B.3 PERFORMANCE WORK STATEMENT (PWS)

I REQUIREMENTS

1.0 INTRODUCTION

The purpose of this contract is to furnish the, Department of Veterans Affairs, Hunter Holmes McGuire Veterans Affairs Medical Center with the services required for Boiler Maintenance/Inspection.

2.0 GENERAL

In accordance with the terms and conditions stated herein, the contractor shall provide all parts, labor, personnel, equipment, tools, supervision and other items and services necessary to perform Boiler Maintenance/Inspection at Hunter Holmes McGuire Veterans Affairs Medical Center (VAMC).

3.0 SPECIFIC TASKS

The Contractor shall provide the following services:

3.1 Assess the external parts of the boilers and boiler accessories and piping and valves for safety, accessibility, proper maintenance and operation, cleanliness, and compliance with codes.

3.2 Inspect for evidence of leakage of combustion gases or fluids, externally and internally, including all gaskets.

3.3 Inspect foundations for signs of stress such as cracking or movement.

3.4 Inspect boilers externally and internally for defects including bulges, blisters, cracks, wasted or eroded material, warping, general corrosion, grooving and pitting, damaged insulation.

3.5 Inspect all accessible internal surfaces for waterside and fireside deposits, including scaling, sediment, debris, carbon deposits.

3.6 Examine all stays and stayed plates.

3.7 Inspect gas side baffles.

3.8 Inspect internal drum surfaces and steam separating internals.

3.9 Inspect tubes, tube ends and tube alignment. Check for sagging tubes.

3.10 Verify that all nozzles of internal piping such as feedwater admission and bottom blow-off collection are clear with no obstructions.

3.11 Inspect seating surfaces of manways and hand-holes.

3.12 Check that boiler structure is plumb and level with no excessive deformations.

3.13 Check for missing items such as bolts and nuts.

3.14 Examine all valve and pipe connections to the pressure vessel.

3.15 Inspect refractory including burner throat, expansion joint, bull ring, furnace seals, furnace walls and floor.

3.16 Verify proper alignment of burner assembly.

3.17 Inspect furnace for evidence of flame impingement.

3.18 Test operation of water level gage glass.

3.19 Perform mechanical and electrical inspection of disassembled low water cutouts (disassembly by VA). After reinstallation, test operation to the extent possible. Test low water cutouts on hot boilers by lowering water level gradually.

3.20 Verify that steam pressure gage is properly calibrated.

3.21 Test operation of high steam pressure cutouts on hot boilers by raising steam pressure.

3.22 Inspect piping connected to the boilers for evidence of leakage, provision for expansion, provision of adequate support, proper alignment, evidence of detrimental conditions.

3.23 Inspect piping to the water column, low water cutoffs and alarms, gage glass.

3.24 Verify that interior of piping is clear of obstructions.

3.25 Inspect valves on Boilers, Feedwater, Blowdown, Drains and Steam Systems.

3.26 Perform lock out tag out on all incoming energy sources.

3.27 Open the boiler.

3.28 Inspect boiler for any broken welds in the steam drum. Note: If any cracks are found, there will be an additional charge for the repair and filing of R-1 paperwork with the Commonwealth of Virginia.

3.29 Inspect and point up any necessary refractory.

3.30 Clean the boiler and prepare for inspection.

3.31 After inspection, install new customer supplied waterside gaskets.

3.32 Close the boiler and prepare to fire.

3.33 Remove existing safety valve and replace with customer supplied, certified safety valve.

3.34 Have removed safety valve recertified.

3.35 Re-install newly certified safety valve.

3.36 Perform final project walk-down with on-site supervisor to ensure customer satisfaction.

3.37 Check in and out through the proper channels upon arrival and departure.

3.38 Ensure Mechanical Room work area is as clean, or cleaner than when they arrived, after project completion.

3.39 This contract will provide for a contingency cost for two (2) certified welders for four (4) days to make any necessary R-1 welding repairs.

3.40 Vendor will file all R-1 paperwork with the Commonwealth of Virginia for the R-1 repairs performed.

3.41 Vendor will coordinate with R-1 Inspector for any onsite certification that is necessary.

3.42 Submit repair plan for review by the VA Hospital prior to any work being performed.

3.43 Provide a detailed service report to the maintenance staff once repairs are completed.

4.0 DEFINITIONS/ACRONYMS

A.Biomedical Engineering - Supervisor or designee.
B.CO - Contracting Officer
C.COR - Contracting Officer's Representative
D.PM - Preventive Maintenance Services, which are periodic in nature and are, required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative conditions.
E.FSE - Field Service Engineer, a person who is authorized by the contractor to perform maintenance (corrective and/or preventive) services on the RICHMOND VAMC premises.
F.ESR - Vendor Engineering Service Report, a documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract.
G.Acceptance Signature - VA employee who is authorized to sign-off on the ESR which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status.
H.Authorization Signature - COR signature; indicates COR accepts work status as stated in ESR.

