training shall be coordinated after contract award.

C.3 VAAR 852.211-70 SERVICE DATA MANUALS (NOV 1984) ALTERNATE I (JAN 2008)

(a) The successful bidder will supply operation/maintenance (service data) manuals with each piece of equipment in the quantity specified in the solicitation and resulting purchase order. As a minimum, the manual(s) shall be bound and equivalent to the manual(s) provided the manufacturer's designated field service representative as well as comply with all the requirements in paragraphs (b) through (i) of this clause. Sections, headings and section sequence identified in (b) through (i) of this clause are typical and may vary between manufacturers. Variances in the sections, headings and section sequence, however, do not relieve the manufacturer of his/her responsibility in supplying the technical data called for therein.

(b) Title Page and Front Matter. The title page shall include the equipment nomenclature, model number, effective date of the manual and the manufacturer's name and address. If the manual applies to a particular version of the equipment only, the title page shall also list that equipment's serial number. Front matter shall consist of the Table of Contents, List of Tables, List of Illustrations and a frontispiece (photograph or line drawing) depicting the equipment.

(c) Section I, General Description. This section shall provide a generalized description of the equipment or devices and shall describe its purpose or intended use. Included in this section will be a table listing all pertinent equipment specifications, power requirements, environmental limitations and physical dimensions.

(d) Section II, Installation. Section II shall provide pertinent installation information. It shall list all input and output connectors using applicable reference designators and functional names as they appear on the equipment. Included in this listing will be a brief description of the function of each connector along with the connector type. Instructions shall be provided as to the recommended method of repacking the equipment for shipment (packing material, labeling, etc.).

(e) Section III, Operation. Section III will fully describe the operation of the equipment and shall include a listing of each control with a brief description of its function and step-by-step procedures for each operating mode. Procedures will use the control(s) nomenclature as it appears on the equipment and will be keyed to one or more illustrations of the equipment. Operating procedures will include any preoperational checks, calibration adjustments and operation tests. Notes, cautions and warnings shall be set off from the text body so they may easily be recognizable and will draw the attention of the reader. Illustrations should be used wherever possible depicting equipment connections for test, calibration, patient monitoring and measurements. For large, complex and/or highly versatile equipment capable of many operating modes and in other instances where the Operation Section is quite large, operational information may be bound separately in the form of an Operators Manual. The providing of a separate Operators manual does not relieve the supplier of his responsibility for providing the minimum acceptable maintenance data specified herein. When applicable, flow charts and narrative descriptions of software shall be provided. If programming is either built-in and/or user modifiable, a complete software listing shall be supplied. Equipment items with software packages shall also include diagnostic routines and sample outputs. Submission information shall be given in the Maintenance Section to identify equipment malfunctions that are software related.

(f) Section IV, Principles of Operation. This section shall describe in narrative form the principles of operation of the equipment. Circuitry shall be discussed in sufficient detail to be understood by technicians and engineers who possess a working knowledge of electronics and a general familiarity with the overall application of the devices. The circuit descriptions should start at the overall equipment level and proceed to more detailed circuit descriptions. The overall description shall be keyed to a functional block diagram of the equipment. Circuit descriptions shall be keyed to schematic diagrams discussed in paragraph (i) below. It is recommended that for complex or special circuits, simplified schematics should be included in this section.

