Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The Department of Veterans Affairs, Network Contracting Office 2, and the VA Medical Center in Canandaigua, NY anticipate a new, single manufacturer (brand name only per FAR 6.302-1(c) firm fixed price procurement action for 57 units of a Welch Allyn Series 777 Wall Diagnostic System.

The new Department of Veterans Affairs Rochester (NY) Outpatient Clinic is scheduled to fully open April 2019. The Rochester VA facility has a requirement to procure new equipment for the exam rooms. The Canandaigua and Bath VA Medical Centers have integrated to what is known as the VA Finger Lakes Health Care System. With that, the expectation is medical providers may work at any of the facilities within the region to include Bath, Canandaigua and/or Rochester. To ensure continuity in care, all facilities within the region must standardize medical equipment.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
This is a brand name specific make and model request. The Canandaigua and Bath VA Medical have integrated to what is known as the VA Finger Lakes Health Care System. With that, the expectation is medical providers and other staff may work at any of the facilities within the region to include Bath, Canandaigua and/or Rochester, as well as three (3) other Community Based Outpatient Clinics (CBOC). To ensure continuity in care, all facilities within the region must standardize medical equipment. The equipment purchased for the new VA Rochester Outpatient Clinic must be consistent with equipment currently in use at the primary facilities and COBCs.

4. Description of market research conducted and results or statement why it was not conducted:
A comprehensive search was conducted on the Veterans Information Pages (VIP), which resulted in four (4) vendors found to offer Welch Allyn Products. Additionally, the regional Welch Allyn representative stated that there are four (4) SDVOSB authorized resellers of the required equipment.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