J.OEM – Original Equipment Manufacture

5.0 PARTS-ITEMS

The contractor shall furnish parts. The contractor shall maintain quantity of parts including unique and/or high mortality replacement parts in such a manner as to be available for shipment within 24 consecutive hours of request. All parts supplied shall be compatible with existing equipment. Contract parts coverage is to include all system options, third party pass thru devices, such as, compact disk devices, remote workstations, etc. and will include all manufacturer supplied software including that used by third party devices. The contractor shall use OEM new or rebuilt parts. All parts shall perform identically to the original equipment specifications.

6.0 REPORTING REQUIREMENTS

Contractor shall be required to report to Engineering to check in. This check in is mandatory. The FSE shall check out with Engineering and submit any reports to Engineering. Must report to Safety Department to get the confined space permit or Hot Work permit.

7.0 TEST EQUIPMENT

Prior to commencement of work on this contract, Contractor shall provide the RICHMOND VAMC with a copy of the current calibration certification of all test equipment to be used by the Contractor on RICHMOND VAMC equipment. This certification shall also be provided on a periodic basis when requested by the RICHMOND VAMC. Test equipment calibration shall be traceable to a national standard.

8.0 EQUIPMENT MODIFICATIONS/UPGRADES/UPDATES:

8.1 Equipment modifications are divided into two kinds; 1) Normal functional improvements that assure that the equipment is operating at the peak of performance and 2) Enhancing improvements are optional improvements that change the equipment’s original design to allow for supplementary or additional functions.

8.2 All modifications, upgrades, updates, enhancements, etc., must be scheduled in advance with the COR. CO and COR must receive notice at least five (5) full business days in advance before any equipment updates, upgrades, enhancements, and modifications are begun along with justification (if Vendor-suggested modification), estimated costs (if applicable), probable effect on equipment operation, and needs for new or additional in-service training.

II SPECIAL CONTRACT REQUIREMENTS

1.0 HOURS OF OPERATION

The services covered by this contract shall be furnished by the contractor as defined herein. The contractor shall not be required, except in case of emergency, to furnish such services on Federal Holidays or during off duty hours as described below. Emergent repair should be provided on-site within 24 hours.

Normal working hours are Monday through Friday, 8:00am – 5:00pm, excluding federal holidays which are as follows:

Any other day specifically declared by the President of the United States to be a federal holiday. When one of the holidays falls on Sunday, the following Monday shall be observed as a Federal Holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday.

2.0 BACKGROUND INVESTIGATIONS

A full background check is not required for this position; however, all contract employees must bring photo identification and advance notice from the requiring service to obtain a VA Badge in order to work on a VA Facility. The Richmond VA Police will issue an ID badge, which needs to be presented by contractor’s employee at the VA entrance security check points.

3.0 BILLING AND PAYMENT

The contractor shall submit payment requests in electronic form via VA’s Electronic Invoice Presentment and Payment System http://www.fsc.va.gov/fsc/einvoice.asp.

Invoices shall be submitted monthly in arrears. The following information must be included on all submitted invoices. Invoices submitted without the following information may be rejected for payment. Invoices shall include at a minimum:

4.0 CONTRACT ADMINISTRATION

Notwithstanding the Contractor's responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor.

The Contracting Officer is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that shall affect price, quantity or quality of performance of this contract.

In the event the contractor effects any such change at the direction of any person other than the contracting officer without authority, no adjustment shall be made in the contract price to cover an increase in costs incurred as a result thereof.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract expiration.

C.3 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days of contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years, and 6 months.

The Contractor shall not make reference in its commercial advertising to Department of Veterans Affairs contracts in a manner that states or implies the Department of Veterans Affairs approves or endorses the Contractor’s products or services or considers the Contractor’s products or services superior to other products or services.

C.5 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2012)

(a) Definitions. As used in this clause—

(1) Contract financing payment has the meaning given in FAR 32.001.

(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).

(3) Electronic form means an automated system transmitting information electronically according to the Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.

(4) Invoice payment has the meaning given in FAR 32.001.

(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.

(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.

(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:

(1) VA’s Electronic Invoice Presentment and Payment System. (See Web site at http://www.fsc.va.gov/einvoice.asp.)

