Acquisition Plan Action ID: 36C244-19-AP-5031

1. Contracting Activity: Department of Veterans Affairs, VISN 04, Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042. PR#: 595-19-4-480-0157.

2. Description of Action: The Lebanon VAMC has a new brand name requirement for a Steris brand V-PRO MAX Single Door Cabinet 208-230V to be used for the sterilization of medical instruments. The commodity is available in VA Federal Supply scheduled.

3. Description of Supplies or Services:

The estimated value of the proposed action is $ 134,259.70.

The Lebanon VA Medical Center has an urgent need to replace its low temperature sterilizer due to ongoing system failures and need for large capacity, fully functioning sterilizer to maintain the increasing needs within SPS. The Lebanon VAMC will be opening a new SPS area and the equipment purchased must be compatible to the items already procured. The need is for a V-PRO MAX Single Door Cabinet 208-230V (English).

This unit is compatible with items already procured.

The supplies require for the acquisition are; one (1) V-PRO MAX Single Door Cabinet 208- 230V (English), one (1) Celerity 20 HP, Celerity 20 HP Challenge Pack (5 Test Packs & 5 Controls), and Operator and Service Manuals.

4. Authority:

ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception:

An urgent and compelling need exists, and following the procedures would result in unacceptable delays (FAR 8.405-6(a)(1)(i)(A);

Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized;

(FAR 8.405-6(a)(1)(i)(B));

In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures (FAR 8.405- 6(a)(1)(i)(C)); or

Items peculiar to one manufacturer. An item peculiar to one manufacturer can be a particular brand name, product, or a feature of a product, peculiar to one manufacturer). A brand name item, whether available on one or more schedule contracts, is an item peculiar to one manufacturer (FAR 8.405-6(b)).

5. Rational/explanation to support cited authority:

The V-PRO MAX maximizes devices per load with the most productive cycle claims and the ability to have mixed loads. The V-PRO MAX has the ability to process more stainless-steel lumens than competing vaporized hydrogen peroxide sterilizers in the Lumen Cycle (up to 20). The V-PRO MAX provides flexibility in load contents with the ability to process dual channel flexible scopes and dual and triple channel rigid scopes.

The V-PRO MAX also increases the speed to process devices, and use less sterilant, with the new Fast and Fast Non Lumen Cycles - both exclusive to V-PRO Sterilizers.

The Steris V-PRO MAX is the only sterilizer that meets the medical center’s performance needs and is compatible with the new sterilization equipment and components in the new SPS area.

6. BRAND NAME ONLY: This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-1(e).

7. Note that this determination satisfies the requirements of FAR 8.405-6. The requirement to prepare a written justification IAW FAR 6.303 does not apply to Federal Supply Schedules.
