1. Is this a new requirement or is an incumbent in place? If so, who is the incumbent contractor and contract number?

The current contractor is Maxim Healthcare Services. The contract number is 36C24520F0398.

1. What is the estimated start date for this opportunity?

The estimated start date is April 1st, 2021.

1. How many perfusionist are currently on staff?

Currently two.

1. What is the current hospital bed count?

1. How many procedures are the providers expected to complete in day/ 8-hour work period?

1. Can offers provide proof of insurance at time of award?

Proof of insurance must be with your proposal submittal.

1. Please provide the number of anticipated cases per year requiring perfusionists’ services, not just the number of hours.

Perfusion covers 110-130 open heart cases; and eventually back up to 40-60 structural heart cases once moved to VAMC; as well as medical center coverage to cell saver and Critical Care plans to expand to an ECMO program.

1. Please provide the number of concurrent cases per year requiring perfusionists’ services.

As above=total 200 coverage cases anticipated; includes weekend, take backs, and emergency cases.

1. The proposal specifies separate PDF files, one for each volume. May offerors provide major sections of a volume in separate PDFs if the PDFs are clearly named?

Any sections you provide are to be included in the two volumes only. Separate PDFs will not be accepted.

1. Does the government desire past performance contract descriptions for all relevant contracts of the offeror (company – prime/subcontractor), all relevant contracts of each key personnel, or both?

The offerors past performance for all relevant contracts is required.

1. Does the government require past performance surveys be sent out only for each of the key personnel (as stated on solicitation page 79), or the offeror/company (as implied in the attachment D.5 instructions), or both?

The past performance surveys should be sent for the offeror/company.

1. Regarding the survey form, it seems most appropriate for a contractor/company more than individual key personnel. For example, it asks “Q3. To what extent did the contractor use appropriate personnel for contract requirements?” If the form is to be used for key personnel, should it be revised or tailored to an individual’s review (instead of a company’s)?

See answer 12.

1. The proposal content instructions for Volume I list section numbers (e.g., B.5, D.1, and D.2) that are incomplete and/or different from the actual attachments (e.g. B.4, D.2, and D.3, respectively). Can offerors assume that actual section or attachment numbers take precedence?

The correct section number is B.4/Price/Cost Schedule.

The correct attachment numbers are D.2 Contractor Rules of Behavior & D.3 Organizational Conflict of Interest.

1. Should Section C and Section E.8 be included in Volume I?

Sections A, B, C, D, & E should be included in Volume I. The price should also be included in Volume I.

1. Will the Technical Capability section/factor be evaluated primarily for completeness and compliance? Or will other more qualitative factors apply?

Technical Capability will be evaluated as stated in the RFP.

1. What qualitative factors will be applied to the Past Performance Section/factor, if any?

Past Performance will be evaluated as stated in the RFP.

1. Is it necessary to submit signed copies of the Contractor Rules of Behavior with the proposal, or is this a certification that will be required after award?

A signed Contractor Rules of Behavior must be submitted with your proposal.

1. The formatting instructions on solicitation page 77 say: “For Volume II, only the 1st page of Volume II shall identify the company name, date and solicitation number in the header and/or footer. No other identifying information should be submitted within Volume II. Submitting any other identifying information within Volume II except for on the 1st page, might deem your proposal unacceptable.”

To fully comply with the above requirement, and to provide all the information requested in Volume II, should an offeror (a) remove, (b) mask/black out, or (c) substitute “Contractor” or “Subcontractor” for its actual company name(s) from the following:

Resumes (where personnel work for the offeror firm) Key personnel references Quality Control Plan Contingency Plan Proof of Indemnification and Medical Liability Insurance Past Performance – Listing and descriptions of ALL relevant contracts, and Program management process and organizational structure (requested on solicitation Pg 78).

You may remove, mask/black out, substitute “Contractor” or “Subcontractor” or whatever you choose to do for your actual company name. I only want to see your company’s name on the 1st page.

1. Please confirm that the Contractor will be allowed to bill consistently for 4,160 hours per week.

It is 4,160 hours per year.

1. In line items 0004, 1004, and 2004, please clarify whether “128” is 128 hours per week or 128 hours per year.

It is 128 hours per year.

1. Several places in the RFP refer to a Contractor’s physician. Please confirm that the RFP should refer instead to Contractor’s perfusionist instead.

It should read contractor’s perfusionist.

1. Will the attachments be provided separately?