(g) Section V, Maintenance. The maintenance section shall contain a list of recommended test equipment, special tools, preventive maintenance instructions and corrective information. The list of test equipment shall be that recommended by the manufacturer and shall be designated by manufacturer and model number. Special tools are those items not commercially available or those that are designed specifically for the equipment being supplied. Sufficient data will be provided to enable their purchase by the Department of Veterans Affairs. Preventive maintenance instructions shall consist of those recommended by the manufacturer to preclude unnecessary failures. Procedures and the recommended frequency of performance shall be included for visual inspection, cleaning, lubricating, mechanical adjustments and circuit calibration. Corrective maintenance shall consist of the data necessary to troubleshoot and rectify a problem and shall include procedures for realigning and testing the equipment. Troubleshooting shall include either a list of test points with the applicable voltage levels or waveforms that would be present under a certain prescribed set of conditions, a troubleshooting chart listing the symptom, probable cause and remedy, or a narrative containing sufficient data to enable a test technician or electronics engineer to determine and locate the probable cause of malfunction. Data shall also be provided describing the preferred method of repairing or replacing discrete components mounted on printed circuit boards or located in areas where special steps must be followed to disassemble the equipment. Procedures shall be included to realign and test the equipment at the completion of repairs and to restore it to its original operating condition. These procedures shall be supported by the necessary waveforms and voltage levels, and data for selecting matched components. Diagrams, either photographic or line, shall show the location of printed circuit board mounted components.

(h) Section VI, Replacement Parts List. The replacement parts list shall list, in alphanumeric order, all electrical/electronic, mechanical and pneumatic components, their description, value and tolerance, true manufacturer and manufacturers' part number.

(i) Section VII, Drawings. Wiring and schematic diagrams shall be included. The drawings will depict the circuitry using standard symbols and shall include the reference designations and component values or type designators. Drawings shall be clear and legible and shall not be engineering or productions sketches.

(j) Initial purchase. The contractor agrees, when requested by the contracting officer, to furnish not more than three copies of the technical documentation required by paragraph 852.211-70(a) to the Service and Reclamation Division, Hines, IL. In addition, the contractor agrees to furnish two additional copies of the technical documentation required by 852.211-70(a) with each piece of equipment sold as a result of the invitation for bid or request for proposal.

(End of Clause) C.4 VAAR 852.219-10 VA NOTICE OF TOTAL SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS SET-ASIDE (JUL 2016)(DEVIATION)

(a) Definition. For the Department of Veterans Affairs, “Service-disabled veteran-owned small business concern or SDVSOB”:

(1) Means a small business concern:

(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans or eligible surviving spouses (see VAAR 802.201 Surviving Spouse definition);

(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans (or eligible surviving spouses) or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran;

(iii) The business meets Federal small business size standards for the applicable North American Industry Classification System (NAICS) code identified in the solicitation document;

(2) “Service-disabled veteran” means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).

(b) General.

(1) Offers are solicited only from verified service-disabled veteran-owned small business concerns. Offers received from concerns that are not verified service-disabled veteran-owned small business concerns shall not be considered.

(2) Any award resulting from this solicitation shall be made to a verified service-disabled veteran-owned small business concern.

(c) Agreement. A service-disabled veteran-owned small business concern agrees that in the performance of the contract, the concern will comply with the limitation on subcontracting requirements in 13 CFR §125.6.

(d) A joint venture may be considered a service-disabled veteran owned small business concern if the joint venture complies with the requirements in 13 CFR 125.15, provided that any reference therein to SDVO SBC is to be construed to apply to a VA verified SDVOSB as appropriate.

C.5 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUL 2018)

This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. C.6 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2012)

(a) Definitions. As used in this clause—

(1) Contract financing payment has the meaning given in FAR 32.001.

(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).

(3) Electronic form means an automated system transmitting information electronically according to the Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.

(4) Invoice payment has the meaning given in FAR 32.001.

(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.

(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.

(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:

(1) VA’s Electronic Invoice Presentment and Payment System. (See Web site at http://www.fsc.va.gov/einvoice.asp.)

(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI). The X12 EDI Web site (http://www.x12.org) includes additional information on EDI 810 and 811 formats.

(d) Invoice requirements. Invoices shall comply with FAR 32.905.

(e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for:

(1) Awards made to foreign vendors for work performed outside the United States;

(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;

(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;

(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or

(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.7 VAAR 852.246-70 GUARANTEE (JAN 2008)

The contractor guarantees the equipment against defective material, workmanship and performance for a period of 365 days, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States, or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.
