Justification for Brand Name IAW FAR 13.106-1 (b)(1)(i) For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K)

Acquisition Plan Action ID: VA246-18-AP-10244

1. Contracting Activity: Department of Veterans Affairs, VISN 6, Durham Veterans Affairs Medical

Center, 508 Fulton Street, Durham, NC 27705. 558-18-3-122-0201

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This is a

Brand Name Only requirement for Optos Widefield California System. It’s the standard of care for

Durham VAMC which are currently provided by Durham physical using Duke Eye Center. There is no widefield imaging capability at the Durham VA Eye Clinic even though there are about 30,000 patient visits annually.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Durham physicals has been using widefield imaging (color fundus photos, fluorescein angiography, indocyanine green angiography, fundus autofluorescence) for 5 years.

Many diagnoses and treatment paradigms are based on the findings from widefield imaging and without it, we are practicing outdate ophthalmologic care of the Veterans who come to this eye clinic. The Optos devices are the only imaging systems that capture non-mydriatic 200-degree field of view through 2mm pupils, showing up to 82% of the retina in a single image or non-montaged image.

4. Description of market research conducted and results or statement why it was not conducted:

In accordance with FAR 8.002 Priorities for Use of Mandatory Government Sources and VAAR

808.002 Priorities for Use of Government Supply Sources. VIP searched using “OPTOS WIDE FIELD CALIFORNIA SYSTEM” revealed 64 SDVOSBs and 13 VOSBs; they were all emailed July 21-23, 2018, 4:30pm. A source sought was posted by Jacqueline Ambala on FBO May 18, 2018, with a closing date May 23, 2018 at 11:00am
