STATEMENT OF WORK

Wilkes Barre VAMC

Allentown OPC and Sayre OPC Air Balancing

1. General:

This requirement is for air balancing and certification of pressure monitors/alarms at the Wilkes Barre VAMC (1111 East End Blvd, Wilkes-Barre, PA), Allentown VA Out Patient Clinic (3110 Hamilton Blvd, Allentown, PA 18103) and Sayre VA Out Patient Clinic (1537 Elmira St, Sayre, PA 18840) to meet VA and industry requirements for infection control.

2. Testing, Certifications and Report Requirements:

The contractor shall have a current National Environmental Balancing Bureau (NEBB) certification and all final test, adjust, balance (TAB) reports shall be stamped and signed with NEBB seal. The contactor shall provide an out brief to the Contracting Officer’s Representative (COR) to identify any preliminary (draft) findings of failures. The contractor shall have two weeks to submit the signed and NEBB sealed final report in electronically in PDF. The report will be certified by the testing, adjusting, and balancing supervisor with the NEBB. The electronic report shall be emailed to the WBVAMC safety group email address (VHAWBPSafetyOffice@va.gov).

The report shall have at least the following;

- Executive summary providing explanation of findings, reporting if equipment does or does not meet the requirements of the specifications.

- Pressure differential readings using a micro-manometer, with a certificate traceable to the National Institute of Standards and Technology. Certificate provided in report annex.

- Measurements for Relative Humidity (%RH) and Temperature (°F) should also be included in the report for each room. This instrument reads differential pressure to .0001" w. c. and is accurate to +/-2% of reading.

- Diagram of each room showing in air supply, return or exhaust, doorways and provide CFM or pressure differential.

- Air Changes per hour for each space or room

Contractor shall calibrate any wall mounted room pressure monitors and verification of the accuracy of unit at the three sites. Calibrated pressure monitors shall be marked with a sticker indicating the date tested, retest date (one year from the calibration) and initials of the testing official.

The contractor’s field personnel shall have training on fundamental practices and precautions hospital infection control procedures. (e.g. gowning, gloving, and hand washing procedures)

3. Work at Wilkes Barre Veteran Affairs Medical Center

Work at the main facility shall be coordinated with the COR and the safety office. There are approximately 90 rooms or spaces that need to be tested on varying frequencies.

There are 21 rooms that shall be tested quarterly, 30 rooms that shall be tested semiannually, and 39 spaces/rooms that shall be tested annually. Attachment 1 provides room number, area description, calibration/test interval, pressure requirement, air changes per hour and notes. Attachment 1 refers to two drawings showing areas (entire OR and Pharmacy) that will be tested annually primarily for pressure differentials to adjacent spaces. The reports associated with this section shall be constant with items identified in section 2. The onsite hours of work are from 7:00 AM to 5:00 PM. Any work determined by the contracting officer’s representative (COR) as having a significant impact on the operations of the facility(s) shall be conducted on an after-hours schedule or limited hour’s schedule.

4. Work at Allentown Out-Patient Clinic

Work at the Allentown Out-Patient Clinic located at 3110 Hamilton Blvd, Allentown, PA 18103 consists of testing rooms 138, 144 and calibrating/certifying their wall mounted differential pressure monitors. The reports associated with this section shall be constant with items identified in section 2. This facility is a leased and serviceman must report to the Administrative Officer or designee at each facility; Karen Leshko prior to service/calibration. The onsite hours of work are from 8:00 AM to 3:00 PM.

5. Work Sayre Out-Patient Clinic

Work at the Sayre Out-Patient Clinic located at 1537 Elmira St, Sayre, PA 18840 consists of testing rooms 139, 142 and calibrating/certifying their wall mounted differential pressure monitors. The reports associated with this section shall be constant with items identified in section 2. This facility is a leased and serviceman must report to the Administrative Officer or designee at each facility; Dawn McCutcheon at Sayre prior to service/calibration. The onsite hours of work are from 8:00 AM to 3:00 PM.

6. Period of Performance

The resultant contract will be for a 12-month base period with four 12-month option years.