(d) Invoice requirements. Invoices shall comply with FAR 32.905.

(e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for:

(1) Awards made to foreign vendors for work performed outside the United States;

(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;

(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;

(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or

(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of Virginia. (d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

SECTION E - SOLICITATION PROVISIONS

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.

E.1 INSTRUCTIONS TO OFFERORS

1.0 OFFER SUBMISSION

Offerors shall submit their offer via email to Stephanie Patterson at stephanie.patterson2@va.gov by 2:00PM, EST, September 21, 2018. Offers shall be assembled by the following Sections; each section shall be easily identified.

Section A - Offer Form (Standard Form 1449) Signed and dated, include the North American Industry Classification System (NAICS) code and small business size standard in Block 10. Data Universal Numbering System (DUNS) Number shall be entered in the block with the Offerors name and address.

Section B – Pricing Pricing shall be submitted as requested in the Schedule of Services. Total price will be evaluated by the Government.

Section C– Offerors Representations and Certifications A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/.

All Offerors shall be registered and current in the System for Award Management (SAM) database at time of award.

All Offerors MUST be a verified Veteran Owned Small Business (VOSB) vendor. To be considered a VOSB, the company must be listed as verified in the vendor information pages in the VIP database, https://vip.vetbiz.gov .

Section D – Technical/Management Approach Offerors shall submit a quote that addresses all aspects of the Performance Work Statement (PWS).

The offeror shall identify any subcontractors proposed under this solicitation. All technical elements applicable to the utilization of subcontractors must be addressed in the technical quote and detail provided.

The technical quote should be as detailed as necessary to fully explain the proposed methods and rationale; it should reflect a clear understanding of the nature of the work being undertaken. The technical quote shall include information on how the project is to be organized, staffed, and managed.

The Technical Volume shall not exceed twenty-five (25) pages in length (minimum 12-point font and no less than ½ inch margins). Page limitation includes any drawings, charts, images, etc., and excludes section dividers, table of contents, glossary of terms, cross-referencing indices, and resumes. If more than 25 pages (not counting excluded pages) are submitted, only the first 25 pages will be considered.

Section E – Past Performance Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last three years. List the following information for each contract:

· Company Name and Address

· Description of Services Performed

· Name, Telephone Number and Email Address of Responsible individuals who have first-hand knowledge of performance relative to similar type of services

· Dates of Contract Performance

· Contract Type (e.g. fixed-price, cost reimbursable) and total contract value

Failure to submit a complete proposal may result in a technically unacceptable determination.

2.0 AWARD BASIS

The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for quote, will be most advantageous to the Government, price and other factors considered. A Lowest Price Technically Acceptable award will be made based upon the following factors: Technical Approach, Past Performance, and Price. If any quote’s technical approach or past performance is determined to be unacceptable, the entire quote will be considered unacceptable.

Upon receipt of offers, the Contracting Officer will record the offers from low price to high on the Abstract of Offers. Once offers are recorded, the CO will submit the lowest priced offer to the technical evaluation team for evaluation. If the lowest priced offer is determined acceptable, the CO will make award. If the lowest priced offer is determined unacceptable, the CO will forward the next lowest priced offer to the technical evaluation team. This process will continue until the lowest priced technically acceptable offer is found and selection is made.

· The offer shall provide references from three (3) current service users.

The Technical Rating shall be rated based on Table A below:

Factor 2: Past Performance Past performance information will be utilized to determine the quality of the contractor’s past performance as it relates to the probability of success of the required effort. The Government will evaluate customer satisfaction, responsiveness to customer needs, and past demonstration of meeting delivery schedules and the delivery of quality services. Emphasis will be on recent, relevant past performance in the previous 3 years. Recent is defined as work performed within the last three years. Relevant is defined as work similar in size and scope of the work described in the Performance Work Statement (PWS).

Past performance information is one indicator of an offeror’s ability to perform the contract successfully. Current and relevant information, source of information, context of data, and general trends in Contractor’s performance shall be considered as it pertains to the performance of work described in this solicitation. Offerors may provide information on problems encountered on identified contracts and the offeror’s corrective action. The Government shall consider this information as well as information obtained from any other sources when evaluating the offeror’s past performance.

Since the Government may not necessarily interview all of the sources provided by the offerors, it is incumbent upon the offeror to explain the relevance of the data provided. Offerors are reminded that the Government may elect to consider data obtained from other sources.

Offerors with no relevant past performance or whom information is not available, shall not be evaluated favorably or unfavorably on past performance, but may be considered less favorably than an offeror with favorable performance history. The Past Performance Rating shall be based on Table B below:
