1. Contracting Activity: Department of Veterans Affairs, VISN 05, VA Maryland Health Care System.
Fixed Firm Price 2237: 512-18-3-042-0376
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
This requirement is for eight (8) non-invasive cardiac output monitors for the operating rooms and Intensive Care Units consistent with the current facility standard.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
These devices are proprietary medical devices regulated by the FDA. Only the manufacturer, Edward Life Sciences, or an authorized distributer is able to meet this requirement.
4. Description of market research conducted and results or statement why it was not conducted:
Review of product catalogs, conversation with vendor representative, and review of ECRI and MD Buyline databases.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
