COMBINED SYNOPSIS/SOLICITATION

(3) Zeiss Cirrus HD-OCT (Model 5000) VA Hudson Valley Health Care System – Montrose, NY

(i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued.

(ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24218Q9663.

(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95.

(iv) This procurement is being issued as 100% set-aside for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSB). It is the Government’s intent to issue a firm fixed price, single award as a result of this solicitation. The North American Industry Classification System (NAICS) code is 339115, Ophthalmic Goods Manufacturing, with a small business size standard of 1,000 employees. This solicitation is 100% set-aside for SDVOSBs.

(v) The Contractor shall provide all parts and labor necessary to deliver three (3) Zeiss Cirrus HD-OCT (Model 5000) with GCGPA, Smart HD Scans, FastTrac, that meet the brand name or equal specifications as identified below. The requested products shall be delivered in new and unused/unopened condition.

(vi) Description of requirement:

The VA Hudson Valley Health Care System – Montrose, NY has a requirement for:

Three (3) Zeiss Cirrus HD-OCT (Model 5000) machines that possess the following characteristics of:

· The Cirrus HD-OCT units must be the standard floor model and must be shipped with software version 9.5, include GCGPA, Smaret HD Scans, Fast Trac, Advanced RPE, GCA and Basic Anterior Segment.

· The unit must have exact Macular Scan Pattern of 512x128 for retina scan with exact scanning speed at 27,000 A-scans a second within a 6mmx6mm.

· The unit must have exact scan pattern of 200x200 in a 6mmx6mm cube for Glaucoma Scan with exact scanning speed at 27,000 A-scans a second. The Scab pattern must be able to capture and provide RNFL , ONH, Neuro retinal Rim Tissue, and #-D Cube Capabilities all in one scan.

· Scanning Speed: Scan at exactly 27,000 A-Scan/sec, as well as the ability to scan at 68,000 A-scan/sec

· Unit must be able to provide a report for Ganglion Cell Analysis that is derived from a scan must be comprised of 6mmx6mm cube scan with a minimum by 200x200 line scan pattern with no more than 30 microns between each slice.

· Minimum pupil requirement must be 2.0mm or less

· Unit must utilize an LSO Fundus Camera (line Scanning Laser ophthalmoscope).

· The unit must automatically track to prior scan. The unit must be able to do this without technician needing to align to prior scan.

· Anterior segment scan that is at minimal 15.5 mm in length x 5.8 mm in depth. Which is the full view of the anterior chamber. Must be able to provide on company literature.

· The unit must be a 90 degree configuration, so that the tech (operator) must sit to the side (90 degree) of the patient

(vii) EVALUATION: Quotes will be evaluated based upon lowest price technically acceptable. Technical acceptability is based upon the ability to meet the above specifications and past performance.

(x) GOVERNMENT INVOICE ADDRESS: All Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests.
